Blood pressure and physical fitness in a population of children--the Odense Schoolchild Study.

This cross-sectional study examined the association between blood pressure and physical fitness in 1,369 children (81.5% of the eligible population) aged 8-10 years participating in the first phase of a primary intervention study with physical training. An inverse correlation between BP and physical fitness was demonstrated, and significant differences have been found between quartiles of distribution of physical fitness in girls with regard to systolic BP and in boys and girls with regard to diastolic BP respectively. Important determinants of physical fitness seemed to be dependent on confounding variables representing body mass index, height, heart rate and age. Our observations suggest that the contribution of confounding variables may explain the relationship between BP and physical fitness in childhood.

[Is earlier diagnosis of oral cancer possible?].

Study of patients with oral carcinomas from the Finsen Institute in a five-year period, shows the survival to be significantly correlated to the tumour stage on referral. In order to improve the prognosis for these patients, earlier diagnosis and earlier referral for treatment to the oncological centre are necessary.

A new approach to assessing maximal aerobic power in children: the Odense School Child Study.

In two experiments maximal aerobic power (VO2max) calculated from maximal mechanical power (Wmax) was evaluated in 39 children aged 9-11 years. A maximal multi-stage cycle ergometer exercise test was used with an increase in work load every 3 min. In the first experiment oxygen consumption was measured in 18 children during each of the prescribed work loads and a correction factor was calculated to estimate VO2max using the equation VO2max = 12.Wmax + 5.weight. An appropriate increase in work rate based on height was determined for boys (0.16 W.cm-1) and girls (0.15 W.cm-1) respectively. In the second experiment 21 children performed a maximal cycle ergometer exercise test twice. In addition to the procedure in the first experiment a similar exercise test was performed, but without measurement of oxygen uptake. Calculated VO2max correlated significantly (p less than 0.01) with those values measured in both boys (r = 0.90) and girls (r = 0.95) respectively, and the standard error of estimation for VO2max (calculated) on VO2max (measured) was less than 3.2%. Two expressions of relative work load (%VO2max and %Wmax) were established and found to be closely correlated. The relative work load in %VO2max could be predicted from the relative work load in %Wmax with an average standard error of 3.8%. The data demonstrate that calculated VO2max based on a maximal multi-stage exercise test provides an accurate and valid estimate of VO2max.

Effect of amiloride on arachidonic acid and histamine release from rat mast cells.

The effect of a putative Na+/H+ exchange inhibition on histamine and [14C]arachidonic acid ([14C]AA) release has been examined in rat peritoneal mast cells, using either addition of amiloride or removal of extracellular Na+. The cells were stimulated by non-immunological agents, i.e. calcium ionophore A23187, nerve growth factor (NGF), thapsigargin and compound 48/80. On the basis of the results obtained, a possible role for Na+/H+ exchange in rat mast cell secretion is discussed.

Arginine vasopressin stimulates phosphoinositide turnover in an enriched rat Leydig cell preparation.

An enriched rat Leydig cell preparation was preincubated with [14C]arachidonic acid. Stimulation of the cells with arginine vasopressin (AVP) (1 microM) for 2 min caused a significant increase in labelled phosphatidic acid and a significant fall in radioactivity in phosphatidylinositol and phosphatidylinositol 4-monophosphate + phosphatidylinositol 4,5-bisphosphate. Preincubation with dibutyryl cyclic AMP had no effect on the AVP-induced phospholipid turnover. Leydig cells were preincubated with myo-[2-3]inositol for 22 h and then with 10 mM LiCl for 10 min. Exposure to AVP (1 microM) induced a rise in labelled inositol phosphates. The response was inhibited when the cells were preincubated with the phorbol ester, 4 beta-phorbol 12-myristate 13-acetate (0.16 microM) for 10 min. These results provide evidence for an AVP-induced phospholipase C stimulation in rat Leydig cells and suggest a protein kinase C-dependent feedback inhibition of the stimulation. Other agonists that might have a regulatory function in the testis were tested for possible effects on phosphoinositide metabolism. Of prostaglandin E2 (10 microns,) angiotensin II (0.1 microM), and bradykinin (0.9 microM), only the latter induced a significant increase in the labelled inositol phosphates. This suggests that Leydig cells possess a bradykinin receptor which can activate phospholipase C.

Misonidazole combined with split-course radiotherapy in the treatment of invasive carcinoma of larynx and pharynx: report from the DAHANCA 2 study.

Between October 1979 and May 1985, 626 patients with pharynx and larynx carcinoma were randomized to two different split-course radiation regimens and given either misonidazole (MISO; 11 g/m2) or placebo during the initial 4 weeks of treatment. Patients in the different treatment groups were evenly distributed among stages. The small number of females (136) gave an uneven distribution, thus making analysis difficult. The results show that females had a statistically better loco-regional control (45 vs. 33%; 5-year actuarial value). Overall, the MISO treated group did not have a significantly better control rate than the placebo groups (37 vs. 34%). However, a difference was found in patients with pharynx carcinomas (38 vs. 27%; p less than 0.05). The pre-irradiation hemoglobin (Hb) concentration was found to be a prognostic parameter. In females, loco-regional control for IIb values below or above 8 mmol/l were 37 and 47%, respectively. In males, the same values were below or above 9 mmol/l 26 and 38%. Hb influence on local control was only seen in supraglottic and pharynx tumors. This effect was independent of tumor size, which also was of prognostic value. In addition Hb concentration and MISO had an apparent additive effect. Thus in the male pharynx group, placebo patients with low Hb had a 14% disease rate compared to 40% in MISO treated patients with Hb above 9 mmol/l. MISO induced significant peripheral neuropathy in 26% of the treated patients, whereas other drug related side effects were minimal and tolerable.

