[Paraurethral cysts--diagnosis, differential diagnosis and treatment (a case report)].

The aim of the authors is to reflect clinically upon a rare pathology of the female genital system vaginal cysts. The presentation includes description, diagnostical methods, differential diagnosis and treatment of the paraurethral cysts and urethral diverticulum. It was fully described on the basis of clinical case from the gynecological practice on a patient with paraurethral cyst, which was removed surgically with good outcome and result.

[Aids and pregnancy--contemporary aspects of the problem].

The authors made a short analysis of the epidemiology, pathogenesis, diagnosis of the HIV-infection and AIDS. There was presented contradictory attitudes about the influence of the pregnancy over the progress of the HIV-infection and AIDS, and the effect of HIV-infection and AIDS on the pregnancy development. There was also described the injuries of the fetus and newborn. There was exposed conceptions about the obstetric behavior and the way of delivery by pregnant women, who have been infected with HIV, AIDS sick persons and zidovudine treated patients.

[A rare case of ovarian cancer in a patient treated for secondary sterility].

The authors described a rare case of ovarian cancer in a young woman who looked for gynecological help because of secondary sterility. There were found cysts of the both ovaries, which were conservative treated by insistence of the patient for three months. After unsuccessful gestagenic therapy there was performed a gynecological laparoscopy, which showed that on one of the ovaries there was an exophyte growths suspected for carcinoma tissue. It was offered an emergency operation, which was made only after 1,5 months because of personal causes of the patient. It was taken a sample that was with unconvincing data for ovarian cancer. The clinical experience and professionalism of the surgical team were put on the test. It was performed a radical operation with removal of the uterus, adnexa uteri, partial peritoneoectomy, total lymph dissection, appendectomy, total omentectomy. The patient was discharged from hospital in good condition and she is liable to postoperative chemotherapy.

[Indications for third and more than three cesarean sections--analysis and pattern of practice].

PURPOSE OF THE STUDY: Authors aimed to analyse indications for third and more than three times cesarean sections (c. s.), as well as, to make an assessment of the adequacy of the taken decisions. METHODS AND MATERIALS: It was conducted a retrospective study for third and more than three times cesarean, including period 1991-2003 in the Department of Obstetrics and Gynaecology, Medical University, Pleven. During that time 28645 births has taken place, 2971 of which are by cesarean section. 2,5% from the last, has been third and more than three times cesarean. Using expert judging, indications for the last were divided into three groups-right, disputable and wrong. RESULTS: In the cases with only one mentioned indication "Re-re c.s." 37,8% (28 patients), ratio between right, disputable and wrong was 2:1:1 and the same ratio in the cases with indication "Re-re c.s.+ other indications" 58,1% (43 patients), was 5:1:1. In 4,3% of the cases (3 patients) no indication "Re-re c.s." was mentioned. CONCLUSION: Reduction of third and more than three times cesarean can be achieved by more careful judgement of indications in second, "disputable", group and erradication of the "wrong", which most commonly are jatrogenic reasons. This will result in diminishing the increasing frequency of abdominal births, as well as, preventing possible complications for mother, connected with operation.

[Antibiotic prophylaxis on treatment outcome after gynecologic surgery].

The aim of our study is to estimate advantages and disadvantages of some antibacterial drugs which were used for prevention from infections in the early postoperative period. Our study is retrospective and involved all women which were undergoing different operations in department of gynaecology for the period January 2000 until June 2004. Due to a large number of patients and long period of time which were evaluated our study shows assume data for efficacy of different antibacterial drugs and drug combination when we using them for prevention. It is very interesting for the authors result about relationship between price-efficacy-durance ot hospital being.

[Gynecologic laparoscopy and Swyer syndrome (two additional cases)].

The syndrome belongs to the group of the congenital abnormalities and is also known as syndrome of the pure gonadal dysgenesis with the presence of medullar elements. The girls with the Swyer syndrome are characterized by the presence of the male karyotype--46,XY and they are sex-chromatin negative. The authors of this publication describe in details etiology, pathogenesis, clinical presentation, differential diagnosis and therapeutic methods for syndrome's treatment. There are presented and two clinical cases, proved by chromosome analysis and verified by laparoscopy. It is about the significance of gynecologic laparoscopy for precise diagnostics in congenital anomalies of the genital system.

[Disorders in sexual function after hysterectomy].

