[The new methodic approach to the study of nutrition of pregnant women].

The author offers a modern technique of study of a nutrition of the pregnant women. It is based on use of a questionnaire-polling method, including the data of obstetrical anamnesis and basic characteristics of a social and economic status. The developed questionnaire, which contains seven sections, including data connected with the social and economic status, with moving activity of the women, with the values of consumption of the basic food substances, mode of a feed, pernicious habits, body weight of a pregnant woman.

[The use of prostaglandins in prolonged pregnancy and their effect on the newborn infant].

OBJECTIVE: The purpose of this study was to evaluate the neonatal outcome in newborns from mothers with post-term pregnancy and vaginal prostaglandin (Prostin E2) for cervical ripening and labor induction. STUDY DESIGN: This prospective study included 82 newborns, w.g. 41-42, divided into three groups according to the management of post-term pregnancy: group I--preinduction with Prostin E2 before induction of labor with oxytocin; group II--induction with oxytocin only; group III--with expectant management. In each newborn group weight, size, head circumference, Apgar score and neonatal morbidity characteristics were noted. RESULTS: The cesarean delivery rate was 14.2% in group I, 12.9% in the group II and 3.3% in the III-d group. There were no significant differences between groups in the frequency of meconial amniotic fluid, neonatal meconium aspiration. The incidences of severe asphyxia, neonatal injury and pathologic neonatal icter were higher in the group I. CONCLUSIONS: Prostin E2, used for the cervical ripening and labor induction with oxytocin increased the rate of surgical interventions at delivery and neonatal morbidity.

[The use of prostaglandins in atonic uterine hemorrhages following vaginal delivery].

The authors examine the results of usage of Prostin 15M (Upjohn) in series of 19 women with hypotonic uterine bleeding after vaginal delivery. For the same period (1997) 1179 deliveries were performed in the Department of Obstetrics and Gynecology in HMS-Plovdiv. Positive effect was noted in 16 of the cases (84.22%). Unsatisfactory effect was marked in 3 of the cases--15.78%. Three of the women demonstrated adverse reactions after application of Prostin 15M.

[Overuse of cooking salt by pregnant women in Bulgaria].

The authors investigate the ovemse of salt by pregnant women in two aspects--as a detrimental nutritive habit and as a factor in the pathogenesis of hypertensive disorders during pregnancy. The study involves 140 pregnant women who were given a specially designed questionnaire in order to examine their nutritional habits. One third (33%) of them intake additional amount of salt during nutrition. Almost half of them (43%) add salt before tasting the meal. This is exclusively seen in the group of women above 30. Additional 2-3 grams of salt are intaken while eating salty cheese and pickles during the winter. The approximate amounts of salt for each trimester of pregnancy are calculated. The authors defined which foods are the greatest contributors of salt and make the conclusions of the detrimental impact of overuse of salt during pregnancy on the mother and the child.

[Non-obstetrical indications for cesarean section at the Obstetrics-Gynecology Department of the Higher Medical Institute, Plovdiv].

The authors have examined retrospectively the non-obstetrical indications for Caesarian section over a six-year period (1994-1999). Such indications have been related to extra-genital diseases. For this period there have been 6647 births, 908 of which (13.66%) by Caesarian section. For non-obstetrical indications 146 CS have been performed. The greatest number of CS is due to ophthalmological reasons--84. The current concepts for performing CS in cases of eye pathology and non-medical indications are discussed.

Prostaglandin E2 in preinduction cervical ripening in postdate pregnancy.

UNLABELLED: Unripe cervix is a widely recognised factor contributing to the failure to induce labour. Its presence is associated with high frequency of operative delivery, adverse fetal outcome and high maternal morbidity. MATERIAL: We studied the efficacy of PGE2 application for preinduction cervical ripening in women with postdate pregnancy. Two PGE2-containing preparations were used--Prepidil-gel and Prostin E2 (Upjohn). They were administered to 42 women with postdate pregnancy, unripe cervices and indications for labour induction. The study sample was divided into two groups: group I (20 women) received Prepidil-gel, group II (22 women)--Prostin E2. RESULTS: 85.7% of the women responded favourably to the ripening effect of PGE2. There was no difference in the efficacy, safety and outcome of pregnancy between the two groups. CONCLUSION: Prostaglandin PGE2 is effective for preinduction cervical ripening in women with postdated pregnancy. The authors give their preference to Prostin E2 because it is easier to apply.

