Difference of immunohistochemical reactions in epithelial cells of adenolymphoma.

An immunoperoxidase staining technique was used for detecting alpha one-antichymotrypsin (alpha 1-ACT), alpha one-antitrypsin (alpha 1-AT), lactoferrin and transferrin in routine histological paraffin sections of 30 adenolymphomas, as well as in normal salivary gland tissue. Microscopically, the epithelial, component of adenolymphomas consisted of tall columnar luminal cells and irregularly shaped basal cells. alpha 1-ACT was detected in the luminal layer of epithelium in 27 (90%) of 30 adenolymphomas, while the basal layer was positive in 4 cases (13%). Lactoferrin could be observed in the columnar cells of 21 cases (70%) and was positive in the basal cells of 2 cases (7%). In normal salivary gland tissue, alpha 1-ACT and lactoferrin were observed in the intercalated duct and serous acinar cells. The staining pattern of alpha 1-AT in adenolymphoma was similar to those of alpha 1-ACT and lactoferrin, however, the number of positive cases for alpha 1-AT was fewer than in the alpha 1-ACT and lactoferrin. alpha 1-AT was not found in the normal salivary gland. On the contrary, the localization of transferrin in the epithelial component of adenolymphomas was exclusively different from those of alpha 1-ACT, alpha 1-AT and lactoferrin. Transferrin was found more often in the basal cells than in the tall columnar apical cells. The staining pattern of transferrin in the normal salivary gland was different from alpha 1-ACT and lactoferrin, and transferrin was positive in the cytoplasm of intercalated ducts, serous acinar and myoepithelial cells.(ABSTRACT TRUNCATED AT 250 WORDS)

Intraoperative imprint cytology of the thyroid gland with computer-assisted morphometric analysis of cell clusters.

Imprint preparations were used in addition to frozen sections in the intraoperative diagnosis of 37 cases of benign and malignant lesions of the thyroid gland, including adenomatous goiter, follicular adenoma, follicular carcinoma and papillary carcinoma. In the imprints, the cytologic features specific for carcinoma, as compared with benign lesions, were (1) the folding of the nuclear contour, (2) the increased density of the cytoplasmic matrix and (3) the frequent appearance of cell clusters of larger size. The size and frequency of cell clusters were morphometrically analyzed by a computer image analyzer. There was an increasing number of large clusters, plus the appearance of clusters of more than 300 micron in diameter, in both follicular and papillary carcinoma. In benign lesions, on the contrary, the majority of cells were isolated or in small clusters, the diameter of which never exceeded 300 micron in diameter. These results demonstrate that (1) the imprint cytology of the thyroid gland is useful in making a rapid intraoperative diagnosis and (2) the introduction of computer-assisted quantitative analysis is of practical value in the diagnosis of malignancy.

Gas chromatography-electron impact-mass spectrometry (GC-EI-MS) analysis of saturated fatty acid compositions in formalin-fixed human tissues.

Applicability of the gas chromatography-electron impact-mass spectrometry and data system (GC-EI-MS) for fatty acid analysis of formalin-fixed human tissues was evaluated by comparison of formalin-fixed and unfixed brain tissues, and the results of GC-EI-MS analyses of the formalin-fixed brain and adrenal cortex from cases with adrenoleukodystrophy (ALD) and the cerebro-hepato-renal syndrome of Zellweger (ZWS) were presented. When formalin-fixed and unfixed materials were compared, a reduction of polyunsaturated fatty acids was observed after formalin fixation, while saturated and monounsaturated fatty acids remained unchanged. The chromatographic patterns of brain tissues fixed in 10% formalin for 3 days or 17 months were basically the same. In ALD, increased C26:0/C22:0 ratios in the cerebral white matter (2.05 in ALD versus 0.74 in control) and in the adrenal gland (15.2 in ALD versus 0.1 in control), and the presence of unusual very long chain fatty acids (VLCFA), longer than C29:0, were demonstrated. In the ZWS brain, all of the VLCFAs were severely depleted. Thus, formalin-fixed tissue may be a reliable subject to fatty acid analysis.

