Effect of neutrophils on the growth of human pre-S1-reactive T-cells in vitro.

Granulocyte factor (GF) derives from the specific granules of polymorphonuclear neutrophils. GF possesses an immunoregulatory activity and augments the immune response to antigens in vitro and in vivo. The Pre-S1 sensitive T-cells reactive to Pre-S1 protein in vitro were developed from peripheral blood mononuclear cells of hepatitis B convalescent by in vitro Pre-S1 protein stimulation. The GF involvement in the activation and development of the Pre-S1 specific regulatory T-cells in vitro was studied.

Modification of some lymphocyte functions in vitro by opioid receptor agonists and antagonist in chronic uremic patients and healthy subjects.

In ten chronic uremic patients on regular hemodialysis treatment in vitro experiments revealed that stimulation of opioid receptors with morphine did not significantly change the mitogen-induced proliferative response of peripheral blood lymphocytes and interleukin-2 (IL-2) receptor expression on PHA-stimulated lymphocytes, while it appreciably decreased surface transferrin (Trf) receptor expression on PHA-stimulated lymphocytes. However, metenkephalin inhibited mitogen-induced proliferation and surface Trf receptor expression on uremic lymphocytes without affecting IL-2 receptor expression on PHA-stimulated cells. In ten healthy subjects opioid receptor agonists did not significantly affect mitogen-induced proliferation of lymphocytes, except for the inhibitory effect of 10(-8) M morphine in relation to lymphocytes stimulated with an optimal pokeweed mitogen (PWM) concentration. At the same time, opioid receptor agonists depressed surface IL-2 and Trf receptor expression on PHA-stimulated normal lymphocytes. In most of our experiments naloxone itself, a non-selective competitive opioid receptor antagonist, decreased mitogen-induced lymphocyte proliferation and IL-2 and Trf receptor expression on PHA-stimulated lymphocytes. Moreover, most frequently naloxone did not reverse inhibitory effects of opioid receptor agonists on lymphocytes. The results seem to indicate that opioid receptor stimulation by high metenkephalin concentrations, which are observed in the uremic blood plasma, may share the responsibility for immunodeficiency in chronic uremic patients. Next, in the presence of opioid receptor agonists directions of changes in the mitogen-induced proliferative response may not follow the alterations of IL-2 and Trf receptor expression on both uremic and normal lymphocytes. Finally the results also suggest that naloxone may possibly exert effects which are independent of its action on opioid receptors on lymphocytes.

Effect of thymic extract on allogeneic MLR and mitogen-induced responses in patients with chronic active hepatitis B.

Cell-mediated immune reactions are involved in the development of chronic active hepatitis (CAH-B). The present studies confirm that lymphocyte from patients with CAH-B have a decreased allogeneic mixed lymphocyte reaction (allo-MLR) and mitogen-induced responses. These abnormalities probably play a role in the immunological dysfunctions underlying chronic liver diseases. Twenty one patients with biopsy-proven CAH-B and a significantly lowered CD4:CD8 cell number ratio were treated with thymic extract (Thymomodulin-TFX) for 6 months. The results of immunological, serological and biochemical findings indicate that this preparation has an immunoregulatory action and produces beneficial clinical effect in patients with CAH-B.

The effect of long-term thymosine factor x/TFx/treatment on T lymphocyte subpopulations and peripheral blood mononuclear cell cytotoxicity in chronic active hepatitis B.

Peripheral blood mononuclear cells (PBMC) cytotoxic activity, percentage of natural killer (NK) cells and T lymphocyte subpopulations in PBMC of 24 patients suffering from chronic active hepatitis B (CAH-B) have been studied. In comparison with healthy subjects CAH-B patients had significantly enhanced both lymphocyte T suppressor (Ts) and NK cell number. Thirteen CAH-B patients with higher Ts number and low lymphocyte T helper/T suppressor ratio (Th/Ts) were subjected to immunostimulation by thymosine factor x (TFx) treatment. After 1 year of TFx treatment all the CAH-B patients improved clinically, the Th/Ts ratio was brought back to normal and 80 per cent of HBe Ag carriers seroconverted to anti-HBe. After one year of TFx administration no significant changes were observed in PBMC cytotoxicity and NK cell number. TFx appears to be a very effective therapeutic agent in some forms of CAH-B. The obtained results show that TFx normalises biochemical parameters of liver lesion and Th/Ts ratio without any effect on NK cell activity in CAH-B patients.

Lymphotoxin production in multiple sclerosis patients.

Lymphotoxin (LT) is a cytotoxic and cytostatic lymphokine produced by activated T and B lymphocytes. Recently, the influence of cytokines on the pathomechanism of demyelinization was investigated. The authors studied LT production in the multiple sclerosis patients. The control group were patients with other neurologic diseases. Spontaneous LT production by T lymphocytes in multiple sclerosis patients was statistically increased during relapse in comparison with the LT production in OND patients. Despite of mitogen stimulation T and B lymphocytes from MS patients produced gradually decreased amounts of LT. The authors suggest that LT might be the product of activated lymphocytes that cause demyelinization during multiple sclerosis.