[The choice of the uterine-contraction agent in managing the placental period of labor based on our data].

Every obstetrician is acquainted with the existing risk during the placental stage. The danger of a greater blood loss is always present. The use of uterotonic medicines reduces the post-delivery bleeding. The authors have carried out observations over 422 delivery cases. In 153 of them ergotamine has been applied in the placental stage, in 121 oxytocine, and in 148 delivery cases the placental stage has passed without uterotonic drugs. The authors have concluded that the application of contracting medicines reduces the blood loss, but their usage has to be selective and after delivery of the fore shoulder of the foetus.

[The characteristics of labor in multifetal pregnancy based on our data].

The pregnancy and delivery in multifetal pregnancy most often are accompanied by complications and by a high degree of risk both for the mother and the foetus. The successful and of the delivery depends on the condition of the mother, the good obstetric status, the gestation age of the foetuses and their presentation. The authors have followed the characteristic of the delivery in multifetal pregnancy over 10-years material in 137 cases of pregnancy. After strict indications, in 26 cases the delivery has been completed though caesarean section. In 111 cases the delivery has been vaginal. A characteristic of the delivery has been made and it has been expressed in diagrams. The authors have been successful in the vaginal deliveries in multifetal pregnancy. They emphasize the active observation of the patient, especially in the first stage, and the monitoring of the foetus.

[A procedure for delivery in multiple pregnancy based on our data].

The multifoetal pregnancy has not lost its significance both in terms of its course and its method of delivery. Its frequency has increased in recent years. The authors have set the goal of observing the method of delivery at multifoetal pregnancy over a period of 10 years. In the Obstetrics Clinic there have been 134 bigemine and 3 trigemine deliveries. In 26 pregnant women 18.98% the delivery has been completed by caesarean section. The vaginal deliveries have been 91 (66.43%). In 7 deliveries a forceps has been applied (5.10%). Extraction has been carried out in 12 deliveries (8.&%). On the basis of the accomplished observations the authors have concluded that the vaginal delivery has priority over the abdominal delivery. The latter has to be strictly indicated.

[Neonatal birth trauma in relation to the means of delivery].

The frequency of birth trauma has fallen considerably in recent years. This decline reflects in particular a greater tendency to perform a caesarean section when difficulties of delivery would appear. Birth trauma often causes not only early, but late and continuous illness in neonates. For this reason it is an important problem in neonatal practice. 1791 newborn have been investigated: 1515 (84.5%) by vaginal delivery, 177 (9.8%) by caesarean section, 38 (2.1%) by forceps, 18 (1.01%) by vacuum extraction and 43 (2.4%) by pelvic presentation. The results show the frequency of birth trauma (5.5%) and asphyxia (5.9%) by vaginal delivery and by operative methods (19.8%--S.C., 60.5%--F., 88.8%--V.E.) and their dependence on some factors: induction or stimulation of labor, meconium stained amniotic fluid, prolonged or extremely rapid labor.

[The biochemical changes in overweight pregnant women during the pregnancy].

The degree of weight gain during pregnancy as a risk factor for a mother and her newborn is a disputable problem. Most of the studies have shown, that maternal overweight gain has a bad effect on labor delivery outcome. The present study included 108 pregnant women, subdivided into three groups, according to weight gain during pregnancy: Group I(32) - < 12 kg, Group II(42) - 12 < - < 20 kg. Group III(34) - > 20 kg. The patients were estimated according to their age, parity, pregnancy induced hypertension and mode of delivery. The authors investigated biochemical indicators of protein, lipid and electrolyte balance. The results illustrate the influence of prenatal overweight gain on the pregnancy complication, mode of delivery and some biochemical changes.

[A comparative study in the treatment of complicated wounds following obstetrical-gynecological interventions].

The results are announced of the treatment of 64 (1.28%) complicated incisions after 4981 obstetric and gynecological interventions over a 3-year period. The cases observed are distributed in two groups: Group One--30 patients treated with the medicinal and surgical methods used in traditional clinic practice, and Group Two--34 patients undergone laser therapy. Comparing the results of the treatment the authors arrive at the conclusions that the He-Neon laser has a place in the treatment of complicated incisions in obstetric and gynecological practices helping to avoid certain allergic reactions, to reduce considerably the hospital stay and therefore has an economic effect.
