Is more heparin necessary for low-dose heparin prophylaxis in uremic patients?

Heparin dose requirements for s.c. heparin prophylaxis were investigated in control individuals and in uremic patients. Heparin levels (neutralisation of factor Xa activity, neutralisation of Xa amidolytic activity; PTT and thrombin time) at timed intervals after single and repetitive s.c. doses of 5,000 or 7,500 IU were measured in 11 uremic patients and 9 individuals with normal renal function. At a dose of 2 x 5,000 IU/d heparin s.c. no difference between controls and uremic individuals was found. In contrast, in uremic patients, peak concentrations and area under the curve (AUC) were significantly lower and plasma half life shorter after 3 x 5,000 IU heparin s.c. After repeated administration of heparin, heparin concentrations (24 hrs) had fallen to nil in uremic patients whereas in controls measurable activity was still demonstrable. After i.v. bolus injection of heparin, peak concentrations were identical in controls and in uremic patients, but elimination half life was prolonged in uremic patients. It is therefore suggested that lower plasma heparin concentration after heparin s.c. is the result of diminished net absorption from the subcutaneous depot. It is concluded that in order to achieve a given plasma activity, higher s.c. doses of heparin must be administered to uremic patients than to non-uremic individuals. But further studies are required to demonstrate safety and efficacy of heparin prophylaxis in uremic individuals.

[Immune reactivity in patients with disorders of lipid metabolism].

The accidental observation of a selective IgM-deficit in a family with hyperlipoproteinaemia type II caused the working hypothese of close interactions of lipometabolism and immunoreactivity. In 69 patients with hyperlipoproteinaemia the quantitative determination of the immunoglobulins G, A and M was performed by the Mancini-test. All decreased values were repeated in a second determination at another moment. In 9 patients with hyperlipoproteinaemia and deficit of immunoglobulins we determined the T-lymphocytes by means of the sheep erythrocyte rosette test, the B-lymphocytes with the help of the method of direct immunofluorescence and the mouse erythrocyte rosette test. Altogether 14 of the 69 patients with hyperlipoproteinaemia showed reproducible deficit of immunoglobulins, which is statistically significant. The immune defect appeared above all as decrease of IgA (11 patients) isolated 8 patients or in combination with IgM-deficit (2 patients) and IgG-deficit (1 patient). 2 resp. 1 patient showed selective IgG- or IgM-deficit. Clinical symptoms of an immune defect were not recognizable anamnestically and prospectively. In the 9 patients who were immunologically further characterized the T-lymphocytes were in the region of the norm or were increased, the subpopulation of B-lymphocytes which reacted with mouse erythrocytes was normal or increased. The classification to the types of hyperlipoproteinaemia of the altogether 14 deficient patients showed the order IIa, IV, IIb. The original working hypothesis could be supported by the present findings, though an obvious clinical importance is not yet to be recognized. Changes of the membrane lipids of lymphocytes, interventions through the prostaglandin system and effect via suppressor cells must be discussed causally.

Adenosine deaminase activity in plasma and blood cells of patients with haematological and autoimmune diseases.

Adenosine deaminase (ADA) has been assayed in plasma, erythrocytes, and lymphocytes from 29 patients with haematological and autoimmune diseases. ADA activity was uniformly low in erythrocytes and lymphocytes from patients with non-Hodgkin lymphoma and multiple myeloma (p less than 0.001). High levels of ADA activity was found in plasma, erythrocytes, and lymphocytes from patients with myeloid leukemia (p less than 0.001). ADA was high in plasma but low in erythrocytes and lymphocytes from patients with autoimmune diseases treated with immunosuppressive drugs (p less than 0.05). 4 adults with congenital immunodeficiency showed decreased ADA activity. In the control group of normal blood donors we found a 34-year-old female with low ADA activity in plasma, erythrocytes, and lymphocytes without any immunological abnormalities. This is the 3rd case of a healthy individual deficient for ADA. 1 patient with Osler's disease and high ADA activity in erythrocytes showed the importance of the purine salvage enzyme not only in lymphocytes.

[Metabolism of myocardial catecholamines and plasma catecholamine levels in patients with chronic aortic regurgitation (author's transl)].

