[The PABA test].

PABA test has proved to be an easy and reliable test for determination of exocrine pancreatic insufficiency. N-benzoyl-L-tyrosyl-p-aminobenzoic acid or 4-(N-acetyl-L-tyrosyl) aminobenzoic acid are split by action of chymotrypsin in the small intestine. N.O-diacetyl-L-tyrosyl-p-aminobenzoic acid is converted easy in vivo in 4(N-acetyl-L-tyrosyl) aminobenzoic acid. The amount of 4-aminobenzoic acid (PABA) in urine collected for 6-10 hours is used as an index of chymotrypsin production. The concentration of PABA (and aromatic amines) is estimated in urine by the Bratton and Marshall method. p-dimethylamino cinnamaldehyde is less useful for the determination of urinary PABA. 60 min are necessary as time for acid hydrolysis of conjugated PABA metabolites. False abnormal test results are found for instance in patients with inflammatory bowel diseases, small bowel resection, impaired liver function, anorexia nervosa, lambliasis or renal insufficiency. The PABA test appears in consideration of these restrictions to be an useful simple method in the assessment of exocrine pancreatic function.

[Effect of iron on Pasteurella multocida].

Iron is an important factor for growth, virulence and immunogenicity of the species Pasteurella multocida. This has been demonstrated in numerous experiments with bacterial cultures in vitro and immunized and not immunized animals in vivo (mice, piglets, calves). Iron substrates or iron chelators affect in different manner the virulence of P. multocida in vivo, depending on chemical character of the given compounds, their dose, route and time of application, and also depending on the host. P. multocida has an up to time unknown iron transport system, which can acquire the essential iron from physiological substances, such as heme, ferritine, transferrine, lactoferrine etc. This conclusion results from in vitro experiments with growing cultures, with insertion of radioactive iron (Fe-59) from different sources, and with iron solubilization in neutral pH ranges. In the same way, the iron of iron dextran and low molecular iron compounds is available for P. multocida. Iron of unphysiological complexes, potassium ferrocyanide, and ferrocene is unavailable. On the other side such iron chelating agents as nitrilotriacetate, tirone, ferrocene, citrate, EDTA, and apotransferrine do not or only a little affect growth, and such chelators as alpha, alpha'-dipyridyle, phenanthroline and the microbial siderophores deferrioxamin B and enterobactin are inhibitory substances for multiplication of P. multocida. This substances also inhibit the insertion of Fe-59 into the bacterial cell. The conclusion is drawn that neither enterobactin nor deferrioxamine B as typical representatives of phenolate or hydroxamate siderophores take part in Fe-transport of P. multocida.

[Clinical aspects of pseudomyxoma peritonei].

On the basis of two cases the problems of peritoneal pseudomyxoma (PP) are discussed with respect to pathogenesis, clinical diagnosis and therapy. The abundant quantities of mucus present in the peritoneal cavity in this syndrome originate primarily in ruptured histologically malignant or benign mucous ovarian cystomas or benign mucocele or mucogenic carcinomas of the appendix, respectively. Independent of the histological findings the clinical course and consequently the prognosis of PP basically depends on the amounts of mucus present. Besides extirpation of the primary tumour therapy consists primarily in removing mucus as completely as possible, even in the form of repeated interventions. Additional measures are mainly confined to radiotherapy and cytostatic treatment. Recently new possibilities in preoperative diagnosis and postoperative control have been opened up by ultrasound and computed tomography.

[Duchenne's muscular dystrophy: also in girls?].

Duchenne's muscular dystrophy is inherited as a recessive X-linked trait: Even-though it rarely appears in females it can be seen. We have examined 5 children of one family. Two boys and one girl showed typical symptoms and clinical as well as light- and electronmicroscopical findings of this disease. In order to understand the mode of the genetic pattern, we have analysed the chromosomes, proved the fatherhood and assured the increased Ca-pooling in non-necrotic muscle fibers; in vitro-examinations of the amino-acid-incorporation in ribosomes and of the synthesis of collagen in muscular cells were done as well. Evaluating all of the results, the inheritance must be X-linked recessive and the girl, with high incidence, is a so called "manifesting carrier". The explanation offers Lyons hypothesis, which suggests that in most of the girl's muscle cells the X-Chromosomes, inherited from the mother, are active and lead to the manifestation of the illness. Consequences in advising the family genetically must be taken.

[Hypothyroidism in the elderly of advanced age].

Thyroid function tests were performed on 445 in-patients over a four month period at the Department of Medicine of a teaching hospital attached to the University of Kiel. Excluding those severely ill, moribund or known to have abnormal thyroid function, the study included all patients over 60 years of age. Diagnostic criteria were clinical symptoms, results of the TRH test and of T4-RIA, in addition to nonspecific tests of blood sugar, serum cholesterol and blood count. Among the 445 patients there were 15 with manifest hypothyroidism (3.4%), previously undiagnosed. A further 23 (5.2%) had the subclinical variant of "borderline hypothyroidism". The sex ratio was normal for manifest and for borderline hypothyroidism. The classical cardinal clinical symptoms were present in only those with severe and marked hypothyroidism, i.e. not even a third of all those with manifest hypothyroidism.

