A clinical and quantitative microbiological study was performed on 32 women with fishy odour of the vaginal fluid. This odour could be intensified by the addition of one drop of 10% potassium hydroxide. Clue cells were found to be present in all cases. Gardnerella vaginalis was isolated from the vaginal fluid in 97%, and large quantities of mixed anaerobes in 100% of the patients. The therapy with metronidazole, 400 mg twice daily for 5 days, was successful in 22 out of 24 patients. After therapy the fishy odour and the clue cells were no longer detectable. G. vaginalis and anaerobes could only be isolated sporadically and in small quantities. The isolation rate of Lactobacillus species increased from 3% to 86% within one week.
In 30 patients with genital infections of different etiology and high fever lamoxactam was used as a single agent antibiotic. In 90% of the cases the treatment was successful. The antibiotic did not need to be changed in any of the cases. The determination of the lamoxactam concentration in the uterus and the fallopian tubes after a single dose of 2 grams pre-operatively at various times showed high tissue levels for a prolonged period of time. The concentrations were still higher 6 hours after the single dose than the in vitro MHK values for most bacteria important in gynaecological infections.

Studies have shown, that the normal flora of the upper airway harbors many different anaerobic bacteria. They play an important role in many chronic diseases such as chronic sinusitis, chronic secretory otitis and chronic tonsillitis. On the other hand they may cause acute situations such as pharyngeal abscess, Ludwig's angina, Vincent's angina or space infections in the neck. The bacteria most frequently reported from infected sites are Bacteroides sp., anaerobic cocci and fuso-bacteria. Bacteroides fragilis plays a minor role. Anaerobic pathogens should also be considered in patients with major surgery of the head and neck following radiation therapy. This concept leads to a considerable reduction of major and minor surgical complications.
Water samples which were obtained from 21 ENT examining units were studied for the presence of bacteria. 10(4)--10(6) colony forming bacteria per ml were found in 13 of the units, while an additional 10(2)--10(4) organisms/ml were found in 8 units. Pseudomonas species were found in all samples and primarily involved Ps. acidovorans and Ps. paucimobilis.
Micro-ID, a new kit system for rapid identification of Enterobacteriaceae, was compared with a conventional media-system, the extended Enterotube system (completed with arabinose, rhamnose and the Voges-Proskauer reaction), and partially with the API-20E system. When the computer generated Identification Manual for Micro-ID was consulted the system showed complete agreement at the genus level, but differed in 5% at the species level. Problems arose from misidentifications by nonfermenting organisms. 15 turbid blood cultures with gramnegative rods were tested additionally. In 11 cases the Micro-ID provided correct identification of the species and in further 3 cases of the genus. In summary Micro-ID is easy to handle and an accurate, convenient kit for the short-term identification of Enterobacteriaceae.
A number of studies show that only a few bacteria are dominating the infectious sinus diseases. In acute sinusitis hemophilus and pneumococci are prevalent. Recent results in chronic sinusitis indicate a greater importance of anaerobic bacteria. The bacteriological findings in sinusitis are influenced by many factors: 1. the sampling techniques (proof-puncture or nasal-swab); 2. transport time and transport medium; 3. bacteriological culture technique; 4. the isolated bacteria are not automatically pathogens. The difference between colonizing or contaminating bacteria and pathogenic bacteria should be taken into account. A lack of leukocytes, a mixture of bacteria and a viable count of 10(3) or lower indicates normal flora or contamination; 5. kind of infection: acut - chronic; dentogen - rhinogen; 6. mode and duration of therapy (antibiotics, surgery, irrigation); 7. age of patient, season of the year.
Six neurosurgical patients were given 2-4 g cephacetril intravenously at six hourly intervals. Serum and cerebrospinal fluid (CSF) concentrations were determined. Cephacetril activity was not detectable in the CSF when the blood-CSF barrier was intact. In two patients with increased CSF cell counts cephacetril was found in the CSF in low concentrations (0.1-0.8 mcg/ml). Our investigations show that cephacetril does not penetrate into the CSF unless there is an inflammation or other impairment of the blood-CSF barrier. Thus we could not confirm the results of some authors who found cephacetril in therapeutically effective concentrations in the CSF of healthy persons.
In 7 dogs renal arterial embolization was performed using the Seldinger technique and an inflatable Swan Ganz balloon catheter. Methyl methacrylate and butyl-2-cyanoacrylate were injected. 6 months later, the tissue concentration of Cefazolin was measured in the embolized and contralateral kidneys and in serum 2 h after a short infusion of 1 h Cefazolin. The concentrations in serum and homogenized kidney tissue were determined by means of the agar gel diffusion method. Angiography showed no vessels in the embolized kidneys. Histologically there was total atrophy of the tissue, especially of the tubular system. The mean tissue level of Cefazolin in the embolized kidney was 70% of serum. The tissue concentration was always above 10 micrograms/g. This concentration is high enough to inhibit nearly all strains of E. coli, Klebsiella, Salmonella, Shigella and a large part of Proteus mirabilis, whereas nearly all gram-positive bacteria are inhibited at a lower concentration. The value and necessity of antibiotic therapy in embolized kidney cancer is discussed.