Primary extranodal non-Hodgkin's malignant lymphoma of the oral region: analysis of 11 autopsy cases.

The authors describe autopsy cases of primary extranodal non-Hodgkin's lymphoma of the oral regions in 11 (8%) of 134 patients: 10 were clinical stage IE, and one was IIE. Histologic subtypes of each lymphoma included the diffuse large cell type (8 cases), diffuse mixed type (2 cases) and Burkitt's type (1 case) according to the Japanese Lymphoma Study Group (JLSG) classification. All patients received anti-lymphomatous chemotherapy and/or radiotherapy. Nine cases had active disease, 3/9 manifesting leukemia. Nodes were involved. There was involvement of cervical lymph nodes in 7 (64%). Viscera most commonly involved were, in order of decreasing frequency: lung, kidney, pancreas, liver, spleen, heart, gastrointestinal tract, bone marrow, and adrenal gland. It is apparent that primary extranodal non-Hodgkin's lymphomas of the oral region arise in a single focus, and extensions of the disease are not random. The primary lesions spread to regional lymph nodes by way of lymphatic channels, and to extranodal organs of the intrathoracic and intraabdominal regions by contiguous direct invasion of adjacent organs and by blood-borne metastases. Wide spread dissemination at the time of death was found in 6 cases and 3 modes of spreading were observed.

An immunoperoxidase investigation of S-100 protein in the epithelial component of Warthin's tumor.

Using the peroxidase-antiperoxidase (PAP) immunohistochemical method, we studied the epithelial component of Warthin's tumor of the human salivary gland with respect to the presence of S-100-protein-positive cells. The tumor epithelium consisted of two layers of oxyphilic cells (an apical layer of tall columnar cells and a basal layer of small cuboidal cells) and irregularly shaped, dendritic cells scattering throughout the neoplastic epithelial cells. Two types of the oxyphilic cells were devoid of the immunoreaction products for S-100 protein. However, in the epithelial layer of Warthin's tumor, S-100-protein-positive material was observed in the cytoplasm and nuclei of dendritic cells. They were more abundant in the lymphoid tissue than in the neoplastic epithelium. The morphologic appearance of these cells was identical to that of Langerhans' cells, which were absent from the normal human salivary glands. The presence of these cells, which have a special function of antigen presentation in immune responses, indicates that delayed hypersensitivity may be the main pathogenic factor in the development of Warthin's tumor.

Immunohistochemical localization of carcinoembryonic antigen in carcinoma in pleomorphic adenoma of salivary gland: use in the diagnosis of benign and malignant lesions.

The localization of carcinoembryonic antigen (CEA) and lysozyme (LZM) was immunohistochemically studied in 34 carcinomas arising in benign pleomorphic adenomas and 25 normal salivary glands in order to assess its potential diagnostic value. CEA in the normal salivary gland was located in luminal cell membranes of intercalated duct cells and serous acinar cells. Strongly positive cell surface and intraluminal staining of CEA appeared in the areas of gland-forming pattern in pleomorphic adenoma. CEA activity was detected in 7/9 cases (78%) of adenocarcinoma, 10/11 cases (91%) of epidermoid carcinoma, 3/8 cases (38%) of anaplastic carcinoma, 5/5 cases (100%) of mucoepidermoid carcinoma, and 1/1 case (100%) of adenoid cystic carcinoma. CEA was always present in the cytoplasm of epithelial cells and luminal contents of neoplastic glands. CEA in epidermoid carcinoma may occasionally react strongly in the cytoplasm. Lysozyme-immunoreactivity was detected in the cytoplasm of intercalated duct cells and serous acinar cells of the normal salivary gland but little or no LZM was observed in any of the tumors. These results suggest that the presence of CEA could be a useful marker that provides valuable information for the differential diagnosis between benign and malignant areas of carcinoma in pleomorphic adenoma of the salivary gland. Moreover, LZM could be of valuable use for discriminating neoplastic from non-neoplastic tissue of salivary glands.

Three-dimensional atypical structure in intraductal carcinoma differentiating from papilloma and papillomatosis of the breast.