Exercise testing in children as a diagnostic tool of future hypertension: the Odense Schoolchild Study.

Systolic blood pressure response and double product (systolic blood pressure x heart rate) response to a progressive maximal exercise test were evaluated in two groups of children aged 8 to 10 years. One group (normal children, n = 68) consisted of children randomly selected from a subset of the population with a resting blood pressure below the 95th percentile, and the other group (high blood pressure, n = 64) included children with a resting blood pressure at or above the 95th percentile. A rise in systolic blood pressure and in the double product was observed during the ergometric performance in both boys and girls in the normal group, and a similar response was observed in the high blood pressure group. These results suggest that the blood pressure response to exercise in childhood gives no further information about the risk of developing hypertension than a blood pressure measurement at rest.

Pregnancy duration and the ratio of long-chain n-3 fatty acids to arachidonic acid in erythrocytes from Faroese women.

Dietary long-chain n-3 fatty acids (FA) may prolong gestation by inhibiting formation of prostaglandins from arachidonic acid. FA were quantified in phosphatidylcholine (PC), phosphatidylethanolamine (PE), and total lipids (TL) of red cells sampled during pregnancy from 29 Faroese women. The ratio of long-chain n-3 FA to arachidonic acid (the (3/6) ratio) was used as the most relevant single measure of exposure. In 18 women with certain gestational age and with spontaneous onset of delivery, gestational age was significantly associated with the (3/6) ratio quantified in PC (correlation coefficient 0.50, P = 0.035), but not with the (3/6) ratio in PE (correlation coefficient 0.21, P = 0.40) or TL (correlation coefficient 0.29, P = 0.26). The association with the (3/6) ratio in PC could not be attributed to confounding by maternal age, weight, marital status or smoking.

The effect of sequential methotrexate and 5-fluorouracil in patients with recurrent head and neck cancer.

Sequential methotrexate and 5-fluorouracil was given to 105 patients with recurrent head and neck cancer. All 105 patients had previously received radiotherapy in curative doses; 41 patients had also been surgically treated and 21 had received previous chemotherapy for recurrence. The treatment consisted of methotrexate 125 mg/m2, followed by 5-fluorouracil 600 mg/m2 after 1 h. Leucovorin rescue was administered after 24 h. A response was observed in 24 patients (23%), including 9 (9%) with complete response. The majority of the responders were patients with a good performance status and with primary tumour in the oropharynx. The toxicity was not negligible, as 5 patients (5%) died of treatment-related causes, mainly due to leukopenia, which was the most frequent and important complication. This sequential methotrexate/5-fluorouracil treatment-schedule cannot be recommended for recurrent head and neck cancer patients.

Thyroid cancer in children and adolescents in Denmark.

From 1960 to 1985, 20 patients (12 females and eight males) with thyroid cancer, developed before the age of 20, have been registered and observed at the Radium Center, The Finsen Institute. Papillary adenocarcinoma was diagnosed in 11 cases, and mixed papillary/follicular adenocarcinoma in six cases. Three patients had medullary carcinoma. No patients had previously received cervical irradiation. On admission, lung metastases were evident in two cases. All patients underwent surgical treatment, which in 15 cases consisted of total thyroidectomy, and in nine also lymph node dissection. External irradiation was given to 10 patients, and radioiodine treatment was performed in five patients. Local and/or distant relapse of the thyroid carcinoma occurred in five cases: three were patients with medullary carcinoma, and two had papillary and mixed adenocarcinoma. The three patients with medullary carcinoma died of their disease, but the other two with recurrence recovered after surgery, external irradiation and/or radioiodine treatment. Currently, 16 patients are alive without evidence of disease. The period of follow-up ranged from 2 to 23 years.

Inhibition by amiloride and by Na+-depletion of A23187-stimulated arachidonic acid and histamine release from rat mast cells.

Rat peritoneal mast cells, labelled with [14C]arachidonic acid, released histamine and [14C]arachidonic acid upon the addition of A23187. The release induced by low concentrations of A23187 was suppressed by removal of extracellular Na+ and by addition of the Na+/H+ exchange inhibitor, amiloride. Addition of protein kinase C inhibitors resulted in only a modest decrease of [14C]arachidonic acid and histamine release. [14C]Arachidonic acid was hydrolyzed from both phosphatidylcholine and triacylglycerol. Amiloride inhibited only the hydrolysis from phosphatidylcholine. It is suggested that an Na+/H+ exchange is stimulated secondary to an A23187-induced increase in intracellular calcium concentration. Increased Na+/H+ exchange seems to facilitate the activation by intracellular calcium ions of phospholipase A2 as well as of histamine secretion.