The authors studied 120 women with hysterectomy, divided into 4 groups according to the volume of the surgical intervention (with preserved or removed cervix uteri and with preserved or eliminated ovaria). In the groups do not enter hysterectomies of oncological diseases and of patients with vaginal hysterectomy. The authors found a disturbed sexual function. Towards the different biological problems exist and a psychological one, the stress and the inhibited self-esteem from the loss of the important for the woman's functioning organs. In a small part of the operated patients was observed an increased sexual activity, as a psychological compensation of the inferiority feeling.

[Sex behavior and contraception among the population of Romany origin].

The modern and reliable methods for contraception are not sufficiently used in our country, especially among the Gypsy population, where the abortions and the births are relatively higher. Through a standardised interview information about the fertility and the familial status of the Gypsy population of Sofia, Pleven [correction of Eleven] and Sliven regions was gathered. The aim of the study is analysis of the sexual behaviour of the population in the given regions and the need of adequate contraception avoidance of unwanted gravidity. The results show that only 12% of the population know how to protect themselves of sexually transmitted diseases, 55% of the women have not knowledge about their own gravidity, 57.3% of them give birth of their first child at the age of 15-17, 78.1% do not use contraceptive means and 3.6% use harmful "protective" methods.

[Myasthenia and pregnancy].

The myasthenia is a neurological autoimmune disease affecting women in reproductive age. The combination of myasthenia and gravidity has two important aspects; reflection of gravidity on the neurological illness and the influence of the myasthenia on the course of the gravidity and the delivery. The authors present one case of myasthenia and gravidity and offer some practical advises: 1. narrow collaboration between the patient and the different specialists. 2. good information about the myasthenia evolution during the gravidity. 3. adequate therapeutic approach to the given case.

[Diagnostic ultrasound and the intrauterine pessary].

The aim of the authors is an analysis of the results in application of ultrasound technique in patients with intrauterine device (IUD). Using the ultrasound criteria for a correct and incorrect position of IUD, the criteria for an elevated risk for expulsion and contraindications for IUD application, the authors have used ultrasound technique for diagnosis in 65 patients with IUD. The causes for the "wrong" diagnosis were discussed: imperfection of the apparatuses, non-observance of the conditions for ultrasound examinations and lack of experience. Practical conclusions and recommendations for an increase of the effectively of this seemingly easy method for diagnosis and follow-up of the IUD were made.

[Laparoscopic fenestration of the ovaries in sterility and the Stein-Leventhal syndrome--is it a modern fad or correct procedure].

The treatment of the sterility due to the Stein-Leventhal syndrome has been always object of many discussions. Sometimes, the approach towards this disease in the different departments is diametrically different. Some offer only conservative therapy other-only surgical interventions and some-combination between the two. The authors of this study present the results of operative laparoscopy in the treatment of sterility and polycystic ovary syndrome, in the department of gynaecology to the Higher Medical Institute, Pleven. Object of observation were 33 women in whom was performed laparoscopic fenestration of the ovaries and 20 women in whom was made resectio cuneiformis ovariorum. Comparative analysis of the results for one year period after the surgical interventions was made. The conclusion of the authors is that after failure of the conservative approach, the laparoscopic fenestration of the ovaries remains the only right therapeutic intervention.

[A rare case of generalized echinococcosis and pregnancy].

The authors describe a rare case of combination of generalized echinococcosis and gravidity. It concerns about a 23-years-old patient in 10 lunar month in whom 10 days before the term, accidentally through ultrasound is found an Echinococcus cyst in the liver. The delivery performed with planned caesarean section without disturbance of the cyst, ended successfully. Two days after the surgery the patient complained of troubled respiration and oppression. The X-ray photograph showed 3 Echinococcus cysts more--one in the right end two in the left lungs. The problem was solved with three more operations after 2, 6 and 10 months, with which the Echinococcus cysts were eliminated.

[A multicenter study of the antimicrobial effect of Macmiror and Macmiror Complex in the treatment of vaginal infections].

