[Pleomorphic adenoma of the salivary glands. Impact of the myxoid component on the prognosis].

In a series of 570 pleomorphic adenomas of main and accessory salivary glands, we found 48 recurrences (8.04%). These recurrences occurred between several months and 19 years after surgical treatment. They were more frequently noted in parotid tumors. Comparatively to a control series, in recurrent tumors myxoid structures were more frequent than epithelial structures, as demonstrated by immuno-markings (anti-protein S 100 and anti-vimentin sera). Besides, in these tumors, extracapsular myxoid expansions were commonly seen. The proliferative potential of myoepithelial cells which are the main cellular component of myxoid areas, is thus suggested as a mainly factor of recurrence of pleomorphic adenomas.

[Facial circumscribed ossifying myositis. Anatomo-clinical and tomodensitometric study].

1 case of circumscribed myositis ossificans of the temporal muscle is reported in a 21 years-old man. The initial complaint was a sudden non traumatic swelling. Roentgenography showed a characteristic feature: peripheral radio-opacity around a non-opacified area. Under microscope, we could find 3 zones: a central fibroblastic area, an intermediate zone with immature osteoid bone, a peripheral zone with mature lamellar bone. In addition, we used histoenzymological and electron microscopical methods. The difficulties of diagnosis, the pathogenesis and the prognosis of this disease were also discussed.

[Central reparative giant cell granuloma of the jaw. Morphological study].

8 cases of central giant cell reparative granuloma of the jaws were morphologically studied. If the histological feature of this disease is theoretically characteristic (giant cells, fusiform cells, numerous vessels and more or less active osteogenesis) in fact other giant cells lesions, especially true giant cell tumors, often set a problem to the pathologist. In addition, the etiopathogenesis of that granuloma is discussed by means of the results of immunohistochemical, histoenzymological and ultrastructural findings.

[Cystic lymphoid hyperplasia of the parotid in relation to acquired immunodeficiency syndrome (HIV+). Apropos of 2 cases].

Cystic lymphoid hyperplasia of the parotid, a rare disease, was recently described in patients with acquired immunodeficiency syndrome. We reported here 2 new cases of this illness. The 2 HIV+ patients showed an enlargement of parotid gland. A superficial parotidectomy was performed. The histological and immunohistochemical studies were completed in one case by a histoenzymological and ultrastructural study. We could see numerous cystic cavities lined with a cylindrical or metaplastic malpighian epithelium and surrounded by a lymphoid tissue, similar to a true lymph node with germinal centers. In these lymphoid structures, we could find some epithelial remnants and numerous epimyoepithelial islands. Besides, anomalies similar to those described in persistent generalized lymphadenopathies were obvious: hyperplastic germinal centers with multiplication of B lymphocytes and thin mantle zone, perifollicular hypervascularization, numerous macrophages, plasmocytes and T lymphocytes with increase of T8 cytotoxic subset in interfollicular and paracortical areas. An early opportunist infection is suggested in the histogenesis of this disease.

Acquired Immunodeficiency Syndrome

[Cystic hamartoma of the mandibular angle. Apropos of a case].

A large cyst of mandibular angle was surgically removed in a 20 years-old woman. This cyst was discovered during childhood and was associated with a branchial para-mandibular cyst. It was histologically lined either with a malpighian epithelium with sebaceous islands or with a ciliated cylindrical epithelium. Because of its topography, its association with a branchial cyst and its histological feature quite similar to that of a cyst of thyroglossal duct, this cyst was thought to represent a simplified salivary ectopy composed only of a cystic secretory duct without any salivary parenchyma.

[Value of modern imagery in the study of coronoido-malar malformations. Apropos of 2 cases].

We report two cases of mechanical limitation of buccal opening. CT scanning of bone anomalies diagnosed on standard radiography allowed to define malformation of the coronoid piston on the one hand, and the temporo-malar groove on the other: a combination of Langenbeck's and Jacob's disease in one case and Langenbeck's disease in the other. Coronoidectomy, systematically bilateral, demonstrated these morphological anomalies together with formation of coronoido-malar neo-arthrosis.

Cherubism. Histo-enzymological and ultrastructural study.

The 3 cases of cherubism reported affected the mandible. They were all studied by means of histo-enzymological and ultrastructural methods. This study demonstrated 3 stages in the morphological evolution of the disease, corroborated by clinical data. The 1st stage was characterized by an osteolytic granuloma with round, fusiform and giant-cells and a high level of activity of acid phosphatase. The 2nd stage showed repair with proliferation of highly active fibroblasts (increase in activity of leucine aminopeptidase). The 3rd stage exhibited an osteogenesis with high activity of alkaline phosphatase and ATPase. The pathogenesis of this rare osteodysplasia is discussed.

[Genioplasty, indications and therapeutic methods in dento-maxillary malformations].

Genioplasty constitutes an adjunctive surgical procedure used in adult plastic surgery of the facial profile. It is used after dental malocclusion has been corrected and facial morphological features have been reassessed (soft tissue changes consecutive to maxillary osteotomy should be previously analyzed by cephalometry). Genioplasty permits achieving a modification of the chin point in the three planes. The authors describe several types of genioplastic operations, depending on the aim.

