[Results of combined surgical and radiological treatment of bladder cancer with particular reference to local tumor recurrence].

The combined surgico-radiological treatment of the carcinoma of the urinary bladder has clearly improved the results of treatment. The local number of recidivations after this therapy is, however, still very high and in the own number of patients is about 50%. Most recidivations appear briefly (1 to 2 years) after the end of the radiological treatment and are spread above all multilocularly in the bladder. Thus an additional more radical surgical therapy is indicated. In the planning of the primary therapy, therefore, the scarcely reduced treatment under conservation of the bladder should be taken into consideration, since the recidivations decisively determine the further prognosis of the patient.

[Influence of various physical forms of the feed on the energy metabolism of ruminants. 3. Investigation of the energetic utilisation of rations with roughage treated with various mechanical methods].

In a total of 170 experiments with the complete metabolism of fullgrown sheep the energy utilisation of rations with roughage treated with different mechanical methods was investigated. The roughage feedstuffs dried green fodder of vetch/grass, lucerne and rye grass and straw were given to 4 parallel animals in 9 rations with differing quotas of concentrates (0 to 60%). The roughage underwent the following 5 methods of treatment: 1) chopping; 2) chopping and pelleting; 3) grinding; 4) grinding and pelleting; 5) grinding and pelleting with concentrates. The results show that the bases given in the GDR system of feedstuff evaluation concerning the energetic evaluation of feedstuffs are to be applied to mechanically treated roughage too. In order to avoid negative effects on the energy metabolism one has to take care that the use of mechanically treated roughage does not cause dysfunctions of the rumen.

Interaction of oleficin with the inner membrane of rat liver mitochondria.

The effects of oleficin, a polyene antibiotic of the nonmacrolide type, on isolated rat liver mitochondria were studied. Oleficin at a concentration of about 10 nmoles/mg protein increases both the rate of state 4 respiration and the "basal" ATPase activity of mitochondria. In contrast to this it inhibits the rate of both state 3 and uncoupled respiration and the DNP-stimulated ATPase activity. These inhibitions can be prevented by low concentrations (2 approximately 5 mM) of magnesium ions. Oleficin induces a high amplitude swelling of non-respiring mitochondria in the isoosmotic nitrate and chloride solutions of K+, Na+, Tris+, Tea+ or Mg2+. In contrast to that it does not induce swelling of mitochondria treated with ruthenium red in isoosmotic calcium acetate. Indirect evidence suggests that oleficin increases also the proton permeability of the inner membrane. The swelling observed in the isoosmotic solutions of monovalent cations can be prevented by low concentration (2 approximately 5 mM) of Mg2+. In the presence of the antibiotic Mg2+ and Ca2+ but not K+ and Na+, are transferred from an aqueous phase into a butanol-toluene bulk phase. Oleficin depletes Mg2+ and Ca2+ from mitochondria in a concentration dependent manner. Complete depletion of Mg2+ occurs only in the presence of EDTA, while that of Ca2+ does not need the chelator. It is concluded that the effects of oleficin on mitochondrial functions can be explained on the basis of an increase of the inner membrane permeability as the consequence of the depletion of Mg2+ from mitochondria caused by the antibiotic.

[Advanced malignant kidney tumors with the clinical picture of septic kidney].

At the instance of 4 patients the differential-diagnostic difficulties between advanced malignant renal tumours and the so-called septic kidney are demonstrated. In fever conditions in connection with unclear urographic findings, in particular with the so-called dumb kidney, it is right primarily to search for a septic renal disease. In spite of urgency of diagnosis and above all of the therapeutic consequences resulting from this the presence of an advanced malignant renal tumour must be included in the differential diagnosis also when there are no leading tumour symptoms. This demands the rapid use of a highly specialised radiologico-urologic diagnostics. Many clinical and paraclinical findings are differential-diagnostically without importance, since they may be typical for the two pictures of the disease. A certain preoperative diagnosis should be achieved above all on account of the differentiated operative approach and the operative way of access.

[Differential diagnosis of pyeloureteritis cystica, follicularis et granularis].

At the instance of the observations of two patients the differential-diagnostic difficulties in the pyeloureteritis cystica and the ureteritis follicularis or granularis are described. Here first of all the following other diseases must be excluded: 1. neoformations in the upper urinary tract, 2. uro-tuberculosis, 3. X-ray-negative calculi, 4. HWI with gas-producing germs. The knowledge of the X-ray diagnostic criteria of the individual diseases is often of life-important significance for making a certain diagnosis for the patient. For this reason also the performance of a retrograde ureteropyelogramme may be indicated, if, exhausting all possibilities of the excretion urography, a diagnostic ascertainment is not possible.

[Utilization of feed energy by growing pigs. 3. Energy requirement for the growth and fattening of pigs].