In 17 patients suffering from severe chronic aortic regurgitation (class III N.Y.H.A.), during aortic valve replacement right atrial biopsies were taken. In these samples myocardial content of norepinephrine, normetanephrine, epinephrine and dopamine were determined. 2-4 days before valve replacement, plasma catecholamines were measured at rest and during isometric exercise. 15 patients with atrial septum defect (foramen secundum) served as controls. In patients with aortic regurgitation demonstrating no clinical signs of cardiac failure, myocardial content of norepinephrine, normetanephrine and epinephrine was significantly reduced compared to tissue content of patients with atrial septum defect. Right atrial content of dopamine did not reveal any difference between the two groups. In all patients, plasma catecholamines at rest were within normal limits. During isometric exercise, however, the increase of plasma norepinephrine in patients with aortic regurgitation was significantly higher compared to the control group. It is concluded from these data that patients with aortic regurgitation, demonstrating an exaggerated increase in sympathetic activity during exercise, probably show a decrease in myocardial norepinephrine tissue content. Therefore, clinical demonstration of hyperadrenergic response during exercise represents a simple metabolic parameter indicating alteration in myocardial metabolism in patients with chronic aortic incompetence. Since decision for aortic valve replacement should be done before irreversible damage of myocardium has established, it is supposed that assessment of sympathetic activity, at rest and during exercise, may reveal practical clinical importance.

[Autoantibodies in malignant tumors].

Autoimmune phenomena are to be referred to the possible immune reactions of the host organism in malignant tumours. By means of the indirect immune fluorescence method in 95 sera of patients with tumours and a control group 0f 30 blood donors was sought for autoantibodies against mitochondria, nuclear factors, smooth musculature, myocardium, delomorphous cells. In the blood donors in one case autoantibodies against the myocardium could be proved, the tumor group, however, revealed positive findings of autoantibodies in 25 patients, which are statistically significant (p < 0.05). In the individual groups of diagnosis the acute myeloic leukaemia, the IgG-plasmocytoma and the cervical carcinoma showed a highly significant positive proof reaction compared with the control. The titres lie in their highest values at 1 : 64, in most cases at 1 : 16 and 1 : 32 and speak for an accompanying reaction are epiphenomenon. Autoantibodies against mitochondria were in no case found against smooth musculature in all positive groups. Apart from the absence of antibodies against delomorphous cells in gynaecological tumours the differentiation of types of autoantibodies do not show a reference to kind of tumour and localisation. The proof of autoantibodies in malignant tumours should be used for the judgment of the immune state

[Clinical significance and study of granulocyte function].

Concerning the total defense of the body the function of granulocytes in the determining parts of phagocytosis (motility, chemotaxis, opsonation, ingestion, microbicidia) are described and the congenital and acquired distrubances of phagocytosis are treated as short survey as well as partly explained by clinical examples. Out of the manifold possibilities of examinations of the function of granulocytes the NBT-test is described as an estimation of the function of granulocytes usable in clinical routine wotk and its application is recommended.

[Conjugation and isolation of POD labelled antibodies [author's transl)].

1. The POD content of the antibody/POD conjugate was measured by means of a method according to Møller and Ottolenghi (1966). Following the incubation of H2O2-benzidine solution the intensity of the yellow-green staining was registered at 410 nm. This method can be applied in the enzyme immunoassay too. 2. For every new test system the optimal antibody/POD ratio of the conjugate has to find out. Conjugates with a IgG/POD ratio in the range of 1 : 0.432--1.114 proved valid in cytological studies. 3. Precipitation, centrifugation, washing, and dialysis did not lead to a reduction of the enzyme activity. 4. In order to decrease nonspecific reactions by conjugate was separated from free POD by means of NH4SO4 precipitation or Sephadex G 200 chromatography. Additionally, nonconjugated antibodies and free POD were separated chromatographically by means of Biogel A 0.5.

Regional myocardial blood flow after hypothermic arrest and cardioplegia.

Malperfusion due to increased coronary vascular resistance is presumably one of the factors responsible for incomplete functional recovery of the heart after aortic cross-clamping. Myocaridal blood flow (MBF, radioactive microspheres) was measured before and after 60 min of hypothermic ischemia in 16 dogs on cardiopulmonary bypass. After ischemia the hearts were reperfused for 30 min. MBF was measured in the empty beating heart and in the isovolumetrically contracting ventricle loaded with enddiastolic volumes (EDV) of 10, 20 and 30 ml (intraventricular latex balloon).

[Immunologic studies of lymph nodes in patients with maxillofacial tumors].

Starting from the basic concept that the regional lymph node, due to its important role in the defensive reaction to cancer, has a great value of information, the authors investigated 14 lymph nodes from patients with cancer in the maxillofacial region. The following immunological examinations were performed: T lymphocyte determination by means of the ovine erythrocyte rosette test; B lymphocyte determination by means of the mouse erythrocyte rosette test, with surface immunoglobulin estimation using the direct immunofluorescence technique; quantitative determination of immunoglobulins G, A and M in the lymph node extract. The results demonstrated that the determination of the absolute number of peripheral lymphocytes and of T rosettes as well as the immunoglobulin quantitation are very valuable. The decrease in T cells in the lymph node is obviously preceded by a similar development in the peripheral blood and seems to be a prognostic parameter. In lymph nodes in the maxillofacial region, a greater importance must be ascribed to IgA (as compared to the other immunoglobulins) than in other regions. The immunoreactivity against cancers in the maxillofacial region has a definite humoral component mainly determined by IgA. In addition to an increase in the number of samples examined by means of the aforementioned methods, parallel T cell determinations in the peripheral blood are advisable with regard to the evaluation by the influence of the cancer on the immunological system and for the purpose of prognostication.

