A patient with an inborn cellular immunodeficiency syndrome developed a graft-versus-host reaction after a transfusion with packed red cells. This diagnosis was confirmed by skin biopsy and finally proved by tissue typing.
A group of 57 newborns was divided into two: group I (27 subjects) received solely 1,000 I.U. vitamin D3 per day for 6 months, whereas group II (30 subjects) were given a combination of 1,000 I.U. vitamin D3 and 0.25 mg fluoride. At the end of the time of observation there was a significant difference between the plasma concentrations in the two groups. The median value was 13.06 microgram/l (range: 8:18-39.4 microgram/l) in group I and 16.54 microgram/l (range: 9.9--41.2 microgram/l) in group II. Fluoride is obviously available to infants when administered in combination with Vitamin D3. In addition, it could be proven, that after 6 month administration of fluoride there are no signs of cumulation, overdosage or even intoxication.

The determination of AFP in amniotic fluid for the prediction of neural tube defects fails in cases of myelomeningoceles which changes their status from the open to the closed form. AFP values laying in a borderline range require a fundamental clarification by ultrasound examination for the detection of spinal malformation.
Thirty patients with severe drug-induced agranulocytosis were admitted to the Ulm University Hospital between January, 1968 and October, 1976. All of 13 already infected patients treated with antimicrobial decontamination in a plastic isolation bed system survived. But nine of 17 patients treated in the open ward died. The cases were not randomised and there was a difference in age between the two groups, but nevertheless the results suggest that antimicrobial decontamination and isolation is superior to treatment in an open ward.
Respiratory distress following aspiration of meconium stained amniotic fluid can be found in about 3% of newborn infants. The lethality rate of this disease is high. Pathophysiology and variability of the clinical course as well as radiological findings and further diagnostic procedures are being described. The therapeutic concept requires recognition of meconium stained amniotic fluid as early as possible, early suctioning of the nasopharynx, intubation and endotracheal suction. Further therapy consists in physical procedures, i. e. lavage, suctioning, positioning, vibration and percussing of the chest, and inhalation. Antibiotics can only be recommended if infection of the amniotic fluid has been proven.
The therapeutic success of antibiotics used at the beginning of treatment and the effect of exchange transfusion in cases of septicaemia were tested in 22 newborn infants. The clinical course of these patients was compared with the outcome of 11 newborn infants who received antibiotic treatment without exchange transfusion. The following results were obtained: 1) All 6 patients initially receiving antibiotics, which were ineffective in vitro, died. In this group of patients the incidence of septic organ involvements (meningitis, ventriculitis, peritonitis) was significantly increased. 2) Following exchange transfusion, an impressive clinical improvement was consistently observed. 3) In patients who had initially received effective antibiotics and exchange transfusion, the lethality was significantly lower than in patients without exchange transfusion. 4) Our bacteriological findings show that continuous monitoring of cultures from blood, CSF and stool is necessary to choose the most effective antibiotic in the prevailing nosocomical circumstances.

The Thorax-Tube according to Matthys and its successful application in 18 premature and mature newborns and older children is described. The advantages of this new device are discussed.
The case histories of 17 newborns developing septic shock due to gram-negative bacilli were studied for pre- and perinatal risk factors, clinical symptoms, and hematologic changes. Immaturity, resuscitation procedures, and hypothermia on admission were found to be the risk factors most frequently preceding septicemia. A skin color fading and changing from reddish-pink to yellow-green was the most early noticeable clinical symptom in all patients. The total leukocyte counts as well as the relative proportion of bands increased significantly at the onset of illness. When septicemia advanced, a marked drop of leukocytes was found, while the relative proportion of bands increased further. Only 1 in 12 cases showed a decrease in the platelet counts at the height of septicemia. A procedure for the early diagnosis of a neonatal septicemia is proposed: (1) Registration of perinatal risk factors. (2) With perinatal risk factors a skilled and attentive clinical observation is necessary. Particular attention should be paid to changes of skin color. (3) White blood cell picture: (a) every day in patients with perinatal risk factors and (b) every 6 h in patients showing suspicious symptoms.
Serum digoxin concentration and half-life were radioimmunologically determined in 9 mature newborns after 7 days medication with digoxin. The newborns were in respiratory distress treated with continuous positive airway pressure or were suspected to have serious congenital heart disease. Loading dose was 26 mug/kg body weight intravenously and 35 mug/kg body weight orally, respectively. Maintenance dose corresponded to 1/8th of the digitalization dose twice daily. The serum digoxin level 12 h after the last dose varied between 1.4 and 2.5 ng/ml (mean 2.0 ng/ml, Sx=0.4). The serum half-life of digoxin varied between 21.7 and 42.4 h (mean 30.0 h, Sx=7.7). The mean serum half-life of digoxin of 30 h attained values found in adults without renal disease. This suggests that the serum digoxin levels of newborns which are usually higher if compared with those of adults result from higher digoxin doses per unit body weight and not from diminished digoxin elimination.

A 33 month old healthy girl accidently took 1.5 g phenylbutazone and 2.6 g aminopyrine. She was treated with gastric lavage and forced diuresis. No clinical symptoms were discovered for 17 hours after drug ingestion. She then developed generalized convulsions. After 19 hours reddish discoloration of urine was observed for first time. High serum transaminase indicated later on a toxic hepatocellular damage. It is recommended to treat patients with phenylbutazone aminopyrine insgestions for at least 24 hours even in the absence of reddish urine discoloration and clinical symptoms.
Although today most injuries of face and head are the result of car and motorcycle accidents, dog bites are a frequent cause of facial injuries in children. In recent years, general guidelines have been established concerning the surgical treatment of facial dog bite injuries in children. We have seen 16 children with such lesions at our hospital in the last 4 years. All children have been treated surgically, under general anaesthesia, by primary closure of the wound with interrupted sutures after having adapted the margins by subcutaneous sutures. In addition, all children have been evaluated 3 months-5 years after the accidents in order to objectify the results of our treatment. Special attention has been attached to the functional and aesthetic outcome. In all cases, we saw good-to-excellent results. Based on this experience we suggest a clinical classification of these injuries in regard to a special therapeutic regime for each stage. In this way, guidelines may be established for the surgeon dealing with these injuries.
