[Cefotaxime in septicaemia and severe infections due to multiresistant Gram-negative bacilli (author's transl)].

Cefotaxime was given to 26 patients with serious infections resistant to other antibiotics. 15 patients were suffering from septicaemia due to Gram-negative bacilli, which were caused either by a single organism (13 cases:" of which Serratia in 6, Klebsiella in 3) or by several organisms (2 cases); 7 had upper urinary tract infections (caused by Serratia in 4 patients), and 6 had miscellaneous infections (two of these were purulent post-operative meningitis). The pathogens were usually resistant to ampicillin and cephalothin. Cefotaxime was administered at a daily dose of 3 g in 22 patients, irrespective of the renal function, and of 6 g in 4 patients (2 meningitis). Treatment included an aminoglycoside in 12 cases. The efficacy of cefotaxime was very satisfactory in 23 of the 26 patients.

[Lymphogranuloma venereum affecting simultaneously cervical and inguinal lymph nodes (author's transl)].

Nicolas-Favre's disease, also called lymphogranuloma venereum, a venereally transmitted disease caused by Chlamydia trachomatis, is usually presenting as an inguinal lymphadenopathy or an anorectal syndrome. Cervical lymph node involvement is uncommon, and in a review of the literature the authors found about fifteen such cases. The mode of transmission frequently consists of oral sexual contact. In this report, the authors describe a case of simultaneous cervical and inguinal lymph node involvement. The inguinal localization secondarily appeared, and allowed to suspect diagnosis.

[Obstructive mitral vegetations in bacterial endocarditis. Disappearance after migration as an embolism].

An unusual form of mitral valve endocarditis was observed on echocardiography. A 49 year old female with well tolerated mitral stenosis and mild aortic incompetence contracted staphylococcal endocarditis. Pulmonary venous hypertension developed and the diastolic murmur increased. The echocardiogramme showed voluminous vegetations obstructing the stenosed mitral orifice in diastole, simulating a left atrial myxoma. An acute ischaemic episode of the lower limb occurred under antibiotic therapy. A voluminous fibrino-cruoric infected embolus was extracted from the iliac artery and a second echocardiogramme showed the intra mitral mass to have disappeared. Concurrently, the diastolic murmur decreased and the signs of intolerance disappeared. When the infective process seemed to have been controlled, the patient died suddenly. Post-mortem examination showed fresh mitral endocardial lesions and renal and splenic infarcts. Five cases of mitral obstruction by vegetations have been previously reported, three of which had echocardiographic studies. The echocardiographic image is stereotyped and resembles a myxoma wedged in the mitral orifice but without the intra atrial mass. This type of mitral obstruction complicated moderate mitral stenosis in all cases. Regression of the echographic appearances of valvular vegetations has been reported in rare cases, but we were unable to find another case of embolism of vegetations reducing the valvular obstruction.

[Lipids and phospholipids in the fluid of broncho-alveolar lavage. Value for the study of alveolar penetration of corticosteroids by aerosol (author's transl)].

Broncho-alveolar lavages from 13 control subjects have been studied for the following parameters : phospholipids, free fatty acids, total last parameter, through the depolarisation of Diphenylhexatriene, is independent of the recovered volume. An identical study has been performed for two bronchiolo-alveolar cancers. A treatment with hydrocortisone-containing aerosol has been associated with an increase in phospholipid amount and a drastic decrease in microviscosity. This observation suggests that the synthesis of tensio-active phospholipids is dependant of corticoids by an enzymatic induction. The effect of therapeutics given in aerosol can be monitored by the biochemical and physical modification in broncho-alveolar fluids thus obtained.

[Lesional pulmonary edema].

The authors having reported all the possible denominations of syndromes of respiratory distress in the adult mention again the main etiologies, stressing on the last ones. They recall afterwards the different clinical, radiological and biological stages of this syndrome. The hemodynamic profile is defined by normal catheterism data and the presence of a true anatomical shunt. On the other hand several hemodynamic variations are possible. The differential diagnosis is essentially cardiac edema. Finally the authors state that treatment is done only by symptomatic therapies: dehydration and assisted ventilation. They do not make use of membrane oxygenator. Assisted ventilation is essential and is mainly based on a good indication and the optimal use of permanent positive pressure.

[Respiratory localizations of Launois-Bensaude symmetrical lipomatosis. Apropos of 3 cases].

The authors report 3 observations of cervico-facial lipomatosis with mediastinal localization. In the 3 cases the rather unusual localization of Launois and Bensaude's disease produced respiratory troubles which worsened a chronic respiratory insufficiency due to another cause. Once a tracheotomy had to be done because of tracheo-malacia. Another time there was a pharynged localization. This disease etiology always remains obscure, usually appearing in alcoholic addicts of about 50. Mediastinal localizations have been unfrequently described but must be searched for systematically. The evolution is chronic, without specific treatment. If there is already a chronic respiratory insufficiency in relation with a chronic obstructive bronchopneumopathy or a cardiac insufficiency, then the mediastinal localization becomes an aggravating factor.