Lymphocytic choriomeningitis virus. VI. Isolation of a glycoprotein mediating neutralization.

A structural glycoprotein of lymphocytic choriomeningitis virus was obtained in pure form by immunoaffinity chromatography using a monoclonal antibody with high neutralizing activity. It blocked neutralization of viral infectivity by antibody and in polyacrylamide gel electrophoresis it migrated with an apparent molecular weight of 44 X 10(3). We conclude that the isolated material is identical with the previously described gp44 (GP-1).

[Homocystinuria].

It is reported on a female patient with a classical homocystinuria who showed all typical symptoms of the cystathionine-synthesis-insufficiency, such as tall stature, phacetomy, arachnodactyly, kyphoscoliosis, generalized osteoporosis and thromboembolisms. While homocystin in the blood plasma and the urine could be proved only in the patient, the concentration of plasma methionine was much increased also in the clinically completely inconspicuous sister.

[Long-term experience in the therapy and prevention of unspecific vaginal discharge with a Lactobacillus vaccine].

This report presents a study of 28 women with chronic non-specific vaginitis treated by Solco Trichovac, a vaccine containing at least 7 X 10(9) inactivated microorganisms of various strains of Lactobacillus acidophilus in each 0.5 ml dose. 3 injections were administered at 2-week intervals, and the results of therapy investigated with regard to quantitative increase in lactobacilli in the vaginal secretion, change in grade according to Schröder and reduction of the vaginal fluor. The vaccination caused a significant (p less than 0.01) quantitative increase in physiological lactobacilli. Accordingly, there was a significant improvement (p less than 0.01) in grade already 4 weeks after completion of the primary course of treatment. Re-examination after one year showed that nearly 80% of the patients were grade I to II; the vaginal fluor was clinically cured or at least markedly improved in 85.6% of cases. In our experience the vaccination presents a promising new concept of therapy and shows a special effect in patients with chronic non-specific vaginitis and with reduced Döderlein's vaginal bacillus due to restoration of the biological protective mechanisms.

[HDL cholesterol in patients with peripheral arteriosclerotic occlusive disease].

The HDL-cholesterol level in the blood serum was determined in 40 male control persons and 73 male patients with peripheral arteriosclerotic obstructive disease of the lower extremities. A reduction of the HDL concentration as a whole could not be proved. The influence of the various drugs applied to the therapy of the arteriosclerotic obstructive disease on the HDL-cholesterol level in the blood serum is different. With increasing severity of the arterial obstructive disease the HDL level significantly decreases. Patients with hyperlipoproteinaemias show lower HDL concentrations than test persons with inconspicuous serum lipid concentrations.

[Beta 2 glycoprotein I analysis in patients with hyperlipoproteinemia, arteriosclerotic occlusive disease and diabetes mellitus].

Beta 2-glycoprotein-I (apolipoprotein H) is an activator of the lipoprotein lipase. The concentration of beta 2-glycoprotein-I in the blood serum was determined with the help of the radial immunodiffusion. In patients with hyperlipoproteinaemia of type IIa and IIb, arteriosclerotic obstructive disease or diabetes mellitus the beta 2-glycoprotein-I-concentrations were increased. In hyperlipoproteinaemia of type IV can be concluded to a relative beta 2-glycoprotein-I-deficiency.

[Immune insufficiency in enzyme defects of purine metabolism].

Adenosine deaminase (EC 3.5.4.4. - ADA) deaminates adenosine and deoxyadenosine to inosine and deoxyinosine. The distribution of ADA isoenzymes depends on a binding protein. Purine nucleoside phosphorylase (EC 2.4.2.1. - PNP) catabolizes inosine and guanosine to hypoxanthine and guanine. Patients with severe combined immuno-insufficiency often suffer from a congenital ADA deficiency. The PNP deficiency is associated with severely defective T-cell immunity and normal B-cell immunity. Deficiency of ADA leads to an accumulation of adenosine, deoxyadenosine, adenine nucleotides (cAMP, dATP). In PNP deficiency an increased production of inosine, guanosine, deoxyinosine and deoxyguanosine was found. The pathogenesis of the immuno-insufficiency is to be traced back to disturbances in the purine metabolism interfering with the mitogenically induced lymphocyte transformation and other lymphocyte functions, as determined by in vitro tests. Deoxyadenine inhibits the ribonucleoside diphosphate reductase and synthesis of DNA. The overproduction of S-adenosyl-L-homocysteine inhibits methyltransferase reactions and 2'-deoxyadenosine the S-adenosylhomocysteine hydrolase. A decrease of ADA activities was found in T-lymphocytes of patients with Hodgkin's disease. Measurements of ADA activity in patients with leukemias do not explain the impairment of the cellular immune response in leukemias and may be regarded as indicator of increased purine metabolism. The ADA activities are increased in patients with acute immature and chronic myeloic leukemias depending on the activity of the disease. The ADA activity is low in chronic lymphatic leukemia. ADA inhibitors were used for the treatment of T-cell leukemias.
