[Chorea-ballism associated with nonketotic hyperglycemia and presenting with bilateral hyperintensity of the putamen on MR T1-weighted images--a case report].

A 55-year-old woman was admitted to our hospital with abrupt onset of chorea-ballism in the right arm and both legs. She had thirst before admission. The blood glucose level was 976 mg/dl, the serum osmolality was 335 mOsm/l, and there was slight ketonuria on admission. Even after normalization of the blood glucose level choreiform movement of the right arm and leg continued. Brain CT showed bilateral and symmetrical high density of the putamen on admission, which diminished at 32 days leaving an area of low density. This suggested that the high density was not calcification. There was no mass affect. Symmetrical high intensity of the bilateral putamen on MR T1-weighted images which was slight on admission increased in intensity after 2 months, but disappeared 4 months after the onset in association with the disappearance of choreiform movement. The abnormality in the putamen was considered to be the cause of her involuntary movements. In nonketotic hyperglycemia, high intensity basal ganglia lesions on T1-weighted MR images have previously not been reported. High intensity in the putamen on T1-weighted MR images in the present case might represent multiple petechial hemorrhages which occurred in association with metabolic disorders including nonketotic hyperglycemia.

[Clinical course of silicotic patients receiving home oxygen therapy].

To investigate the clinical course of patients with silicosis receiving home oxygen therapy (HOT), we compared the clinical data of HOT patients with silicosis and those with other respiratory diseases, and of silicotic patients with HOT and those without HOT in our hospital. In 23 (34.3%) of 68 patients receiving HOT the underlying disease was silicosis. This figure differs from recent official statistics in Japan. In the patients with silicosis, the survival rate was lower than that of those with chronic obstructive pulmonary diseases or old pulmonary tuberculosis, but the degree of decrease in PaO2 was smaller, suggesting that the prognosis of silicotic patients may be mainly related to pulmonary hypertension and cor pulmonale. In silicotic patients, large opacities on chest X-ray were frequently observed in the patients with HOT compared to those without HOT, indicating that the presence of large opacities is an important factor influencing hypoxemia in silicotic patients.

Radiosensitizing, toxicological, and pharmacokinetic properties of hydroxamate analogues of nitroimidazoles as bifunctional radiosensitizers/chemical modifiers.

We examined the pharmacokinetics of KIH-802, potassium 2-nitroimidazole-1-acetohydroxamate, using its radioisotope-labelled compound and the acute toxicity in mice. We discovered that the concentration of KIH-802 was very low in the brain and its LD50 was nearly half the value of that of MISO. We also present here new 2-nitroimidazole radiosensitizers/chemical modifiers (KIN-804, 811, 831, 841, 844, 821, 823 and 824) designed to enhance their sensitizing ability intensely by substituting various biologically active groups, such as hydroxamic acids and oximes, with moderate lipophilicity to the aromatic ring, if necessary, through some spacers. The sensitizing effects of all compounds were estimated to be almost equal to or better than that of MISO. The results of their toxicities shows that new hydroxamates KIN-804 and 831 are less toxic than KIH-802 and MISO. Their in vitro enhancement ratios are 2.00 and 1.75, respectively, compared with those of KIH-802, MISO and SR-2508, 1.77, 1.72 and 1.72, respectively, at each dose of 1 mM for EMT6/KU single cell. We concluded that they may be superior radiosensitizers of hydroxamic acid analogues to KIH-802.

[A case of topographical disturbance following a left medial parieto-occipital lobe infarction].

A 59-year-old man was admitted to our hospital for his sudden-onset right hemianopsia. Thirty days after the onset, neuropsychological examination revealed obvious topographical disorientation and mild optic ataxia. Magnetic resonance imaging showed abnormal intensity area at the left medial parietooccipital region and left splenium of the corpus callosum. Although single photon emission CT showed uptake decrement in the left hemisphere, almost normal uptake was observed in the right hemisphere. He could recognize landmarks, but fail to recognize the relative position of landmarks. Therefore, his topographical disorientation was considered to be due to perceptual disturbance, memory disturbance of relative position of landmarks, or both. He was right-handed with no sinistral relative, and showed dyslexia and dysgraphia early in his clinical course. The laterality index of the dichotic listening test revealed the right ear dominance. These results indicated that his left hemisphere was language dominant. His topographical disorientation could be caused by the medial parieto-occipital lesion in the dominant hemisphere.

