[Possibilities of predicting the outcome of diseases after cardiosurgical interventions].

A multifactorial correlation and regression analysis with the use of a pack of applied programs "SAND" in the computer SM-4 was performed for prognosis of the outcome of the disease in 38 patients with acquired heart diseases operated upon under conditions of extracorporeal circulation. A linear mathematical model was obtained which allowed predicting the course of the postoperative period and outcome of the disease of cardio-surgical patients with 70-80% probability.

[The surgery of suppurative-septic diseases of the heart: its characteristics and outlook].

An analysis of the experience with surgical treatment of more than 320 patients with infectious endocarditis has shown the necessity to isolate pyo-septic cardiosurgery into an independent line. The arguments are: the specificity of the contingent of patients, special features of diagnosing, operative technique and period of rehabilitation, sanitizing regimen of the artificial blood circulation and others. The organizational aspect includes the necessity of active surgical tactics in treatment of pyo-surgical foci of cardiac localization. Successful treatment of this category of patients can be performed due to the availability of special antiseptic centers which must closely cooperate with cardiosurgical departments functioning in the regimen of emergency diagnostics and cardiac surgery.

[Plasmapheresis and the immune system in heart surgery patients].

An analysis of results of investigation of the influence of apparatus plasmathrombocytopheresis on the immune status of 29 patients with heart diseases is presented. It was shown that intensive plasmapheresis was not followed by substantially decreased level of IgA, IgM and C3 fraction of the complement. The curative effect of this operation in cardiosurgical patients manifests itself as decrease of the level of circulating immune complexes.

[Surgical treatment of septic endocarditis in drug addicts].

The clinical picture of endocarditis in addicts is characterized by mainly right-sided localization of the process in the heart, poor auscultatory picture of the heart, pleuro-pulmonary manifestations against the background of a feverish course of the disease. The leading role in diagnosis of infectious endocarditis in addicts should be given to echocardiography. A description of three cases of surgical treatment of infectious endocarditis in addicts is presented.

[Use of cryopreserved autologous thrombocytes in cardiac surgery].

Cryopreserved autologous thrombocytes were used for the prevention of thrombohemorrhagic complications after operations with extracorporeal circulation in 17 patients. An obvious hemostatic effect was produced. Studies carried out in cardiosurgical patients showed satisfactory maintenance of the main hemostatic characteristics of the blood platelets during cryopreservation. Plasma thrombocytapheresis in an apparatus allows simultaneous preparation of the autothromboconcentrate and large amounts of autoplasma used for filling the extracorporeal circulation apparatus, and compensation for the blood loss.

[Odontogenic septic endocarditis].

In the operative treatment of 34 patients with septic endocarditis the identic microflora was isolated from odontogenous and cardiogenic foci of infection in 35% of the patients. Surgical sanitation of foci of odontogenous infection immediately before operation on the heart contributed to prophylactics of postoperative complications and recurrences of the main disease.

[The choice of surgical intervention in mitral defects].

Under analysis were clinical data and roentgencardiometric parameters in 181 patients with mitral diseases. The most informative roentgencardiometric parameters were established and a quantitative diagnostic algorithm was created with the help of a mathematical method for the choice of the operative procedure. The correct choice was made in 79% of the cases. The diagnostic algorithm developed is thought to be most expedient for use in patients with combined mitral defects. The procedure of quantitation of cardiometric parameters and summing their diagnostic coefficients is simple and not time-taking.

[Cardiac abscesses].

The author describes the experience gained with the clinical and necropsy data on heart abscesses in patients with active infectious endocarditis. Reviews the problems of the pathogenesis and some criteria of lifetime diagnosis of abscess formation in different parts of the heart. Out of 98 patients operated on for active infectious endocarditis, abscesses in the heart were found in 21 patients. Of these, 13 demonstrated good long-term results of the treatment. Emphasis is laid on the necessity of devising a special method for lifetime recognition of heart abscesses.

[Sparing approach to the heart in surgery employing artificial circulation].

A technique of the right-side anterio-lateral thoracotomy without a sternotomy is described. An analysis of operations with this access for different heart diseases in 154 patients has shown this access to be less traumatic and followed by less amount of postoperative complications.

[Infectious intracardiac complications of gunshot wounds of the heart].

The author describes two cases of gunshot wounds of the heart complicated by an abscessing form of an infectious septic endocarditis. The disease resulted in the destruction of the mitral valve with multiple arterial embolism in one case and in the perforation of the interventricular septum in the other. The patients were operated on. Remote results were good.