[The role of various methods of electrophoresis in the diagnosis of paraproteinemia].

For the estimation of different electrophoresis methods for indication and identification of paraproteins as routine methods are proposed the cellulose acetate electrophoresis and the fixation immunoelectrophoresis in cellulose acetate foils. Under adequate conditions the agar gel electrophoresis working with larger separability can be used together with immunochemical investigations. In examinations of uroproteins and also for the identification of rare and atypical cases of paraproteinaemia (narrower and several M-gradients, immunoglobulin fragments and complexes) in connection with the methods mentioned also the electrophoresis in the polyacrylamide gel (PAG and in the system PAG-SDS) can represent a valuable information.

[Animal experimental studies on immunogenicity, humoral response and danger of anaphylaxis in parenteral administration of hyaluronidase].

The widespread intravenous application of hyaluronidase rises questions for its potential immunogenicity, formation of humoral antibodies, and danger of anaphylaxis. In experiments on 21 rabbits and 40 rats, the authors searched for precipitating antibodies after subcutaneous, intramuscular, and intravenous application of hyaluronidase in doses equivalent to the human. Intravenous and intramuscular shots of 150 to 75 000 IU of Hylase were applied in order to test anaphylaxis. By all proving procedures antibodies against Hylase were found. The formation of antibodies occurred earlier and in higher concentrations after subcutaneous and intramuscular application. The antibodies belonged to the IgG group. One third of the animals showed anaphylactic responses at doses which were 13 to 630 times as high. 26 per cent of human patients developed antibodies after application of Hylase. No anaphylactic reactions were observed in 17 patients with antibodies when intravenous application of hyaluronidase was continued. In the dosage used in the man anaphylactic response is obviously rare though it is possible.

[Familial IgM deficiency and type II hyperlipoproteinemia].

It is reported on the common appearance of a hyperlipoproteinaemia type II after Fredrickson and of a selective insufficiency of IgM in a family. The two generations of a family which have been observed since three years show these disturbances in a 42-year-old patient and all his four children. The diminution and the lack of a stimulation of B-cells under antihuman-IgM in the transformation test of lymphocytes refer to an isolated disturbance of the IgM-synthesis. Cellular immune reactions and haematologically cytological findings were normal. A connection between the lack of IgM and hypercholesterolaemia is not excluded, since experimental investigations of Alderson and Green show an influence of cholesterol on the function of the lymphocyte membrane. The necessity of systematic investigations on patients with immune defects and hyperlipoproteinaemias is emphasized for the exclusion of an accidental coincidence.

[Technics for B-cell determination in lymphocytes].

The conception of the subdivision of lymphocytes into T- (dependent on thymus) and B- (dependent on bone marrow or bursa) cells which was transferred from animal experiments on mice to man has already clinical significance despite some deficiencies. For immunoproliferative clinical pictures, immune defects and autoimmune diseases a determination of the B- and T-lymphocytes in the peripheral blood is important. As methods for the determination of T-lymphocytes the rosette-test with erythrocytes of sheep (E-rosettes) as well as the immunofluorescence labelling of anti-T-lymphocyte globulin are used. For the demonstration of B-lymphocytes to-day the immunofluorescence microscopy, the autoradiography, the immune enzyme technique and the rosette tests for the determination of the Fc-receptor (EA-rosette) and the complement receptor (EAC-rosette), respectively, are used. Possibilities of differentiation between the two populations of lymphocytes are furthermore obtained by the grid electron microscopy and the important functional tests of the transformation of lymphocytes by means of unspecific and specific stimulants. In own investigations by means of the direct and indirect immunofluorescence and immune enzyme technique of the peripheral blood and from lymphatic nodes were tested. In normal persons a large proportion of IgM-receptors on the lymphocyte membrane was established. Patients with chronic lymphadenosis had a high number of B-lymphocytes, which clearly decreased under cytostatic therapy. In plasmocytoma, Waldenström's disease and primary immune defects the connections between the dominating classes of immunoglobulins on the membrane and in the serum. Investigations of the different populations of lymphocytes in there are peripheral blood and in the lymphatic nodes are suited to develop new conceptions on pathogenetic intergrades, therapeutic influences and variants of the course of immunoproliferative and immunodefective clinical pictures.