Reduced glutathione levels in senile cataractous lens epithelial cells.

A sensitive assay, utilizing high performance liquid chromatography and sulfhydryl (SH) fluorescence labeling, was used for the quantitative determination of reduced glutathione (GSH) and cysteine (CySH) in senile cataractous lens epithelial cells. The capsule-epithelia (CE), obtained following cataract surgery, were soaked in 0.3 ml saline at room temperature for 1 hour. Detached epithelial cells and the capsule with attached residual cells were assayed for GSH and CySH. Regression analysis of the relation between epithelial protein content and capsule wet weight was performed to evaluate the amount of contamination of the CE samples with lens cortex. GSH levels in the cataractous lens epithelial cells were 23.0 +/- 11.2 (Mean +/- S.D.) nmol/mg protein (n = 15); CySH levels were 0.51 +/- 0.50 nmol/mg protein (n = 12). No differences in GSH levels were observed between immature and mature cataracts. Thus, GSH levels in the lens epithelial cells did not appear to decrease with the progression of the senile cataracts.

[A case of gold-induced pneumonitis showing a positive reaction in the drug lymphocyte stimulation test (DLST) for gold].

A case of acute interstitial pneumonia developing during gold therapy is reported. A 67-year-old female with rheumatoid arthritis for about twenty years who received a total dose of 80 mg of gold thiomalate (Shiosol) for about two months, developed high fever and dry cough. Exertional dyspnea developed and chest roentgenogram showed diffuse small nodular and reticular shadows. RA and RAPA tests were positive. Drug lymphocyte stimulation test (DLST) for Shiosol was positive. The dyspnea resolved on administration of methylprednisolone. Chest roentgenogram and CT-scan showed improvement. A total dose of 80 mg of gold thiomalate, as administered in this case, is the minimum dose previously reported in Japan. It has been recently reported that allergic reaction is the mechanism of gold lung. In the present case, positive DLST indicated the existence of many activated lymphocytes, and was very useful in the diagnosis of gold lung.

In-vitro DNA synthesis of keratinocytes in normal human skin, psoriasis, seborrhoeic keratosis, Bowen's disease and basal cell carcinoma.

The DNA-synthesizing cells in benign and malignant keratotic diseases including psoriasis, seborrhoeic keratosis, Bowen's disease and basal cell carcinoma were studied. Cells in S-phase were labelled after small pieces of the lesions were incubated in tissue culture medium that contained 5-bromo-2'-deoxyuridine (BrdU). The labelling of DNA was analysed using the labelling index and pattern. Although the malignant diseases tended to show a higher DNA labelling index, neither this nor the loss of polarity alone was associated with malignancy. However, a higher DNA labelling index together with a loss of polarity was characteristic for malignancy.

[Tolerance to a sustained release form of isosorbide dinitrate: comparison between 20 mg four times and 40 mg twice-daily administrations].

Attenuation of antianginal effects of oral isosorbide dinitrate in a sustained release from (ISDN-SR) were studied using the treadmill exercise test in eleven patients with stable exertional angina and a positive exercise test. We compared the four-time-daily administration of 20 mg with a twice-daily regimen of 40 mg for 10 days using the double-blind, crossover method. On the 1st day of the administration, with both regimens, the treadmill walking time, the time to 1mm and 2mm ST depression, and several hemodynamic parameters increased significantly. These improvements were reduced gradually on day 5 and day 10 after the administration. In the comparison between the four-time-daily and two-time-daily regimens, the former application showed clearer antianginal and hemodynamic effects than the latter regimen, but these effects were attenuated more markedly in the four-time daily administration. With both dosage regimens, the plasma ISDN concentrations increased gradually. With the long-term four-time-daily regimen, the time-to-time changes of the ISDN concentration were smaller each day. These data suggest that tolerance to oral ISDN-SR exists even if it is applied only two time daily, but its degree is less than when a four-time-daily dose in administered.

