[Epidemiologic study by interview on carcinoma in situ of the cervix uteri].

From 1950 to 1979, 1248 patients with cancer in situ of the cervix uteri were treated at the Department of Gynecology, Cancer Institute Hospital, Tokyo. Information was obtained from 585 of these patients by direct interview. Many patients revealed early experience of first sexual intercourse and many had had more than two sexual partners and a large discrepancy between the age at first sexual intercourse and at first marriage. Early experience of first sexual intercourse, marital status and the number of sexual partners all seem to have a relation to the development of cancer in situ.

[Therapeutic results of malignant ovarian tumor].

One hundred and fifty patients with malignant ovarian tumors were treated in the Cancer Institute Hospital between 1949 and 1977. Most (103; 68.7%) of the patients had simple primary ovarian cancer. A retrospective study was performed in these 103 patients to investigate the relationship between the 5-year survival rate and the clinical stage and primary treatment. The 103 patients were classified according to FIGO criteria. The survival rates for stages I, II, III and IV were 73.7%, 50.5%, 17.4% and 0%, respectively. As the primary treatment, operations were performed on 92 of the 103 patients. To conclude, of primary importance in the treatment of ovarian cancer is the surgical, complete as possible removal of the tumors. Postoperatively, radiotherapy and chemotherapy should be used. Recently, second look operations have come into wider use. General procedures for the surgical and postoperative treatment of progressive ovarian cancer have been established. Although surgical procedures for the treatment of early ovarian cancer have also been established, there are no consistent procedures for postoperative treatment. Since the incidence of ovarian cancer is increasing, the development of appropriate methods for early diagnosis and treatment of this carcinoma is eagerly awaited.

[Preoperative diagnosis for stage II endometrial carcinoma using endocervical conization].

It is generally difficult to identify stage II endometrial carcinoma correctly. To differentiate cervical involvement, endocervical conization was performed on 31 patients with endometrial cancer between May 1982 and September 1983. The preoperative diagnosis of stage II endometrial carcinoma was confirmed by postoperative histologic examination. In stage I patients, there was no microscopic cervical involvement after the operation. Fractional curettage and hysteroscopy are commonly used to detect stage II endometrial carcinoma. However, these examinations may overlook cervical stromal invasion. Endocervical conization is a simple procedure and highly useful to detect cervical stromal involvement. We recommend the of endocervical conization combined with hysteroscopy to diagnose stage II endometrial carcinoma.

[Review on diagnosis and treatment in cancer of the uterus].

Diagnosis and treatment for the uterine cancer since 1949 were reviewed. During the period of 1949 to 1975, a total of 6986 cases of cervical cancer, including stage 0, were treated. Whereas the five year survival rate for the first five years was 56.3%, it rose 80.8% for the latest five years. This improvement depends mainly on the marked increase in stage 0 cases. There is not so much difference in survival rate between surgery and radiotherapy. Adequate therapy for the recurrent diseases, which were found by the follow-up examinations after the primary treatments, contributed to 2.24% raise in five year survival rates. This fact shows the important role of the periodical follow up examinations for all patients previously treated. As the endometrial cancer has been gradually increasing in Japan, the necessity for paying attention to the early detection were pointed out.

Primary malignant melanoma of the vagina: study of four autopsy cases with ultrastructural findings.

Four cases of primary malignant melanoma of the vagina in women aged 23, 44, 51 and 65 years are presented. In these 4 cases, thorough clinical and postmortem examinations ruled out the possibility of a primary melanoma elsewhere. The primary tumors showed exophytic growth with superficial ulceration. Three of the melanomas arose from the middle third of the vagina and one from the upper third. Melanin was visible in sections stained with hematoxylin and eosin in 3 of the tumors. In the other one, the first biopsy failed to reveal melanin. However, the second biopsy performed following irradiation showed abundant melanin pigment. Electron microscopic examination of 3 tumors revealed premelanosomes and melanosomes in the tumor cells, thus confirming the diagnosis. Two neoplasms showed atypical histologic features, and only the presence of melanin enabled us to make diagnosis of malignant melanoma. One melanoma was associated with an adjacent widespread intraepithelial component of superficial spreading type indicating its probable mode of origin. All 4 patients died of widespread metastases within 13 months after initial treatment. These 4 cases, in which clinical diagnosis was confirmed by thorough autopsy, strongly indicate that malignant melanoma can arise directly from the vagina.

Histochemical study on lipid metabolism in acute myocardial infarction.

As a part of the series of studies on lipid metabolism in ischemic myocardium, the present study was attempted to analyse 29 infarcted hearts with acute or scar lesions by a routine histopathology and lipid histochemistry. The time after the onset of attack till death ranged from one day to years. Sixteen were of the acute form, less than 2 month-old, and 13 were of the chronic scarred form. Within the first day, fat deposition appeared in the survived muscle cells around the infarcts reaching the peak from 2 days to one week, but necrotized cells never contained fat droplets. Macrophages in granulation tissue in periphery of the infarcts tended to have significant fat droplets usually in 10 days to one month. Fat disappeared from the survived muscle cells and also from the scar later than 2 months. The survived cells around the infarcts might be degenerated with fat, probably triglyceride, which accumulated by a relative ischemia not severe enough to produce coagulation necrosis, and they bear some relationship with extension or limitation of size of infarcts.

Childhood coronary sclerosis.

Childhood coronary sclerosis was studied histologically, histochemically immunohistochemically and biochemically on 60 autopsy cases aged from new-born to 20 years. Intimal thickening often started even in the newborn babies particularly at branching or bending points of the arteries, where fibrocellular proliferation with acid mucopolysaccharide accumulation was the main histological feature. Microthrombi were occasionally demonstrated even in infants, and fatty streaks appeared generally in the second decade. Microthrombi were usually smaller and fewer than fatty streaks. Beta-lipoprotein and fibrinogen infiltrations into intima were proven in some cases. Mural vascularization was occasionally observed especially in the areas beneath intimal thickening. Pressure effect of the blood stream and anatomically specific points in the arteries seemed to be important as initial factors of coronary sclerosis, where plasma constituents might pass in an easier manner into intimas by increased permeability. Secondarily mural capillaries might play some role in developing atheromas which would produce an eccentrically located luminal obstruction later in adulthood, by local circulatory disturbances within the arterial walls.

Circumscribed carcinoma of the uterine cervix, with marked lymphocytic infiltration.

The present study was carried out to investigate a distinctive type of carcinoma of the uterine cervix categorized under the designation as circumscribed carcinoma with lymphocytic infiltration. Grossly this carcinoma is characterized by defined circumscription with a superficial ulceration. A microscropic feature characteristic of this tumor is the presence of a loose fibrillary stroma infiltrated densely and uniformly by lymphocytes throughout the tumor mass. The tumor is arranged in solid cords separated by a lymphoid stroma with evidence of minimal squamous differentiation. The authors picked up 709 patients with carcinoma of the uterine cervix who underwent radical hysterectomy and pelvic lymphadenectomy at the Cancer Institute Hospital, Tokyo, between 1956-1967, without preoperative radio- or chemotherapy. Among them, 39 or 5.5% were identified as this type of carcinoma. Patients with this types of carcinoma had a significantly better prognosis than those with other types of cervical carcinoma of the same stage (p less than 0.05). This favorable prognosis is probably due to the less regional node metastases found in the group. A proposal was made to segregate this particular type of carcinoma from other types of cervical carcinoma on the basis of its morphologic and prognostic distinctiveness.
