Association between recurrent aphthous ulcers and tobacco habits.

The study is a complication of data on oral mucosal lesions collected among 20 333 people aged 15 yr and over. The aim was to calculate the correlation between the prevalence of recurrent aphthous ulcers and various tobacco habits. The prevalence of aphthous ulcers in the population was 17.7% including cases referring to episodes within the last 2 yr. There was a negative correlation between the prevalence of ulcers and tobacco habits. The suppression of ulcers was most evident for those groups smoking pipe or cigarettes without filter and only moderate for those using snuff. Further, heavy smokers had a lower frequency of ulcers than moderate smokers.

The occurrence of recurrent aphthous ulcers in an adult Swedish population.

Aphthous ulcers were found in 2% of a total of 20,333 individuals aged 15 years or more examined in urban, suburban, and rural regions of Sweden. When a history of 2 years was considered, the prevalence was 17.7%. A slight predominance was found for females. The younger age groups showed the highest prevalences of the condition, which then decreased steadily with age. Interviews were conducted with a fraction of the population comprising 2771 people with regard to their experiences of aphthous ulcers. Thus, for example, the average number of episodes per year was two to four and the most prevalent time for healing, 5-8 days. Among the factors thought by patients to trigger the condition, catching a cold was reported as the main one, followed by stomach problems and menstruation.

Treatment of recurrent aphthous ulcers with Aureomycin mouth rinse or Zendium dentifrice.

Clinical trials with a cross-over double-blind technique were undertaken to test chlortetracycline (Aureomycin) and the enzyme-containing dentifrice Zendium with regard to therapeutic effects on recurrent aphthous ulcers. Aureomycin was found to reduce the number of ulcers and diminish pain when compared with placebo. When groups of patients treated with Zendium and placebo dentifrice, respectively, were compared, no statistically significant difference could be demonstrated. However, when the pH value of Zendium was stepwise changed from 5.9 to 6.8, an increased fraction of patients reported complete relief from pain and ulcer(s) during the trial periods.

Oral exposure to a nickel-containing dental alloy of persons with hypersensitive skin reactions to nickel.

In 10 partially dentate subjects, all with long histories of skin reactions to nickel, prosthetic treatments were performed with fixed prosthetic appliances manufactured in a dental alloy containing 66% nickel. During time periods varying from 12 to over 40 months, these subjects were controlled using a variety of dental and medical methods. Neither adverse general nor oral clinical nor histological reactions were noted during the observation.

Clinical evaluation of patients referred with symptoms related to oral galvanism.

Sixty-eight consecutive patients (9 males, 59 females) referred for symptoms alleged to be caused by oral galvanism were investigated according to a standardized examination program including clinical examination, epicutaneous patch test with dental materials, haematologic screening, salivary test, calculation of currents created at metallic contacts between restorative materials and testing of electrical taste thresholds. No clinical entity could be identified but recognized clinical conditions that could possibly explain the complaints for which the patients were referred were prevalent. The most common symptoms reported were a smarting sensation in the mouth (71%) and different distant symptoms (47%). As regards oral signs, some of them showed similar prevalences as have been found in a general Swedish population. The main exception was changes at the apex of the tongue - found in 15%. Allergic skin reactions to dental materials were found among 25% of the patients. Mean values of blood and saliva parameters did not differ from what is considered normal. The distribution of currents created by contacting dental metallic restorations showed no deviation from the corresponding currents calculated in a control group. The median value for lowest electrical taste threshold in the group of referred patients was significantly lower than in a control group.

Traumatic ulcerative gingival lesion.

Gingival ulcers caused by tooth brushing in 13 patients with an average age of 29.9 years are presented. The lesions show characteristic clinical features not compatible with simple traumatic ulcers, e.g. running a more chronical course, the duration averaging 6.5 months. Repeated trauma and, in some cases, infection might explain the chronicity. The lesion is suggested to be designated traumatic ulcerative gingival lesion. As treatment, cessation of tooth brushing for 2 weeks and concomitant mouthrinses with chlorhexidine are recommended.

Epidemiologic study of excessive oral melanin pigmentation with special reference to the influence of tobacco habits.