The test series for the investigation of the energy consumption of growing pigs of the breeds large white and improved land race pig as well as cross breeds of the two breeds in a total of 369 metabolism periods (as described in the first two pieces of information of this publication series -- Hoffmann and others, 1977 and Jentsch and Hoffmann, 1977) were statistically analysed for the purpose of the derivation of the energy requirement for maintenance and the partial energy requirement for growth in order to test the possibilities of the factorial analysis for the derivation of energy requirement values of growing pigs. The dependence of the maintenance requirement of growing pigs (investigations in the live weight range of 10 to 40 kg -- see 1st information--were made with boars those in the live weight range of 30 to 120 kg were made with gelded boars, 2nd information) on the live weight can best be characterised by applying a power exponent of 0,61 or 0,62 for the live weight. A definition is offered to be discussed for the energetic maintenance requirement of productive live stock and laboratory animals as a conventional value. The energy requirement values derived from the doubly-factorial statistical analysis show a satisfactory adaptation to the measured values as such concerning energy intake and observed growth performance of the test animals. The conclusion is drawn that the factorial analysis of the energy requirement (maintenance plus partial performances) results in a better estimate of the requirement of growing animals than the assessment according only to live weight and live weight increase without characterising the energy requirement for partial performances. This is important for the further working on and more exact definition of requirement norms.

[Compliance and active cooperation of patients in preventing recurrence of urinary calculi].

An anonymous questioning of 200 patients with urinary calculi concerning the observation of the general and special measures of the metaphylaxis of urinary calculi resulted in the fact that in particular the measures of the general metaphylaxis (bodily movement, diet, dilution of the urine) are not or only parlty obeyed by a large part of the patients. The examinations show that for the improvement of the understanding of the disease and the behaviour of the patients concerning their health further on a great attension must be paid to the quality of the information by the physician and to the communication between physician and patient. An insufficient cooperation of the patients in the metaphylaxis of the urinary calculi would call in question the effectivity of all efforts and measures concerning the prevention of relapses.

[High doses of thiopental for therapy of post-ischaemic anoxia of the brain. A case report (author's transl)].

The case of a 7-year-old child is presented, who suffered circulatory arrest during induction of anaesthesia for surgery for a posterior fossa tumour. A brain ischaemia lasting 6 minutes duration had to be assumed. After restoration of circulation, 825 mg ethiopenta were administered in order to ameliorate a possible post-ischaemic anoxia of the brain according to a protocol by Safar [18]. 11 hours after circulatory arrest the child awoke. Except for a more pronounced left sided hemiparesis and paresis of the left n. abducens no additional neurological deficit was observed compared to the neurological status before induction of anaesthesia.

[Morbidity patterns in a non-German population (migrant workers) (author's transl)].

Differing patterns of morbidity are demonstrated in a group of 181 foreign patients (from Turkey, Jugoslavia, Portugal, Italy, Spain) compared to an age matched group of native Germans hospitalized during a given time in the meidcal department of a regional urban hospital. In non-Germans peptic ulcers and severe infections more often lead to hospitalization than native patients. The expected statistical frequency of hospitalization is not different in the two groups investigated. Epidemiologic differences between the two populations are demonstrated by a higher percentage of tuberculosis and work accidents in the foreign group. At present there are no special obstetric problems leading to differing maternal or neonatal mortality. Concomitant imported infections do lead to complications in single cases in the foreign patient group. Parasitic infestation is more often found in the foreign group. Herditary diseases such as minor thalassemia or G-6-PD deficiency are rare as well in foreign patients. Chain infections due to imported poliomyelitis wild virus or salmonellosis have been observed in the foreign population. Epidemiologic progress may be highlighted by the decreasing percentage of hepatitis A and B antibody-positive-population in the central European countries.

[Energy metabolism and energy requirements of growing boars].

The nitrogen and energy metabolism and the energy consumption of growing boars were measured in 2 metabolic and feeding trials using 8 parallel animals each. The studies covered the 30 to 150 kg live weight range. The growth intensity of the boars was found strongly influenced by the protein level of the ration. At a crude protein level of 18% in the ration, the boars gained, on the average, 780 g per day during the fattening period under study. Energy conversion was found to decline as the protein amount went up. The energy expediture for protein deposition was estimated at 1.8 to 2.0 kcal metabolizable energy per kg deposited. The energy and feed expenditures were calculated to be 7.1 Mcal net energy--fat retention for the whole development period or 3.0 kg dry matter per kg live weight. Boars proved to have an energy requirement differing from that of barrows and gilts; equations are presented for derivation.

[Digestibility and utilization of rations containing differently treated straw. I. Digestibility by sheep of rations containing differently treated straw].

During 63 digestibility periods of adult sheep, the digestibility of wheat and oat straw sujected to different kinds of treatment was compared. The straw was fed in the following forms: chopped, chopped and treated with 6% sodium hydroxide, pelleted without and with NPN-addition (ammonium bicarbonate and/or urea) during pelleting. Notwithstanding pressing temperatures over 90 degrees C and light moistening of the straw during pellet production, the addition of the two NPN-components did not allow to increase digestibility and energetic feed value. Two replications averaged an increase in energetic feed value by 42% due to NaOH-treatment. Energy digestibility went up by 39 to 55%, and energy concentration increased from 322 to 457 energetic feed equivalents (cattle) per kg DM. In addition, a number of rumen-physiological indices were obtained.