Synthesis and expression in E. coli of the gene for human tumor necrosis factor (h alpha TNF).

A gene coding for human tumor necrosis factor (h alpha TNF) has been assembled by ligating short oligodeoxyribonucleotides and cloning into plasmid vectors. These oligonucleotides were prepared by the modified phosphoramidite methodology using isopropoxyacetyl (IPA) as a protecting group for exoamino- functions of nucleosides. Gene was expressed in E. coli and the protein product was purified to homogeneity by ion-exchange chromatography.

Treatment with thymic extract TFX for chronic active hepatitis B.

Eighteen patients with biopsy-proven chronic active hepatitis B (CAHB) and a significantly lowered CD4+/CD8+ cell number ratio were treated with thymic factor X (TFX): group I (n = 12) - for 12 months, group II (n = 6) for 6 months. As early as 14 days after starting the treatment a lowering of CD8+ cell numbers with a rise in CD4+/CD8+ cell number ratio were found. These changes continued to progress during the next months and were accompanied by decreased in NK cell numbers with enhancement of NK cell activity. Normalization of the biochemical and immunological parameters occurred after 5-6 months of the treatment. In both groups of patients seroconversion in the HBe system was observed after 9-12 months of the treatment. After two years complete clinical remission persists with normal biochemical and immunological findings and without reversion in the HBe system. The results seem to indicate that TFX has an immunostimulatory action and exerts beneficial effects on the course of CAHB.

The effects of occupational exposure to hydrocarbons on some immune parameters of workers of the phenol division of a petrochemical plant.

The subjects of investigation included 34 workers at the Phenol Division of the Mazovian Refining and Petrochemical Plants and 18 persons working in administrative positions; the purpose was to elucidate some functions of the immune system cells. The lymphocyte subpopulations (CD3, CD4, CD8) and NK cells were evaluated using monoclonal antibodies and the immunofluorescence method. To search for the functions of lymphocytes and monocytes such as the in vitro production of interleukin-1 (Il-1) and interleukin-2 (Il-2), macrophage inhibitory factor generation (MIF), a cytotoxicity test and T cell proliferation in autologous mixed lymphocyte reaction (AMLR) were also examined. We have shown that workers from the Phenol Division exhibited pronounced deviations in the tested parameters as compared to the administration workers. We found a decrease in CD3 lymphocytes and in Il-1 production. After analysis of the individual results we selected three persons from the Phenol Division who showed abnormal values in four or more parameters (decreased CD4:CD8 ratio, abnormal values of lymphocyte subpopulations, impaired lymphocyte functions in functional tests). These cellular-immunity control tests are promising methods for studies of the biological effects of environmental and/or occupational pollution to toxic derivates of petroleum.

In vivo evaluation of bronchoalveolar lavage (BAL) fluid in atopic bronchial asthma, chronic bronchitis and sarcoidosis patients.

The intensity of inflammatory response was evaluated in skin test on guinea pig using bronchoalveolar lavage (BAL) fluid obtained from patients with some diseases of the respiratory tract. The results of skin test were verified with activities of proteases in BAL fluid. The study was performed on 24 patients with atopic bronchial asthma, 21 with chronic bronchitis, 13 with sarcoidosis (II phase) and 18 control subjects. All patients were undergoing fiberoptic bronchoscopies and BAL fluid was obtained. The results of skin test on guinea pig using BAL fluid were correlated with the activities of acid and neutral proteases. The highest activity of proteases and intensity of skin reactions were noted in patients with atopic bronchial asthma and sarcoidosis. Authors suggest that the skin test on guinea pig with BAL fluid may be useful tool for total evaluation of inflammatory response in patients with atopic bronchial asthma, chronic bronchitis and sarcoidosis.

Immunomodulatory effect of naloxone in duodenal ulcer patients.

The effect of a single dose of the opiate receptor antagonist naloxone (NAL) on some immune parameters in chronic duodenal ulcer patients and healthy controls was investigated. Twelve hospitalized men, aged 19-23 years (nine duodenal ulcer patients and three controls) were given NAL intravenously at a dose of 0.03 mg/kg. Blood samples were drawn before and 30 and 150 min after NAL injection. Results obtained in duodenal ulcer patients and healthy subjects revealed the same direction of changes despite a rather wide scatter. Baseline NK cell activity was lower in duodenal ulcer patients than in healthy persons. In approximately one fourth of patients the changes in individual parameters observed following NAL injection were slight. Numbers of total lymphocytes, Th, Ts, NK cells, monocytes and Th/Ts ratio did not significantly change after NAL administration. T-lymphocyte counts moderately decreased at 30 min after NAL injection. NAL did not affect spontaneous IL-2 receptor expression and it moderately increased PHA-induced IL-2 receptor expression in most of the investigated persons. IL-2 generation and NK cell activity slightly, but significantly, increased at 30 min following NAL injection. NAL markedly inhibited lymphocyte proliferation in an AMLR test 30 min after its administration. Most of the investigated parameters returned to their initial levels after 150 min following NAL administration. The studies showed that not only endorphins and enkephalins may have an immunomodulatory action, but NAL, their antagonist, may also affect some functions of the immune system in humans, although its action is transient.

