Is routine antenatal booking vaginal examination necessary for reasons other than cervical cytology if ultrasound examination is planned?

Sixteen ovarian cysts, including one borderline ovarian malignancy, and ten trichomonal vaginal discharges but no cases of cervical incompetence or ectopic pregnancy had been detected at booking vaginal examination in 11,622 consecutive antenatal case records abstracted retrospectively. In an attitude survey of pregnant women 77% believed that vaginal examination was reassuring, 55% found it unpleasant, and 18% thought it could cause miscarriage. This, and a review of the literature, suggests that routine vaginal examination if ultrasound is planned has few advantages beyond the taking of a cervical smear.

A quantitative structure-activity relationship study of 3-hydroxy-3-methyl-glutaryl-coenzyme A reductase inhibitors.

A quantitative structure-activity relationship study has been made of some 3-hydroxy-3-methylglutaryl-coenzyme A (HMG-CoA) reductase inhibitors. The HMG-CoA reductase inhibition activities of mevinolin analogs and 6-substituted 4-hydroxypyran-2 ones have been mostly found to be significantly correlated with the molecular size of substituents. In one case, however, the inhibition potency was found to be related to the hydrophobicity of molecules. These findings led us to suggest that the enzyme HMG-CoA reductase possesses an active site which is involved in dispersion interaction and another site which is involved in hydrophobic interaction with inhibitor molecules, depending upon the proper orientation of the latter towards these sites. Furthermore, the results indicate that both active sites possess limited steric bulk tolerance.

A structure-activity relationship study on papaverine analogs.

The structure-activity relationship of papaverine analogs, the inhibitors of adenosine cyclic 3', 5'-monophosphate (c-AMP) phosphodiesterase, is discussed. The enzyme inhibition activity of these compounds are found to be dominantly controlled by hydrophobicity and steric factors. A significant quantitative correlation has been obtained between the inhibition activity and the van der Waals volume.

Isolation and mutation of cellulolytic fungi.

Nineteen fungi were isolated from different soil samples on the basis of clear zones formed on Rose Bengal Cellulose agar medium. In shake flasks th isolate K1 gave 12.1 units/ml of CMCase activity. A mutant of the isolate K1, KM7, was selected after N-methyl-N'-nitro-N-nitrosoguanidine treatment of the wild-type. This mutant differed morphologically from the parent strain on RBCA medium and gave 36.2 units/ml of CMCase activity which represented about 50% of the enzyme yield from the standard organism, Trichoderma viride QM 9414 (80 units/ml of CMCase activity). The isolate K1, which was identified as a Phoma species, produced 48 units of beta-glucosidase. The yield of beta-glucosidase was increased about 8-fold in the mutant KM7 and was about 68% higher than the level found in T. viride QM 9414.

Serum protein polymorphisms in goitre.

Serum albumin, ceruloplasmin, transferrin and haptoglobin were studied in 50 patients with both toxic and non-toxic forms of goitre. Albumin was monomorphic in all cases and its amount was substantially lower than normal in two patients. Ceruloplasmin was both qualitatively and quantitatively normal in all patients. No genetic variants of transferrin were encountered but 22% of the patients had this protein, at levels barely detectable by electrophoretic means. Such cases of hypotransferrinaemia occurred oftener in patients with the toxic form of goitre than in those with the non-toxic one. The distribution of haptoglobin types in patients was much the same as in the control blood donors. However, the patients had a greater incidence of ahaptoglobinaemia.

Properties of cellulase from Trichoderma viride.

Cellulase produced by Trichoderma viride acted on carboxymethyl cellulose with a Km of 4.9 g substrate per litre, showing a pH optimum at 4.5 and a temperature optimum at 55 degrees C. Ag+, Hg2+, Zn2+, Cu2+ and N3- were inhibitory..

Jejunal immunopathology in amyotrophic lateral sclerosis and multiple sclerosis. Identification of viral antigens by immunofluorescence.

Jejunal mucosa from 7 patients with amyotrophic lateral sclerosis (A.L.S.), 20 newly reported patients with multiple sclerosis (M.S.), and 35 control patients without either disease was studied by immunofluorescence. An immune reaction was present in all A.L.S. specimens and consisted of altered ratios of immunoglobulin-labelled cells in the lamina propria, complement-labelled cells in the same location, and, in some, immunoglobulin and complement deposits in the epithelial basement membrane. Poliovirus antigen was detected in 4 cases, and in 1 of the these cases measles antigen was also present. A fifth specimen showed large amounts of herpesvirus antigen. In 2 cases studied at necropsy, both viral infection and immunological change was confined to the proximal jejunum. Measles antigen was identified in every case of M.S., and in biopsy specimens from 16 of the 20 M.S. patients immunological reactions similar to those seen in A.L.S. were present. With 2 exceptions, the controls did not show these changes in the jejunal mucosa. The exceptions were a patient with Friedreich's ataxia, who had an increase of IgG-labelled cells and some complement-bearing cells in the lamina propria, and a patient diagnosed as having non-tropical sprue, in whom large quantities of herpes antigen were seen.

Multiple sclerosis and malabsorption.

Malabsorption tests were studied in 52 patients with multiple sclerosis. The stools were examined microscopically for fat and undigested meat fibers and were found to be abnormal in 41.6 and 40.9% respectively. Abnormally low five hour excretion of d-xylose was demonstrated in 26.6% cases. Malabsorption of Vitamin B12 was found in 11.9% cases. The jejunal mucosa was examined histologically and by tissue immune technic including viral studies. Histology showed normal mucosa in all except seven patients in whom an increased inflammatory infiltrate was present. Fluorescent antibody studies revealed the presence of measles virus antigen in all patients and immunofluorescent studies showed a variable degree of immune reaction in the majority of cases. The significance of these findings in the pathogenesis of multiple sclerosis is discussed.