The prevalence of oral melanin pigmentation was studied in a population of 30 118 adult individuals in Sweden. Among these, 9.9% showed melanin pigmentation in the oral mucosa. The anterior labial gingiva in the mandible was the most frequently pigmented location. The presence of melanin pigmentation was positively correlated to tobacco smoking. Among tobacco smokers 21.5% were pigmented as compared to 3.0% among individuals not using tobacco. The smoking-related oral pigmentation, smokers' melanosis, could thus be calculated at a prevalence of 18.5% among smokers and the total frequency of smokers' melanosis was calculated at 6.8%. The prevalence of pigmentation was found to increase prominently during the first year of smoking but also to decrease to the level found among non-tobacco users about 3 years after cessation of smoking. Snuff dipping did not significantly elevate the prevalence of oral melanin pigmentation.

Oral carcinomas associated with snuff dipping. Some clinical and histological characteristics of 23 tumours in Swedish males.

Some clinical and histological characteristics of 23 oral carcinomas in the anterior vestibulum of snuff dipping Swedish males with an average age of 76 years were investigated. Examples of verrucous carcinomas as well as of ordinary, usually verrucoid but also ulcerating, squamous cell carcinomas were encountered. Widespread oral leukoplakia and second primary carcinoma--oral or other--was present in 5 and 6 cases respectively, and occurred together in 3 of these. Eleven cases out of 19 available for additional sectioning and staining showed candidal infection.

Chronic lip fissures. Prevalence, pathology and treatment.

In a prevalence study of oral mucosal lesions carried out 1973-1974, 20,333 individuals, aged 15 years or above were examined. Chronic lip fissures were found in 110 people, corresponding to a prevalence rate of 0.57%. The fissures were more prevalent among males (0.92%) than females (0.20%) and more prevalent in the age groups below 45 years than in those above. In 1976-1978, 7 patients with annoying, chronic lip fissures were treated by excision. The mean observation time was 26 months (range 17-33 months). No recurrences of the fissures were observed. Histologic examination revealed a chronic inflammatory non-specific infiltrate and no dysplasia or cellular atypia was seen.

Leukoedema - an epidemiologic study with special reference to the influence of tobacco habits.

Among 20,333 individuals aged 15 years and above, examined in urban, suburban and rural regions in the middle of Sweden, leukoedema was found in 48.9%. The sex difference, males 61.9% and females 35.9%, was highly significant (P less than 0.001). The condition was most prevalent in the age groups 25-34 and 15-24 years being 60.6 an 53.7%, respectively. Leukoedema was significantly more prevalent among individuals with some daily tobacco habit (60.0%) than among those without (36.3%) (P less than 0.001), being most prevalent among smokers. The lesion seemed to be related to the amount of smoking since it was most prevalent among heavy smokers and also the most clearly established lesions were found in this group.

Focal epithelial hyperplasia in Sweden.

A prevalence of 0.11% of focal epithelial hyperplasia (FEH) was found among 20,333 adult Swedes. There was no sex difference, the lesion was most prevalent in age groups above 45 years and the lesion was most frequent on the tongue. The frequency of FEH in 15,132 consecutive routine biopsies was 0.26%. Four FEH-cells were ultrastructurally examined. They exhibited a clear cytoplasm with scattered ribosomes, a peripheral condensation of tonofilaments, a central aggregation of chromatin clumps with loss of nuclear membrane and an accumulation of desmosome fragments. No viral particles could be identified in these FEH-cells.

Prevalences of removable dentures and edentulousness in an adult Swedish population.

In three geographic regions, urban, suburban and rural from the middle of Sweden 20,333 adults (over 15 yrs of age) were examined clinically regarding denture wearing and edentulousness. 30.3% had removable dentures of some kind and 1.0% were edentulous and did not wear any dentures. Removable dentures were significantly more common amongst females (33.8%) than amongst males (26.8%). For complete dentures in both jaws this sex differences was even more pronounced, 19.3% and 11.4%, respectively. The prevalence for removable dentures was highest in the rural region, and lowest in the suburban. This finding may, however, be partly explained by the different age distribution within the areas examined.
