[The importance of liver scanning in pancreatic carcinoma (author's transl)].

One of several liver scans were obtained in 29 patients with carcinoma of the pancreas. Scanning of the liver offers several advantages in cases of suspected carcinoma of the pancreas: (1) It may prove malignancy in jaundiced patients without a detectable primary tumor (frequency carcinoma of the pancreas). (2) It may contribute to the differential diagnosis of jaundice when signs of extrahepatic biliary obstruction or metastases can be seen. (3) It is the least strenuous diagnostic method in seriously ill and old patients who are often in very poor condition; sometimes it can spare the patients a laparotomy.

[Considerations on the information content of diagnostic tests (author's transl)].

The value of a diagnostic test lies in its ability to support the doctor's efforts to detect a certain disease. The aim is to achieve the highest possible probability that the disease is correctly diagnosed by means of the test. Doctors are not always aware of the extent to which a test can contribute to the final diagnosis. This contribution, the information content of the test, can be calculated by various algebraic manipulations. An example (carcinoembryonic antigen) is given, which demonstrates the significance of certain terms such as true and false positive results or likelihood and, moreover, the application of Baye's theorem.

Evidence for a new intermediate state in the mechanism of (Na+ + K+)-adenosine triphosphatase.

A rapid mixing technique was used to follow the intermediate formation of phosphorylated enzyme and liberation of inorganic phosphate by a microsomal preparation of (Na+ + K+)-ATPase. In the presence of 100 mM Na+,but without added K+, phosphorylation reaches a constant level at a rate which is dependent on ATP concentration. Inorganic phosphate production lags during the inital phase of phosphorylation and then accumulates at a constant rate. These observations favor a scheme in which Pi is liberated as the result of turnover of the phosphorylated enzyme. In the presence of 100 mM Na+ and 2.5 mM K+ phosphate production was resolved into two phases consisting of an initial 'burst' and late steady state phase...

[67Ga-whole-body-scanning for non-invasive diagnosis and observation of the course of Hodgkin's disease].

Whole body scanning with 67-Ga citrate, could stand the test as a valuable non-invasive agent for the diagnosis and course study of Hodgkin's disease. It shows a differential accuracy in various nodal and extranodal manifestations, as well as in treated and non-treated cases. The method is not capable of replacing the common diagnostic methods which has been thus far used, but it can complete and occasionally correct them. The best diagnostic information can be gained in the thorax and in extranodal manifestations, which are difficult to evaluate by conventional studies. Our own results, which were obtained by means of studies in 18 cases, are discussed in comparison with reported results in the literature.

[Hemodynamic long-term effects of a beta-receptor blockader (Pindolol) in primary essential hypertension].

1. Pindolol lowers blood pressure both, immediately after administration by a reduction of cardiac output and heart rate and after long-term administration by reducing peripheral resistance. Cardiac index, initially decreased, reached the control value after 8 weeks of treatment. 2. The persistent reduction in heart rate and increase of the cardiac index during exercise after prolonged treatment together with the increase of right and left heart filling pressures seems to be due to augmentation of the Starling mechanism. The increase of the stroke volume at rest and during exercise diminished the blood-pressure lowering effect of the drug. 4. The exercise-induced increase of noradrenalin was abolished by acute administration of pindolol. After long-term treatment, however, we found a consistent and significant elevation of the plasma-catecholamines. This may reflect the development of heart failure [1], a tyramin- or cocain-like action of pindolol [2] or an indirect sympathicomimetic effect of this drug at low doses (10--15 mg/day orally ) [3]. 5. The elevation of plasma-noradrenalin reflects increased sympathetic tone and may be responsible for the increase in stroke volume and plasma renin activity observed by several authors. 6. Since there was no evidence for a hemodynamic mode of action, the lowering of blood rpessure by pindolol may be a central effect.

Blood Pressure

[Frequency distribution of an index for free thyroxin in relation to age and thyroid status].

We compared an index for free thyroxine (FT4I) between groups of patients which were formed according to the results of radioiodine testing. An elevation of free thyroxine (as measured by the FT4I) which has been reported in elderly women by other authors could not be confirmed. Overlapping between euthyroidism and hyperthyroidism was not found as disturbing as reported elsewhere, a finding which can probably be explained by the different criteria for the categories "euthyroidism" and "hyperthyroidism". A group with depressed radioiodine uptake had an unexpected high percentage of elevated FT4I. These cases may be explained as toxic goitres. The incidence of hypothyroidism (as judged by FT4I) in this group is low compared with the incidence of obvious exogenous exposure to excessive iodide. Cases presenting with a normal radioiodide uptake together with absent suppression by thyroxine administration are more often connected with a normal FT4I and are therefore believed to be euthyroid; less frequently they are truly hyperthyroid (FT4I). In patients previously treated with radioiodine an elevated FT4I is found suprisingly often. A reevaluation of the radioiodine dosage and the course of the disease in these numerous patients seems to be indicated.

Age Factors

[Thyroxine in the treatment of euthyroid goiter and the prevention of goiter recurrence].

Patients with non-toxic goitre, or patients after an operation of non-toxic goitre, were given 75 mug thyroxine daily. The level of TSH decreased significantly only for the non-toxic goitre group. The TSH-TRH-test showed a high percentage of abnormal results, even after several months of treatment. An index for free thyroxine increased a little, but significantly. The neck sizes decreased only in the post-operative group significantly. The final scans showed a high percentage of goitres. 75 mug thyroxine is too small a dosage in these patients and 100 to 150 mug daily seem to be indicated in the above mentioned states.

Goiter

[The value of pyrophosphate scintigraphy for the assessment of the activity of spondylarthritis ankylopoietica (author's transl)].

Seriological and scintigraphic activity was compared in 29 patients with spondylarthritis ankylopoietica (Bechterew's disease). Among our cases, 11 serologically active patients also had positive scans. the details being: cervical vertebral column in 5, thoracic vertebral column in 7 cases and the lumbar vertebral column and iliosacral joint in all 11 patients. In the group of 16 serologically inactive patients, 12 cases could be confirmed radiologically, in the remaining 4 early detection of ankylosing spondylarthritis was possible from the scintigraphic findings in combination with the clinical symptoms. The clinically suspected diagnosis of an incipient Bechterew's disease could not be justified in the remaining 2 patients who had no positive scintigraphic or serological results.

Adolescent

[Isotope diagnostic findigs in nephroptoses].

Radiorenography, serial scans and clearance determinations using 131-I-Iodohippurate and 51-CPr-EDTA were performed on a total of 106 patients with unilateral nephroptosis. In 45 of them with pathological findings in the upright position the examination was repeated in the prone position. 19 patients underwent nephropexy and were reexamined 2 to 6 weeks there after. The results suggest that the combination of different radioisotope methods provides a valuable and sensitive means to detect posture-dependent impairment of kidney and urinary tract function and yields an additional objective basis for the selection of patients for nephropexy.

[20 min 99mTc thyroid uptake (author's transl)].

The 20 min 99mTc thyroid uptake compares favourably with the more time consuming 24 h 131J uptake because it results in a lower radiation dose to the thyroid gland. The reliability of both methods is nearly the same. In all conditions in which the 131J uptake is altered, a similar deviation of the 99mTc uptake has been found. A single exception are patients treated with antithyroid drugs (together with thyroid hormones), who show frequently an elevated 99mTc uptake (in contrast to the 131J uptake). Prognostic conclusions have been drawn by other autors from the 99mTc uptake in similar cases.
