[Primary observation of a congenital intracranial teratoma in a twin with a healthy brother].

Primary intracranial teratomas are rare germ cell tumors arising from embryonic tissues, which contain cellular or tissue derivates from all three germ layers. Today the early intrauterine diagnosis by ultrasound is possible. We first describe a patient with such a tumor which was diagnosed by routine ultrasound in the 33rd week of gestational age in a twin-pregnancy, of which the 2nd twin developed normally. This must be considered in the obstetrical situation. In such a case the parents should be advised accordingly.

[Serum potassium level in healthy neonates and infants with asphyxia in the first 144 hours of life].

Postpartal increase in the plasma potassium concentration of asphyxiated newborn infants is a very rare complication with possibly deleterious outcome. In a group of 98 asphyxiated and 87 healthy infants we have not seen either a case of severe hyperkalemia or rhythm disturbances. We found lower reference-ranges for plasma potassium concentration in the group of healthy infants than previously published, which we would like to introduce. The pathogenesis of hyperkalemia in hypoxia is not yet fully understood. Further experimental investigations will be necessary.

Asphyxia Neonatorum↗

[Sonographic detection of an intragastric blood clot in neonatal hemorrhage disease].

Caused by a hemorrhagic disease of a newborn without laboratory findings of coagulopathy, a dramatic blood loss due to gastrointestinal bleeding was observed in a high risk female newborn of 37. weeks of gestational age. By ultrasound the globular 3 centimeter diameter tumor in the stomach was defined as a blood clot. It disappeared in the following days under ultrasound observation.

[Acute hematogenic osteomyelitis after impetiginized scabies].

A 15 year old boy with scabies has been treated with local form of therapy for 3 month without effect. He was then admitted to our pediatric clinic with pyrexia and restricted movement of the right knee. The med. history, the consequent laboratory findings, positive bloodcultures, skin swabs and bone scintigraphy lead to diagnosis of acute haem. osteomyelitis. The early antibiotic therapy assured uneventful recovery.

[Fatal course of infectious mononucleosis in an 11-month-old girl].

Fatal infectious mononucleosis is vary rare in the human population. Only two case reports of girls suffering an Epstein-Barr virus-associated lymphoproliferation without evidence of an underlying immunodeficiency came to our knowledge. We report on the case of an 11-months-old girl with fatal infectious mononucleosis. Some findings allow distinct delineation from previous reports. Firstly, the present "pulmonary lymphoid hyperplasia" has been formerly described in patients with HIV infection exclusively. Secondly, only the EBV surface antigen LMP was expressed on infected B-cells. The nuclear antigen complex EBNA could not be demonstrated. Overall, the results suggest a so far unrecognised type of EBV-associated lymphoproliferation in a female infant.

[General anesthesia or peridural anesthesia in primary cesarean section; a comparative study].

In a prospective cooperative study of gynecologists, anaesthesiologists and pediatricians we compared the effect of continuous epidural anaesthesia (23 cases) and of general anaesthesia (24 cases) on mother and newborn. All patients were delivered in the 39. to 41. week of an uncomplicated pregnancy by primary caesarean section for breech presentation or disproportion. Nearly one third of the patients with epidural anaesthesia showed an initial transitory decrease in blood pressure- up to 25%. The mean pO2 was higher (155 mmHg) in the group with the general anaesthesia than in the group with epidural anaesthesia (88 mmHg). Corresponding values for pO2 were found in the umbilical artery (22,8 mmHg after general anaesthesia, 16,6 mmHg after epidural anaesthesia). Concerning the pH in the umbilical artery, the better results were obtained in the group with continuous epidural anaesthesia (7,3 versus 7,27). After general anaesthesia 9 children had a one minute Apgar-score below 8 versus 3 children after epidural anaesthesia; five and ten minutes after birth the Apgar scores were practically identical. The neurologic examination 15 min after delivery revealed a pathologic muscle tone in three cases after general anaesthesia and in one case after epidural anaesthesia. After 7 days all children showed a normal neurobehavior. As no severe differences between the two types of anaesthesia in the effect on mother or newborn could be seen, in uncomplicated pregnancies the choice should be left to the mother.
