Out-patient treatment of psoriasis: short contact and overnight dithranol therapy compared.

Short contact (30 min) and overnight therapy with 1%, 2% and 3% dithranol in an ointment base were compared in 30 out-patients with plaque psoriasis. Both regimes produced significant clearing, there being no statistically significant difference between the two. There were three withdrawals from each group. The remaining 12 patients on the 30 min regime found it both effective and practical. It is therefore a useful alternative regime for out-patient dithranol therapy.

Comparative study of three methods of plastic embedding in diagnostic dermatopathology.

Three methods of plastic embedding were assessed for their value in routine dermatopathology. The J B 4, Taab embedding, and Taab transmit techniques were compared for quality, convenience, and safety. The Taab transmit method was the most satisfactory method, being both rapid and simple. An additional advantage is that the resin can be removed and the immunoperoxidase technique carried out on thin sections.

Natural-killer-cell activity in atopic dermatitis.

Natural-killer (NK)-cell activity was measured in the peripheral blood of twenty patients with atopic dermatitis and in a group of thirteen age-matched non-atopic controls (nine subjects on thirteen occasions). The method uses a chromium-release assay with the human leukaemia cell line, K562, labelled with 51Cr as the target cell. A highly significant reduction in NK-cell activity was found in the patients with atopic dermatitis.

Use of NK1 C3 monoclonal antibody in the assessment of benign and malignant melanocytic lesions.

The monoclonal antibody NK1 C3, synthesised by the Netherlands Cancer Institute, has been used to assess its value in the diagnosis of melanocytic lesions. The antigen recognised by this antibody is not denatured by formalin fixation, with the result that the antibody can be used for retrospective studies on conventionally processed material. Positive results were obtained in primary melanoma (18/18), secondary melanoma (21/21), junctional and compound naevi (32/32), intradermal naevi (9/12), congenital naevi (3/3), so called dysplastic naevi (13/13), blue naevi (5/5), and Spitz tumours (3/14). Non-melanocytic tumours were tested for comparison. The results showed relative but not complete specificity of the antibody for melanocytic tumours, with positive results only in breast and prostate tumours (2/6 and 2/5 respectively). Negative results were obtained with basal and squamous cell carcinoma, appendage tumours, neural tumours, and apudomas. The staining pattern of NK1 C3 was compared with that of antibodies to S100 protein and to neurone specific enolase. Compared with S100 protein NK1 C3 gave stronger staining of a higher percentage of cells in the 12 specimens in which a direct comparison was made. Antibody raised against neurone specific enolase in sheep gave very poor results with heavy background staining. We suggest that NK1 C3 is a useful addition to the battery of monoclonal antibodies of value to the diagnostic histopathologist.

A multicentre phase II trial of vindesine in malignant melanoma.

Fifty-three evaluable patients with advanced malignant melanoma have been treated with vindesine 3 mg/m2 i.v. weekly for a minimum of 6 weeks. An objective response rate of 26% was attained with 17% complete remissions, 78% of which were in stage II disease. The treatment was well tolerated, with alopecia the only clinically significant side-effect (43% of patients). Vindesine is superior to DTIC and should be considered as the best currently available drug for malignant melanoma.

The use of a double-label immunoperoxidase monoclonal antibody technique in the investigation of patients with mycosis fungoides.

Using a double-labelling immunoperoxidase and monoclonal antibodies raised against T helper and T suppressor cells and Langerhans cells, we have found that the clinically involved skin of seventeen mycosis fungoides (MF) patients shows an increase in number of Langerhans cells which are in contact with T helper lymphocytes. T suppressor lymphocytes are also present, but are generally seen singly situated at a distance from the T helper/Langerhans cell clusters.

The use of paraffin-processed tissue and the immunoperoxidase technique in the diagnosis of bullous diseases, lupus erythematosus and vasculitis.

Twenty-two skin biopsies have been split and each half examined either by the immunoperoxidase technique on paraffin-fixed tissue or by both immunoperoxidase (IP) and immunofluorescence (IF) technique on snap-frozen tissue. All twenty-two specimens gave positive results by frozen IF techniques, nineteen out of twenty-two (86%) were positive by frozen IP techniques and sixteen out of twenty-two (72%) were positive by IP on paraffin-fixed material. If retrospective immunopathology is required on a specimen and only paraffin-fixed material is available a positive result may be expected in the majority of cases.

Personal solar UV-A doses received by patients undergoing oral psoralen photochemotherapy for psoriasis.

Patients undergoing oral psoralen photochemotherapy (PUVA) for psoriasis receive a known amount of UV-A in the treatment cubicle and an unknown amount of UV-A from sunlight. In order to measure the solar UV-A dose, fifty-six patients received a UV-sensitive film badge to be worn from the time they took their psoralen tablet throughout the course of the day, except during treatment. The UV-A doses measured in this way varied from less than 0.2 J cm-2 to 7 J cm-2, with a median of around 1 J cm-2. These results show that the solar UV-A received by patients who have been photosensitized by 8-MOP may not be insignificant in relation to the treatment dose.

Loss of beta2 microglobulin from the cell surface of cutaneous malignant and premalignant lesions.

Biopsies from twenty-three patients with malignant skin lesions and from twenty-two patients with premalignant or benign skin lesions were stained for beta2 microglobulin by use of the immunoperoxidase technique. The results showed a significant loss of demonstrable surface beta2 microglobulin from the surface of malignant cells compared to benign, and a partial loss in the premalignant cases. This difference could prove to be a useful tool in the histological diagnosis of malignant skin lesions, and in assessing whether or not the lesion has been completely excised.

In vitro reaction of cancer patients and others to Bacillus Calmette-Gueérin.

We examined the in vitro reaction of leukocytes from 71 cancer patients and 121 control donors (disease-free individuals or patients with non-malignant non-debilitating conditions) against Bacillus Calmette-Guérin using a one-stage capillary leukocyte migration assay. The proportion of reactive cancer patients and control donors increased with increasing BCG concentration, but the proportion of patients who reacted was less than that of control donors at all concentrations of BCG. As cancer stage advanced and the volume of clinically detectable tumor increased, the proportion of reactive patients decreased. There was a small age-related decline in reactivity in the control population. The results of simultaneous in vitro tests and Mantoux reactions were concurrent in a majority of normal individuals so tested.

Plasma zinc in psoriatic in-patients treated with local zinc applications.

Zinc is a major component of many local skin applications. An investigation into its systemic absorption in thirty-one psoriatic hospital in-patients is reported. Plasma zinc, copper and whole blood delta-aminolaevulinic acid dehydratase were followed before and during in-patient treatment over 14 days. No significant alteration in these parameters was detected. Significantly reduced plasma zinc levels were found in the patients compared to a matched control population.
