[Value of computer tomography and lymphography for staging of malignant testicular tumors].

Findings of computer tomographic and lymphographic examinations of 74 patients were evaluated concerning their evidence in the pretherapeutic staging compared with the histological results of lymphonodectomy. The security of the computer tomography was about 82.4%, the specifity about 95.12%, the sensitivity about 66.66%. Lymphographically, a security of 85.14%, a specifity of 95.12% and a sensitivity of 66.66% was achieved. The combination of the methods achieved a security of 91.7%, a specifity of 100% and a sensitivity of 71.72% was achieved. Metastases of lymphatic nodes less than 0.5 cm diameter were not recognized by means of the two methods. In 2 cases no metastases, which were not covered by means of computer tomography, could not be recognized lymphographically in slightly enlarged lymphatic nodes. In contrast to this in 11 out of 74 patients (14.8%) in the computer tomography an enlargement of the findings was the result in comparison to lymphography. Since none of the two methods is absolutely reliable for the proof or the exclusion of smaller metastases, in a secure evidence of the computer tomography before a lymphonodectomy the lymphography can be omitted.

[Percutaneous transluminal angioplasty of carotid stenoses].

The use of percutaneous transluminal angioplasty for the treatment of stenoses of the carotid artery in three patients is reported. In two cases, the artery could be dilated to its original lumen and in the third a significant dilatation was achieved.

[Importance of post-therapeutic computer tomography follow-up studies of patients with malignant testicular tumors].

120 patients with malignant tumours of the testicles were subjected to follow-up studies for up to 7 years after the completion of therapy. In 27 cases (22%) metastases were found, which appeared almost exclusively within the first 3 years after treatment. There was definite evidence of metastasis in 9 out of 46 cases (19%) of seminoma and with 18 out of 74 nonseminomatous tumours.

[Generalized angiomyolipomatosis within the framework of Bournville-Pringle disease--urological aspects].

2 cases of angiomyolipomatosis of the kidneys are taken as the basis for presenting the symptoms, diagnosis and therapy of this rare disease. In one case there was also an angiomyolipomatous change in the retroperitoneal lymph nodes. In differential diagnosis this gave rise to a suspicion of metastasis or a malignant system disease. In another case there was a coincidence with cystic degeneration of the kidneys.

[Results of angiography studies of the solitary kidney].

Renovasography is better able than any other diagnostic method to distinguish the lack of one kidney from other anomalies and diseases which can give a false impression of this condition. In the retrospective evaluation of the renal angiograms of 3 858 patients, solitary kidney was diagnosed in 170 cases. In 123 cases the tentative diagnosis of agenesia or aplasia was confirmed by intra-aortic injection of contrast medium due to the apparent lack of the renal artery and the nephrographic effect with smooth aortic walls. In another 27 patients short remnants of the renal artery were found as the expression of a thromboembolic arteriosclerotic obstruction and 20 patients had a solitary kidney as the result of a nephrectomy. The discussion goes into the angiographic findings as regards age, sex, side, vascularization, variations of size and additional diseases of solitary kidneys.

[Correlation of computer tomographic, kidney vasographic and histological study results in solid kidney tumors].

Computertomography evaluates renal expansion precisely. In 105 patients differential diagnosis by means of computertomography was 95% accurate. In 93% of the cases it agreed with renovasographic and histological findings. It was possible to demonstrate or exclude the intrusion of a tumour into the renal vein in 61% of cases of tumour of the left kidney and in 55% of growths on the right side. The computertomographic evaluation of the vena cava inferior was confirmed by the histological findings in 71% and 65% of the cases, respectively. It was possible to exclude lymphogenous metastasis in 89% of the cases. In 4% computertomography gave falsely positive findings. In so far as computertomographic examination can give an unambiguous evaluation of a renal growth and a certain proof or disproof of the intrusion of a tumour into the venous system is made, preoperative renovasography can be dispensed with. Information on the operability of the tumour should be supplemented by clarification of lymphogenous and distant metastasis.

[Post-traumatic hemobilia].

Diagnosing posttraumatic haemobilia is difficult since the symptoms are neither specific nor early or easy to detect. At present the mortality rate comes up to 20%. Clinical signs and diagnostic procedures are described in detail. Selective ligation of the liver arteries or partial resection of the involved lobes of the liver seem to be the best treatment. 3 cases are presented.

[Results of computer tomographic and lymphographic diagnosis of malignant testicular tumors].

89.5% accuracy was achieved using the pre-therapeutic combination of CT scanning and lymphography to diagnose the spread of malign testicular tumours in 28 cases. In 14% of the patients CT scanning was able to increase the diagnostic information. CT scanning should follow lymphography. The use of CT scanning to check on progress proved useful in 42 cases, both after lymphonodectomy and after irradiation therapy. In 7 cases (17%) the CT examination revealed metastasis.

[Quantitative determination of regional cerebral circulation with Xe-133: lst experiences using this method in the clinic].

After injection of the radioactive noble gas Xe133 into the A. carotis interna, the regional blood flow in the brain is determined with a 10-channel measuring unit. From the first clinical evaluation of the findings in 35 patients, the following conclusions can be drawn: 1. In epileptics, a localised hyperaemia very probary indicates a latent increased convulsive activity. 2. The measurement of the cerebral blood flow does not yield a contribution to the diagnosis of the type of cerebral tumours. 3. As a consequence of mass shifting of space occupying processes particularly endangered brain sections, also remote from the focus proper, show a localised pathological hyperaemia (morbid "luxus perfusion"). 4. Of greatest importance is the method for clearing up of pathophysiological relations, the clearing up of questions in connection with the blood/brain barrier function, and for the indications for and estimation of the results of vascular-surgical interventions.

[Results of computer tomographic diagnosis of the kidney].

The diagnostic value of CT scanning in kidney diseases was checked on the basis of a retrospective evaluation of the findings of 400 examinations. The significance of expansive processes could be established, with the exception of a blood-filled cyst, which was classified as malign. Even in cases of urinary stasis and unilateral renal non-functioning a comprehensive clarification of the pathological finding was possible.