[A case of whole visual field visuomotor ataxia of bilateral hands associated with bilateral parietooccipital lesions in the white matter].

A 29-year-old male with visuomotor ataxia was reported. Visuomotor ataxia of bilateral upper extremities was noted in whole visual field including central visual field (optische Ataxie). Neither contraction nor depression of visual field was detected by the test using kinetic or static perimeter, respectively. Magnetic resonance imaging and computed tomography of the brain revealed bilateral parietooccipital lesions in the white matter, and pathological examination of brain biopsy specimen demonstrated demyelination. We considered that bilateral parietooccipital lesions in the white matter were responsible for visuomotor ataxia in this case.

Regional and subcellular distribution of ascorbate free radical reductase activity in the human lens.

This paper reports that the human lens is endowed with a characteristic regional and subcellular distribution of ascorbate free radical (AFR) reductase (EC 1.6.5.4) activity, and it is associated with the soluble fraction of the lens protein. AFR reductase activity in either immature senile cataractous or transparent lens was higher in the cortex, where the level of insoluble protein is lower, than in the nucleus. The high reductase activity and low insoluble protein content of the cortex were comparable between the assayed cataractous and transparent lenses. In the nucleus, in contrast, the reductase activity tended to decrease with an increase in the level of insoluble protein and with the development of nuclear coloration (sclerosis). As for the subcellular distribution of AFR reductase activity, 70-90% of the enzyme activity in the lens was located in the cytosol fraction. Based on the above results, it was proposed that cytosolic free radicals may be involved in oxidation, coloration and aggregation of lens protein in senile cataractogenesis and in lens aging.

[Wrapping operation for Stanford type A acute dissecting aneurysms].

Four cases of Stanford type A acute aortic dissection were treated by wrapping operation. After careful dissection between the dilated ascending aorta and right pulmonary artery, 8 mm woven Dacron vascular prosthesis was passed around the aorta and tightened. Further enforcement wrapping was carried out using several prostheses from aortic root to the innominate artery. We call this technique "wrapping operation". Emergent operations were performed on all patients because of cardiac tamponade, 6.8-33 hours after the onset of dissection. Three patients are doing well 17-39 months after the operation, but only 1 patient died early postoperative period because of DIC and multiple organ failure. And 2 patients was showed closure of entry of the ascending aorta by the postoperative aortography. We think that the wrapping operation is a useful technique for the patients of ruptured Stanford type A acute dissecting aneurysms.

[Slowly progressive fluent aphasia--clinical features and an imaging study including MRI, SPECT and PET].

Three cases of slowly progressive fluent aphasia were presented. In all it began with word amnesia or stuttering, and in one to five years impairment of auditory comprehension, and reading and writing difficulties with kanji (Japanese morphograms) appeared. The neuropsychological and radiological findings were as follows: Case 1: a right-handed 65-year-old man showed severe fluent aphasia (the Western Aphasia Battery profile was rated as corresponding to Wernicke's aphasia but his score on the Token test was too high for Wernicke's aphasia) and memory disturbance. Cortical atrophy was prominent in the left temporal and parietal lobes. Hypoperfusion was evident in the bilateral anterior temporal region, suggesting the diagnosis of Pick's disease. Case 2: a right-handed 42-year-old man presented amnesic aphasia with buccofacial and ideomotor apraxia. The left frontal, temporal and parietal lobes were all atrophic. Hypoperfusion was marked in the left perisylvian and temporo-parietal regions, being similar to the pattern of Mesulam's slowly progressive aphasia. Case 3: a right-handed 55-year-old man with amnesic aphasia. Left-side dominant cortical atrophy involved the frontal, temporal and parietal lobes. The metabolic rate was lower in the left anterior temporal lobe, compatible with the finding in Pick's disease. The common clinical features of these patients were selective deficits in vocabulary, resulting in impairment of confrontation naming, and auditory comprehension. They sometimes could not recognize things even when they were told their names; case 1 could not even point to objects on command, while their syntactic comprehension was well preserved.(ABSTRACT TRUNCATED AT 250 WORDS)

[Marchiafava-Bignami disease with symptoms of the motor impersistence and unilateral hemispatial neglect].