UNLABELLED: The aim of the present multicentre study was to examine the therapeutic possibilities of the wide-spectrum medicament MACMIROR & MACMIROR COMPLEX for the treatment of the vaginal infections. MATERIALS AND METHODS: The study included 159 nonpregnant women among 15 and 54 years (middle age 35.6) with different by kind and intensity colpitis complaints. The following microbiological characteristic was established: in 26 cases Gardnerella vaginalis, in 46 Candida spp., and in the rest 87-mixed aerobic bacterial flora, with a combination of Gardnerella, yeast and Trichomonas. The treatment of the patients was done in combined scheme: peroral and vaginal administration, simultaneously with local treatment of the partner. The control examination was performed bistagely: on 7-10 day and on 30-40 day. RESULTS: The good clinical and microbiological influence of the treated patients was established, for the first control examination the effect was found in 88.1% and 86.8% and for the second--respectively in 81.1% and 82.4%. CONCLUSIONS: The received results give us a cause to approve, that the combination "Nifuratel and Nystatin" (Macmiror & Macmimor complex) has the good possibilities to influence the mixed forms of vaginal infection.

[Tocolysis and its place in premature labor].

Premature labour ranges from 6 to 8 per cents from all of the deliveries but it causes 75 80 per cents of the perinatal infant mortality. The purposes of the present research are: to prove the benefit of the tocolytic therapy in case of progressing premature labour (cervical dilatation 3 and more cm and distinguishable uterine contractions--at least 2 in 10 min for more that 1 hour); to compare the efficiency of the medicines used in tocolysis, as well as their influence on the maternal and infant morbidity and mortality; A successful tocolysis is considered to be the one, which delays the delivery for at least 48 hours. The authors concluded, that there is not any significant statistic difference in the tocolysis success with beta-adrenomymethics and the use aquatic solution of Magnesium sulfate. Better results were achieved in the cases when Indomethacin was added to the basic tocolytic medicine. No significant harms were observed in the maternal and infant organisms in result of the tocolytics application. It has been found that after the tocolytic therapy application the newborns were with greater birth body weight and the percentage of the respiratory distress was lower, compared to the group with expectant behaviour at hospitalization.

[Disorders in sexual function following hysterectomy].

The authors aims to examine the sexual function among patients prior to hysterectomy. Four groups were examined: group one--total hysterectomy with bilateral adnexectomy; group two--total hysterectomy without adnexectomy; group three--supracervical hysterectomy with bilateral adnexectomy and group four--supracervical hysterectomy without adnexectomy. A questionnaire aimed at Learning the sexual function and psychologic state of the patients and more importantly the effects of the hysterectomy on then was passed out. From the completed questionnaires we found that the most affected group was the one of total hysterectomy with bilateral adnexectomy. In all respects sexual function is disturbed in this group. Least affected are patients in groups three and four. In some of them we even found a slight increase in sexual function, which is regarded compensatory for eliminating the infertility complex arising from surgery of the most important female organs.

[Antibiotic prophylaxis in cesarean section with cefoxitin (Mefoxin)].

The ain of this presentation is to show the advantages and disadvantages of a single prophylactic dose of Mefoxin, in comparison to the classical antibiotic prophylaxis with penicillin and gentamicin. The authors conclude that: single dose prophylaxis with Mefoxin significantly reduces the rate of infection morbidity after caesarean section; a single application of 2 g Mefoxin after clamping of the umbilicus is enough for prophylaxis of infectious complications after caesarean section.

[The diagnostic and therapeutic problems in genital infections caused by Chlamydia trachomatis].

There have been examined 28 married couples from 25 to 35 years old with fertility and 55 patients from 25 to 40 years old with symptoms of genital infection and promiscuity. Chlamydia-antigens in urethral and endocervical specimens and antibodies in blood were searched by the Microimmunofluorescence method. Only 7.1% of the married couples and 1.8% of the patients had positive results for antigens. 53.6% of the married couples and 54.25 of the patients had positive results for antibodies, at rates from 1:32 to 1:512, confirming a profound Chlamydia trachomatis-genital infection. Outline laboratory analysis are needed for solving the diagnostic and therapeutic problems of Chlamydia trachomatis-genital infections.

[Multiple pregnancy--ultrasonic biometry after the 28th gestational week--nomograms].

The authors aimed at working out normograms for BPD, FL, FAC and EFBW among twins in multiple gestation. They find a delay in the rate of increase of sonographic indices in multiple gestation in comparison to singleton pregnancies, as follows: in BPD--after 26-28 wks; FL--after 34-35 wks; and FAC--after 34-36 wks. Lower EFBW was found after 32-33 wks. The differences in the biometric indices in the first and second twins were also noted. In conclusion it is stated that it is mandatory to monitor by sonography BPD in the 26th wk, and FL and FAC in the 34th-35th wk.