Effect of hemodialysis on T lymphocyte subsets, con-A-activated suppressor cell activity, and interleukin-2 receptor expression on lymphocytes in chronic uremic patients.

Effect of hemodialysis (HD) on some indices of immune response was studied in nine chronic uremics. Total lymphocyte, OKT4+, and OKT8+ cell numbers significantly decreased during the first 20 min of HD, and they were decreased till the third hour of the procedure, whereas the OKT4+/OKT8+ cell number ratio did not change significantly. Before HD, Con-A--activated suppressor cells exerted a stimulatory action on autologous responder cells measured in two-step culture. During HD, Con-A-activated suppressor cell activity transiently appeared, with its peak at 60 min after the start of HD. It was accompanied by a transient rise in lymphocyte count with spontaneous interleukin-2 (IL-2) receptor expression, whereas the number of cells expressing IL-2 receptor following phytohemaglutinin (PHA) stimulation was progressively decreased during HD. A significant correlation was found between the increment of Con-A-activated suppressor cell activity and the increment of spontaneous IL-2 receptor expression on lymphocytes during one single blood flow through the dialyzer. The results supply further evidence that HD may impose additional disturbances on immune regulation in chronic uremics.

T lymphocyte subsets, natural killer cell cytotoxicity and autologous mixed lymphocyte response in children after splenectomy because of hereditary spherocytosis.

Lymphocyte subpopulations and autologous mixed lymphocyte reaction (AMLR) studied in 13 hereditary sphereocytosis and 21 postsplenectomy patients; the operation was performed because of hereditary spherocytosis. Significant increases of Th cells and the Th/Ts ratio have been noted in children with hereditary spherocytosis. Splenectomy because of spherocytosis has resulted in an increase in Ts cell number and diminution of Th/Ts ratio. Hereditary spherocytosis patients showed both diminished natural killer cell activity and AMLR proliferation. Splenectomy normalized the above parameters. The presented data demonstrated disturbances in T lymphocyte number and functions both before and after splenectomy in children with hereditary spherocytosis.

Effect of epinephrine, propranolol, and verapamil on the chemiluminescence of neutrophils in duodenal ulcer patients.

To answer the question whether beta-adrenergic receptor agonists or antagonists and calcium channel blocking agents affect activation of neutrophils in vivo, the chemiluminescence (CL) test was employed. The intensity of emitted photons was amplified by luminol. The effect of investigated agents was measured after stimulation of isolated peripheral blood polymorphonuclear leukocytes (PMNLs) with opsonized zymosan particles. The investigations were performed on 9 duodenal ulcer patients, aged 19-23 years, after informed consent. Single subcutaneous dose of epinephrine (0.014 mg/kg) induced a marked increase in PMNL number and a moderate, but significant, decrease in CL 30 min after injection. Verapamil (0.15 mg/kg intravenously) diminished CL, propranolol (0.1 mg/kg intravenously) enhanced CL, but 150 min after injections CL was approaching the initial values. The obtained results suggest that the investigated compounds may modify the PMNL function in vivo.

Effect of verapamil on chemiluminescence of peripheral blood neutrophils during hemodialysis in chronic uremic patients.

Luminol-dependent chemiluminescence of resting and opsonized zymosan-activated neutrophils isolated from the peripheral blood was determined in 10 chronic uremic patients during early period of hemodialysis without verapamil or preceded by verapamil injection. Verapamil was found to diminish depression of neutrophil chemiluminescence both at rest and following activation with opsonized zymosan. The drug also diminished transient hemodialysis neutropenia. The results seem to indicate that verapamil may exert beneficial effects on inflammatory reactions induced by hemodialysis in chronic uremics.

T lymphocyte subpopulations in patients with chronic active hepatitis B virus infection characterized by monoclonal antibodies.

Peripheral blood T lymphocytes and T cell subsets were analyzed in 19 patients with chronic active hepatitis B virus infection. The T cell subpopulations were defined by indirect immuno-fluorescence using monoclonal antibodies, and additionally, natural killer cell activity against K 562 human erythroleukemic cell has also been defined. All investigated patients have possessed nearly identical clinical and biochemical signs of disease, but on the basis of T lymphocyte subpopulations they could be divided into two groups: low CD4/CD8 ratio group and normal or high CD4/CD8 ratio group. Mechanism responsible for such differences remains hypothetical but it seems reasonable to propose different protocol for immunomodulatory treatment of patients in both groups.