[Pleomorphic adenoma. Diagnostic difficulties. Value of ultrastructural and immunohistochemical studies. Apropos of 2 cases].

2 cases of pleomorphic adenoma with unusual histological feature are related. In the 1st case, the tumor was mistaken for a schwannoma. The electron microscopic study only permitted the diagnosis; in the 2nd case, the tumor, obviously malignant, was firstly diagnosed as synovial sarcoma; in this case also, ultrastructural study permitted to recognize a malignant mixed tumor with an unusual "sarcomatous" change of its myoepithelial component.

[Fibrous dysplasia and ossifying fibroma. Morphologic criteria].

Among a series of 42 ossifying fibromas and 32 fibrous dysplasias, 5 cases were studied by histoenzymological and electron microscopic methods. The histological study in 2/3 of cases is sufficient for the diagnosis between the two diseases: trabecular bone in a connective tissue with regular collagen fibres in the ossifying fibroma; nodular indented bony areas with irregular collagen frame in fibrous dysplasia. By histoenzymology, the diagnosis between the two affections is not easy: same activity of ATPases and alkaline phosphatases in fibroblasts and osteoblasts. By electron microscopy, the morphology of the two lesions is different: in ossifying fibroma, numerous well-differentiated osteoblasts and large areas of ossification are seen; in fibrous dysplasia, undifferentiated cells are numerous and the collagen frame is irregularly mineralized. This method is also useful for the histogenetic understanding of these two osteopathies.

[Reconstruction following anterior sulco-mandibulectomy. The advantages and disadvantages of different procedures. Apropos of 85 cases].

A retrospective study was conducted on 85 case-reports of patients with epithelioma of anterior and anterolateral region of floor of mouth operated upon in the Stomatology and Maxillofacial Surgery Clinic of Salpêtrière hospital, Paris. An update review of recent advances in surgery for reconstruction of floor of mouth is completed by results of study of the case-reports showing evolution of ideas related to functional results and to vital prognosis (as function of tumoral stage).

[Malignant xanthogranuloma of the cervicofacial region. Optical and ultrastructural morphology. Apropos of 4 cases].

The 4 cases of malignant xanthogranuloma reported here are peculiar because of their unusual localization in the cervico-facial area and of their good outcome (3 patients are alive without recurrence more than 6 years after chemotherapy and radiotherapy). Besides the histological study, the 4 cases were examined by immunohistochemical methods : vimentin was positive in 20 to 40% of cells and lysozyme in 5 to 15% of cells. In addition, one case was studied by electron microscope which showed the polymorphism of tumoral cells.

[Histiocytosis X of the jaw. I. Anatomico-clinical study apropos of 61 cases].

61 cases of histiocytosis X of jaws were reported here. They occurred more often in man (72% of men) than in woman, with an average age of 27 years. The mandible was affected more frequently than the maxilla. Clinically, dental expulsion, gingival swelling, fractures or pain were the usual symptoms. Roentgenographic examination showed either central or peripheral (alveolar bone) osteolytic areas. The histologic diagnosis was easy when eosinophilic polymorphous leukocytes or histiocytes were preponderant. In the other cases, immunohistochemistry (positivity of S-100 protein) and electron microscopy (presence of Birbeck granules about paraffin embedded specimens) were of a great usefulness. The prognosis of the disease was mainly related to the diffusion of lesions to other skeletal portions or extraskeletal localizations (37% of our cases). It has been improved by combination of chemotherapy to surgery (despite of frequent recurrences), 40 upon 45 patients are well after a follow-up of 2 to 10 years after the last localization.

[Neural tumors of the tongue. An anatomicoclinical study apropos of 13 cases].

During the ten last years, 13 neural tumors of the tongue have been observed in the department of stomatology and maxillo-facial surgery of Salpêtrière Hospital Paris (Prof. J.M. Vaillant): 4 amputation neuromas, 5 benign schwannomas, 1 malignant schwannoma, 2 neurofibromas and 1 multiple endocrine syndrome type III. The authors analyze the clinical and histological data from these case-reports; they point out difficulties encountered in the diagnosis, and the prognostic problems brought up by such infrequent lesions.

[Approach to age determination based on dental morphological criteria obtained in microradiography, stereomicroscopy and scanning electron microscopy].

The determination of the age owing to the morphology of permanent teeth has been done in 14 men and women, using criteria proposed by Gustafson. It was faithful, the difference between age determined by this method and the true age of these patients being never superior to 3 years. The application of scanning electron microscopy allows to improve some of the criteria used for this determination, especially that based upon the dentinal senescence.

[Ameloblastoma in an 11-year-old child].

The authors report a case of ameloblastoma occurring at the unusual age of 11 years. The surgical management is conservative. Complete and careful tumorectomy was performed without reconstruction. Complete spontaneous restitution was seen one year after the operation. No recurrence was seen and the patient may new be considered as cured, with a 6 year follow-up.