The basic architectural pattern of intraductal proliferative lesions of the breast was established by reconstructing serial sections of luminal spaces and interluminal areas of glandular structures. The materials were surgical specimens from twenty patients with intraductal carcinoma, papilloma, papillomatosis, or so-called borderline lesion. In papilloma and papillomatosis, the luminal spaces were tubular and interconnected forming a three-dimensional (3-D) network, whereas carcinoma was a porous structure with dispersed lumina. The latter represented 3-D atypical structure in intraductal carcinoma. In borderline lesion the architecture was an intermediate type, with separate lumina partially transformed into tubular shapes. It was also confirmed that the porous structure of carcinoma observed in 3-D reconstructions corresponded to the cribriform pattern seen in 2-D figures, while the network of papilloma produced a complex glandular pattern. A geometric parameter was devised to measure the different 3-D patterns of lumina and their 2-D expressions. It was concluded that the 3-D architectural pattern of intraductal proliferations was sufficiently characteristic to be of diagnostic value in differentiating these diseases.

Frozen section diagnosis of hypertensive pulmonary vascular changes at surgery for congenital heart anomalies.

Frozen sections of lung biopsy were prepared in 30 cases of congenital heart anomalies with pulmonary hypertension, and evaluation of severity of the pulmonary vascular changes was made during the surgery. Statistically, significant differences was not found between the diagnosis made by frozen sections and those made by paraffin sections. It is concluded that rapid and accurate diagnosis of pulmonary vascular changes can be made by means of frozen section diagnosis. This method will prove to be clinically important by assisting during operation in rapid assessment of indication for total correction of congenital heart anomalies.

Non-invasive estimation by cross sectional echocardiography of myocardial damage in cardiomyopathy.

Retrospective and prospective studies of high resolution cross sectional echocardiograms were undertaken in order to establish an ultrasonic method for the non-invasive estimation of degeneration and fibrosis of the endomyocardium in cases of cardiomyopathy. When the echocardiograms of the ventricular wall were compared with the histological specimens intense abnormal echoes were seen at the sites of myocardial degeneration and fibrosis of the ventricular wall. The abnormal echoes classified into five types: types I, II, III-1, III-2, and III-3. Type I and type III-1 echoes were the strongest followed by those of types II and III-2, and then those of type III-3. The intensity of the abnormal echoes was 5-20 decibels stronger than that from intact tissue and was closely related to the consistency and density of the diseased tissue. These findings strongly suggest that the boundary between degeneration or fibrosis and the intact normal myocardium was the source of the abnormal myocardial echoes and that the extent and the pattern of the distribution of the sites of degeneration and fibrosis in the myocardium were reflected in the echo patterns. Thus the tissue characteristics of the sites of degeneration or fibrosis of the myocardium may be determined non-invasively by measuring the echo intensity.

Origin and extension of intraductal papillomas of the breast: a three-dimensional reconstruction study.

Surgical specimens from fifteen patients with intraductal papilloma were reconstructed three-dimensionally from semi-serial sections to visualize the intraductal distribution of papillomas. Our results showed two basically different papillomas. In five patients, the papillomas were single and originated in the large ducts such as the segmental or subsegmental duct, but did not involve the terminal ductal-lobular units (TDLU); this type corresponded to the so-called solitary papilloma. In the remaining ten specimens, the papillomas were multifocal three-dimensionally; each had a root in the TDLU and spread into the large ducts, suggesting its purely peripheral origin. In view of this striking difference, and of possible canceration of ductal peripheries, a nomenclature of peripheral vs central papillomas is proposed instead of the conventional multiple vs solitary. Duct papillomatosis, invariably situated within the TDLU, was shown to be a continuation of peripheral papilloma and was regarded accordingly as a prepapillomatous condition.

[Susceptibility of intraductal papillomas to carcinogenesis based on 3-dimensional reconstruction study].