Machiafava-Bignami disease (MBD) occurs in the alcoholic patients. It is characterized clinically by interhemispheric disconnection syndrome, resulting from demyelination and necrosis of the corpus callosum. We performed a neuropsychological study of an alcoholic patient, diagnosed as MBD by X-ray computed tomography (CT) and magnetic resonance imaging (MRI). 123I IMP-SPECT of the patient revealed the reduction of the blood flow in both cerebral hemispheres without laterality though both the X-ray CT and MRI showed no abnormality in the cerebral hemispheres. In neuropsychological studies, we observed not only interhemispheric disconnection syndrome such as an unilateral left agraphia and left apraxia but also left hemispatial neglect demonstrated in right-hand performance and motor impersistence. Because there were no abnormal lesions in the right hemisphere, these symptoms might be attributable to the lesions of corpus callosum.

Activities of ascorbate free radical reductase and H2O2-dependent NADH oxidation in senile cataractous human lenses.

Changes in activities of ascorbate free radical (AFR) reductase (NADH:AFR oxidoreductase) and H2O2-dependent NADH oxidation were correlated with levels of insoluble protein in senile cataractous human lenses. The H2O2-dependent NADH oxidation activity was measured to reflect the content of free glutathione. AFR reductase activities in all the cataractous lenses assayed here tended to decrease with increase of insoluble protein contents. A similar tendency in the relationship between lens protein aggregation and H2O2-dependent NADH oxidation activities, i.e. free glutathione contents was recognized in the lenses with pale yellow, yellow or dark yellow nucleus. However, for the highest levels of insoluble protein, some of the brunescent cataractous lenses exhibited very high activities of H2O2-dependent NADH oxidation, and some brunescent lenses had very low activities. From the above results, it is suggested that lens protein aggregates in the brunescent and non-brunescent cataractous lenses may be formed through significantly different oxidation processes, respectively. The possible mechanisms such as free radical reaction and disulfide bond formation are discussed.

H2O2-dependent NADH oxidation activity in senile cataractous human lens: its relation to glutathione redox cycle.

The present study has demonstrated that H2O2-dependent NADH oxidation activity in aged transparent and senile cataractous human lenses is due to a glutathione redox cycle which consists of reduced glutathione, oxidized glutathione, glutathione peroxidase and glutathione reductase. Especially, the NADH oxidation activity was proportional to the content of free glutathione (reduced plus oxidized forms) under our assay conditions. Employing this simple assay method, the present report has also shown that in the aged and cataractous lenses with pale yellow, yellow or dark yellow nucleus, the free glutathione contents tend to decrease with the increase of insoluble protein contents. However, lens protein was remarkably insolubilized in the lenses with cataracta brunescens, regardless of their exhibiting low or high glutathione contents. The relationship among protein aggregation, coloration and glutathione concentration in the cataractous lenses has been discussed from these findings.

[Primary coronary artery dissection: its incidence and genesis studied by clinical and coronary angiographic findings].

The incidence and genesis of primary coronary artery dissection which occurred spontaneously in the coronary artery system were elucidated in this study. We examined clinical and coronary angiographic features of the patients who presented this dissection. Primary coronary artery dissection was observed in 23 (0.43%) of 5,400 consecutive patients with ischemic heart disease undergoing coronary angiography. The basic diseases were myocardial infarction in 17 cases and angina pectoris in 6. Sixteen patients (70%) had smoking habits, which might be related to this disease. The dissection of the right coronary artery in 12 cases, of the left anterior descending artery in 10, and of the left circumflex artery in only one was found. All lesions occurred in the proximal positions of the coronary branches. Seven (30%) of all cases had fixed coronary stenosis in other coronary branches. The other cases had "one-vessel" disease. Seventeen patients were treated medically, 5 had PTCA therapy and one had CABG operation. All the patients, except one who died of cancer are surviving in a 3.8-year follow-up. Patients can be divided etiologically into three groups. The first was comprised of those in the post- or peri-partum period. The second was those associated with coronary vasospasm, and the third was those with atherosclerotic coronary artery disease. The third group was the most frequent.
