[Surgical cerclage in the treatment of cervical incompetence during pregnancy (determining the legitimacy of the procedure)].

The authors studied the effectiveness of the cervical cerclage operation in the therapy of cervical insufficiency during the pregnancy. Two Hundred sixteen pregnant women took part in this trial in whose pregnancy the necessity of cerclage operation was not unequivocal. It means, that the therapeutical cerclage operation was not necessary, but the necessity of prophylactic cerclage operation was doubtful as well. From the randomized 216 cases they performed cerclage operation in 108 pregnancies and in 108 cases they did not perform it. In the respect of pregnancy care they did not make any difference between the two groups. The authors from their trial draw the conclusion, that in doubtful cases where the necessity of cervical cerclage operation is not unequivocal--e.g. at serious gestational case history--it is advisable to perform the operation. The danger of premature birth can be diminished in this way.

[The role of different committees and individuals in the ethical position on the new methods of human reproduction].

Due to the research on assisted human reproduction and the application of its results we face new type ethical dilemma which had never existed before. The traditional system of ethical values does not provide a safe basis for judgement of our actions. One of the challenging questions is who has the right to authorize or prohibit performing different procedures. There might be necessary and justifiable taking into account the opinions of various bodies (professional committees, national and institutional ethical committees, religious bodies, parliamentary representatives and private organizations or persons). The authors introduce the activities of the role and importance of these bodies and investigate the position of the research and clinical professionals in obtaining permission for the application of ethically sound or less criticizable procedures. The importance of the provision of continuous and many-sided information for wide range of the society is underlined.

Tissue characterization by transvaginal colour Doppler for the evaluation of gynaecological tumours. 1. Review of the literature.

This is the first review published in Hungary about the usefulness of transvaginal colour Doppler (TVCD) in gynaecology. The history of "ultrasound tissue characterization", the conventional and colour Doppler methods are discussed as well as the pathomorphological basis of abnormal Doppler signals. The summary of tumourangiogenesis, of neovascularization and its manifestation in Doppler spectrum are given. The differences between normal and pathological Doppler signals in female small pelvis are shown. It is concluded that the routine application of TVCD will reduce the rate of false positive results and "luxury" operations from screening procedures.

Tissue characterization by transvaginal colour Doppler for the evaluation of gynaecological tumours. 2. Clinical experiences.

Transvaginal colour Doppler was used to evaluate the blood flow patterns in pelvic vessels in a group of 315 patients including 168 with uterine tumours and 147 with adnexal masses. Neovascularization of malignant tumour tissue was successfully displayed by colour Doppler in the vases of endometrial and ovarian cancers but no abnormal blood supply was observed in the cases of early cervical cancers. A comparison between the characteristics of blood flow within benign and malignant lesions showed lower resistance index in cases of malignancy. The sensitivity, specifity, positive predictive value, negative predictive value and the diagnostic accuracy of this new method in the recognition of endometrial and ovarian cancers are higher than 95%. By the help of transvaginal colour Doppler (together with the classical methods as colposcopy, cytology etc.) it will be possible to establish of complex screening programmes for all types of gynaecological cancers.

[The role of radical abdominal hysterectomy and lymphadenectomy (Wertheim's operation) in contemporary gynecology].

During a 20-year period between January 1964 and December 1983, 317 radical hysterectomies and pelvic lymphadenectomies were performed for stage IB, IIA and IIB carcinoma of the cervix at the Department of Obstetrics and Gynaecology, University Medical School of Debrecen. The 5-year survival rate for Stage IB patients is 88.3 per cent, in Stage IIA 82.7%, in Stage IIB 68.0% and the major prognostic factor clearly being the status of the pelvic lymph nodes. In comparison with the literary data their results seem to suggest that routine pre- and postoperative radiotherapy reduces pelvic metastases and improves survival in patients with positive pelvic lymph nodes. It was noted that closed retroperitoneal suction drainage after radical operation effectively removes fluid from the pelvis and probably prevents some of the postoperative complications. Evidence seems to suggest that properly performed radical pelvic surgery with radiation therapy is successful in treating early cervical carcinoma. There does not appear to be any contraindication to operation in this group of patient as long as the condition is deemed medically operable.

Ovarian function after the menarche and hormonal contraception.

The aim of the study was to determine the date of regular ovulation after the menarche to better understand the physiology of female adolescence, especially as it pertains to the use of hormonal contraception. Early morning urine samples were collected from 51 girls in the perimenarche for 9 weeks semi-annually during 2 years. Estrone- and pregnanediol-3-glucuronide values were determined. Cycles lasting 35-40 days at the onset of menses shortened to 28 days after the 23rd-25th cycle. Menses reached a 5.0-5.5 day average length at about the same time. After the 20th cycle, ovulation could be demonstrated in more than 50% of the study patients. Using a scoring system, regular ovulation could be expected on the basis of somatic data with scores of greater than 16. Hormonal contraception may be prescribed 2 years after the menarche, based on scores of greater than 16 and ovulation proven be hormonal cytology and basal body temperature.

Interruption of pregnancy by cervical rivanol predilatation.

Experience obtained in 1361 cases of first trimester pregnancy interruption with cervical Rivanol predilatation is reported. Rivanol-induced predilatation of the cervical canal prior to interruption was found to be an efficient procedure. The dilating effect of Rivanol could not be enhanced by i.m. administration of 5 IU oxytocin. After the interruption of more advanced pregnancies a higher degree of dilatation of the cervix was observed. The necessity of further dilatation prior to vacuum aspiration is, in the majority of cases, not higher than 2-2.5 H, which is insignificant on the softened cervix. The immediate complications of medical induction and interruption were infrequent and not severe. Thus, cervical predilatation with Rivanol prior to vacuum aspiration is a recommendable method of pregnancy interruption, worthy of widespread application.

Early complications of first trimester interruption of pregnancies with rivanol predilatation.

The early complications of the non-invasive interruption of 1361 first trimester pregnancies induced by Rivanol predilatation were studied. For this purpose the patients were called back for follow-up examination 6 weeks after the operation. The number of patients reporting at the control examination was 769. The number of febrile complications of genital origin was low. Slight postoperative bleeding exceeded the amount of normal menstruation only in 4.9% of the patients and, in general, it did not last longer than 7 days. The same applied to abdominal and lumbar complaints. The proportion of early complications was lower than after interruptions with the D + VA and D + C methods. The first menstruation after the operation did not differ from the earlier menstruation pattern of the particular patient in 75.9% of the cases. This also showed that the use of Rivanol did not involve lasting and severe damage to the endometrium.

Examination of gestations and deliveries subsequent to rivanol-induced interruption of first trimester pregnancies.

The pregnancies and deliveries of 550 parturient women were studied. The women previously had undergone interruption of pregnancy induced by Rivanol predilatation of the cervical canal. As controls served the data of 557 pregnancies and deliveries in which the pregnant women had formerly undergone invasive interruption of pregnancy. In addition the data of further 531 women were studied whose last delivery occurred before term or of a small for dates baby. The examinations showed that the necessitating cerclage occurrence of cervical incompetence was significantly lower in pregnancies following non-invasive interruption than after those following invasive interruption. There were no differences in the start and course of delivery and in the number of complications that could be traced back to the method of interruption. There was no difference in the intactness, site of adherence, separation and gross structure of the placenta. The condition of neonates at delivery was similar in the two groups. On the other hand, in the group of invasive interruptions the proportion of neonates that needed medical treatment was higher. An important observation was that after non-invasive interruption the proportion of preterm and small for dates births was significantly lower. These findings may be important in everyday practice.