In ten patients continuous estimations of the C.S.F. and serum levels were performed after the administration of a Cephalosporin derivative (Cefazolin - 4 cases) and a new alpha-amino-glycoside antibiotic (Sisomicin - 6 cases). After the evaluation of the results some doubts must be cast on the value of both these antibiotics for the treatment of meningitis or for per-operative chemo-prophylaxis in neurosurgical procedures. The values found were compared with those well-known in the literature, regarding the cephalosporins and the alpha-amino-glycoside Gentamycin.
Under general anaesthesia cultures were taken from infantile tonsils prior to T & A. The bacteriological results and the serological classification of hemophilus are questionning literature reports. According to our findings one must consider haemophilus influencae as a resident of the common oral flora. There was no significant difference culturing tonsils with or without repeated antibiotic treatment. Hemophilus influencae combined with beta-hemolytic streptococci is common. Immunological considerations against infantile T & A, based on bacteriological observations are questioned.
Tissue concentrations of cefazolin were measured in 18 patients undergoing urological operations. Specimens of skeletal muscle and blood were taken simultaneously at predetermined intervals. The concentrations in serum and homogenized skeletal muscle were determined by means of the agar well diffusion method. A comparison of standard curves obtained with phosphate buffer solution and the supernatant of muscle homogenate revealed no substantial binding of cefazolin to the supernatant of muscle homogenate. Because of methodological difficulties in determining the different compartments of a tissue specimen concentrations in tissue were not corrected. Within two hours after a short infusion (25 min.) of 2 g of Cefazolin in muscle tissue a peak concentration of 20 mug/g was reached. A tissue level above 10 mug/g maintained for three hours. This concentration is high enough to inhibit nearly all strains of E. coli, Klebsiella, Salmonella, Shigella and a large part of Proteus mirabilis, whereas nearly all gram-positive bacteria are inhibited at a lower concentration.

It is well known that long transport diminishes the chances of isolating causative micro- organisms, especially from swabs. In a trial, in which beta-haemolytic streptococci were kept for 24-hours at room temperature, culturing failed even if more than one million bacteria had been placed on the swab. Culture medium consisting of agar with 0.5% human albumin made it possible to isolate beta-haemolytic streptococci even after transport and with a count of less than a hundred micro-organisms per swab. The use of selective nutrient media may be necessary because of the sometimes marked multiplication during transport when there are mixed organisms. The nutrient medium was used during six small epidemics of scarlet fever in Freiburg and proved superior to the usual dry swab. In parallel studies of patients and contacts the causative beta-haemolytic streptococcus was isolated three times as often.
