[Effects of diagnostic and treatment errors on survival of patients with stage I malignant tumors of the ovary (data of the Tatarstan state oncological dispensary in Kazan)].

The influence of diagnostic and tactical errors on the survival has been studied in 86 patients with malignant tumors of the ovaries (stage I). It has been shown that the survival and the duration of life depend on the correctness of diagnosis and treatment, and diagnostic errors have a more substantial influence on the disease prognosis than tactical ones.

[Results of simultaneous resection and plastic surgery of the esophagus with extra-cavitary cervical esophagostomy in esophageal cancer in its thoracic part].

The work analyses the results of one-stage resection and plastics of the esophagus with a tube formed from the greater curvature of the stomach and creation of an ++extra-cavitary anastomosis on the neck in combined and surgical treatment of carcinoma of the thoracic esophagus in 279 patients. Various complications occurred in 181 (64.8%) patients. The mortality was 19.3% (16.8% among patients who underwent radical operation and 25.6% among those treated by a palliative operation). The number of complications in the groups of surgical and combined treatment was approximately equal. The results of 5-year survival were better in the group with combined treatment. Postponed + extra-cavitary anastomosis was formed in 65.2% of cases. One-stage resection and plastics of the esophagus with ++extra-cavitary anastomosis is an adequate operation from the oncological standpoint in the treatment of carcinoma of the thoracic esophagus. Its further perfection is necessary for improvement of the immediate and late-term results of treatment.

Fibre-laser IR luminescence diagnostics of malignant tumours using rare earth porphyrins.

To eliminate the masking effect of the background luminescence of endogenic substances in biological tissues and to increase the luminescence contrast values of malignant tumours, sarcoma-implanted mice were administered with ytterbium porphyrins instead of free porphyrin radicals. This was followed by in vivo luminescence localization of tumours using fibre-laser spectrofluorometry. The luminescence contrast values obtained reached 10-45 which is evidently not a limit.

Photodestruction in vitro of tumour cells sensitized by porphyrins and their conjugates with specific antibodies.

The photodynamic action of a number of carboxylic porphyrins and their covalent conjugates with specific monoclonal IgM antibodies on a human lung adenocarcinoma cell line was studied. All the conjugates showed strong phototoxicity which did not correspond with the phototoxicity of the free porphyrins. Absolute quantum yields of singlet oxygen photogeneration by the porphyrins and their conjugates were obtained and compared with the phototoxicity of the compounds.

[The clonogenic capacity of human lung cancer cells].

Comparative studies of clonogenic ability of twenty four lung cancer samples have been performed in semisolid agar in vitro and in diffusion chambers in vivo. It is concluded that plating efficiency of lung cancer cells varies from 0.00002% to 0.03% in vitro assays and from 0.01% to 0.11% in vivo. A small number of colonies (less than 20) in 15 specimens from 16 does not permit using this method for preclinical individual sensitivity test. The cloning of lung cancer in vitro gives the efficiency more than twenty colonies per a dish in 5 cases of 11. There is some evidence that these models can be used for prediction of treatment response of individual tumors to chemotherapy.

[Characteristics of pulmonary cancer metastasis to thoracic lymph nodes in primary tumor (T1)].

The characteristic features of primary T1 tumor metastasis to the intrathoracic lymph nodes were studied in specimens removed during surgical treatment of 404 patients with lung carcinoma. Regional metastases were detected in 31.9% of cases. The frequency of metastases in central carcinoma was determined by the level of involvement of the bronchus. The percentage of affection of the regional collectors was highest in patients with small-cell carcinoma. It is shown that in primary T1 lung carcinoma not only the bronchopulmonary and root but also the tracheobronchial lymph must be removed and examined immediately during the operation.

[Approaches to predicting the reaction of human tumors to radiotherapy].

Immunofluorescent method using anti-thymidine antibodies was employed to assess proliferative activity of 217 human tumors including oropharyngeal, esophageal, gastric, rectal and lung cancer. The activity was evaluated before and, in 38 neoplasms, in the course of radiotherapy. It was shown that changes in the proliferative activity of oropharyngeal and gastric malignancies observed early in the course of radiation treatment may serve for predicting response.

[Organization of ambulatory care of cancer patients].

The paper reveals shortcomings of medical care for cancer outpatients. Basing on cancer incidence and analysis of the activities of outpatient oncological units, measures are brought forward to advance prophylactic, diagnostic and therapeutic service for cancer outpatients.

[Errors in the diagnosis of malignant tumors during the prehospital stage].

A group of experts from The P. A. Herzen Research Institute of Oncology reviewed 6000 histories of cancer. The analysis showed errors by outpatient hospital staff to account for untimely diagnosis of cancer in the majority of cases of advanced disease. A classification of medical errors was developed. Objective and subjective factors leading to errors were analysed. Review of records of diagnostic data is suggested. It is also suggested that physicians should bear responsibility for errors in oncology which have grave consequences.

[Organ-sparing surgery in cancer of the lung].

Indications for different variants of functionally sparing operations in carcinoma of the lung were determined from study of the long term results. Comparison of 5-year survival after pneumonectomy and lobectomy showed the adequacy of the latter in advancement of T1-2N0M0 tumor (39.7% and 37.0%). In T1-2N2M0, these values were 35.7% and 34.5%, respectively. The indications for the type of bronchial resection and lobectomy (wedge, sleeve) were specified depending on the level of the neoplastic lesion of the bronchi and the condition of the intrathoracic lymph nodes. The problem of sublobar lung resections (segmentectomy, wedge, marginal) in early forms of peripheral carcinoma is discussed. The high efficacy of endoscopic removal of early forms of central carcinoma of the lung is shown on the basis of analysis of 54 cases.

[Criteria for selecting patients with breast cancer for organ-saving operations].

The study discusses the results of treatment of 597 cases of stage I-IIa breast cancer who underwent either mastectomy after Halsted, Patey or Pirogov (434 patients), or organ-saving radical resection of the breast (163). The analysis of the data identified biologic, morphologic, psychologic and anthropometric criteria as well as laboratory tests which may serve as criteria for selecting candidates for organ-saving surgery and adjuvant radiotherapy. In a group of patients who met the criteria, sparing surgery yielded good results in terms of five-year survival rate (88.9%) and degree of socio-occupational rehabilitation.

[Immunological tests in a follow-up of patients operated on for gastric cancer].

Timely detection of recurrences and adequate therapy thereof together with improvement of methods for multiple-modality treatment may essentially improve remote results of gastric carcinoma treatment. The authors' studies have demonstrated the usefulness of immunologic tests carried out in the postoperative period for the prediction of the results of surgery: these tests are highly informative in the course of a postoperative follow-up.

[Immediate results of the use of prospidin in a cyanoacrylate glue compound in cancer of the esophagus].

Antitumor efficacy of a medical adhesive MK-8-based polymer drug composition with prospidin has been studied in 49 Wistar rats, and the kinetics of the drug elimination has been followed up in 112 rats. The findings evidence a prolongation of a relapse-free period and a longer survival of the animals. Clinical trials of the composition have been carried out. Of the 70 patients with esophageal carcinomas 52 ones had local disseminated forms, conforming to T4P3-4 indices. Application of the adhesive composition has not increased the rate of postoperative complications and in some cases considerably prolonged the life span of patients after palliative surgery.