Surgical specimens from 16 patients with intraductal papilloma were subjected to 3-dimensional reconstruction studies of mammary ducts in order to detect early cancerous foci developing in papilloma, to analyze growth behavior of papilloma and to establish morphological features of papilloma particularly prone to cancerous change. Papilloma of the multiple type (10 cases) originated in the terminal duct lobular units (TDLU), while papilloma of the solitary type (6 cases) originated in the large ducts except for one. In 4 out of the 16 patients small foci of intraductal carcinoma were found during specimen reconstruction. Two of the four patients harbored minimal cancer as small as 3 mm or less. The carcinomas with multifocal origins in the TDLUs were anatomically connected with peripheral papilloma so far as to take the form of "cancer in papilloma". Papilloma preceding carcinoma was of the multiple type in 3 cases and of the solitary type in one. But according to the sites from which papillomas originated, 4 out of the 11 cases originating in the TDLU had cancerous foci, whereas none of the 5 cases originating in the large ducts did. This suggests that the peripheral papilloma is highly susceptible to cancerous change. From a clinical point of view, we proposed a nomenclature of peripheral vs. central papillomas instead of the conventional multiple vs. solitary.

Hyperplasia of small hepatic cells in the precancerous condition of cirrhotic livers.

Karyometrical analysis was performed in order to characterize histologically the precancerous condition of cirrhotic livers. The following parameters of hepatocytic nuclei were estimated in 29 normal and 35 cirrhotic livers: Nvo, the number of nuclei in a unit volume of hepatic tissue; D, the mean of nuclear diameters; s. the standard deviation of nuclear diameters (the degree of anisokaryosis); and N/C, the nucleocytoplasmic volume ratio. Results indicate: (1) In normal livers, Nvo is inversely correlated with age (r = 0.53, p less than 0.01), D and s are positively correlated with age (r = 0.68, p less than 0.01, and r = 0.75, p less than 0.01, respectively) and N/C is almost constant regardless of age. (2) Each of the parameters is independent of age in cirrhotic livers. (3) Nvo and N/C are larger, and D and s are smaller in cirrhotic livers with hepatocellular carcinoma than in those without carcinoma (p less than 0.05). From these results, it is concluded that the parenchyma of cirrhotic livers with hepatocellular carcinoma, which is in strongly precancerous conditions, can be characterized by hyperplasia of small hepatic cells rather than by liver cell dysplasia which is defined by marked anisokaryosis and nuclear pleomorphism.

[Non-Hodgkin's malignant lymphoma of the oral cavity and Waldeyer's ring--clinico-pathological Studies of biopsy cases].

Forty-five patients with stage IE and IIE malignant lymphoma originating from the oral cavity (23 cases) and Waldeyer's ring (22 cases) were histopathologically studied and their prognoses were assessed. Histologically, 42 cases were identified as diffuse lymphoma. The histiocytic (large cell) type was the prevailing type. The incidence of undifferentiated (large cell) type was relatively high and a few pleomorphic type and Burkitt type lymphomas were found in the oral cavity. The 5-year survival rate was 17% for the oral cavity and 50% for Waldeyer's ring. The survival rate of patients with stage IE malignant lymphomas of Waldeyer's ring was better than that of patients with stage IIE malignant lymphomas of Waldeyer's ring and stage IE X IIE oral malignant lymphomas.

[Genesis and extension of intraductal papillomas of the breast--a three-dimensional morphological study].

Graphic reconstruction of semi-serially sectioned breast specimens allowed a three-dimensional observation of intraductal proliferative lesions and their growth characteristics. Ten surgical specimens with intraductal papillomas obtained by microdochectomy were subjected to this study, and the primary site and extending behavior of papillomas were analysed. The results indicated: (1) In seven out of the ten cases papillomas appeared to arise from terminal duct lobular unit (TDLU) as multiple foci three-dimensionally independent of each other. (2) Secondary ductal involvement by multiple papillomas attained to as far as subsegmental or segmental level. (3) On the other hand solitary papilloma arose from segmental duct without involving TDLU. (4) Both solitary and multiple papillomas transformed and distorted the duct system while sustaining its basically arborescent pattern. (5) Duct papillomatosis, situated within TDLU, was often proved to be connected with multiple papillomas in more proximal ducts, and therefore was regarded as a prepapillomatous lesion. (6) Retention cysts associated with multiple papillomas were accounted for by intraductal proliferations obstructing outflow tracts.
