[Preventive activities of a district physician-internist].

The results of prophylactic examination of a district population were compared with medical documents. Information on 85.7% of patients with CHD, 70.5% with arterial hypertension, 80.3% with chronic bronchitis and 83.3% with diabetes mellitus detected during the examination was absent in the medical documents. Prophylactic examination of the population, education of a district physician in methods of primary and secondary prophylaxis of chronic incommunicable diseases, the availability of health educational literature brought about an increase in the volume of therapeutic, diagnostic and preventive activities in this district, however they did not have great influence on the prevalence of risk factors in the population over a 2-year period.

[Conservative treatment of breast cancer].

The paper is concerned with clinical characterization of 81 breast cancer patients after conservative radio- and hormonotherapy. Of the total number of patients 47 suffered from inoperable tumors as a result of local spreading, 30 patients had serious contraindications for the operation, and 4 patients refused surgical intervention. Of the total number of patients 85% were aged over 50, 69%--over 60.41%--over 70, and 4 patients--over 80. The results of radiotherapy after a radical program and administration of antiestrogens (tamoxifen, zitosonium) were evaluated in 71 patients: in 42 tumors were undetectable, in 16 they were reduced by more than 50% of their primary size. An analysis of short-term results during 1-year follow-up showed that of 50 patients complete remission was observed in 24, stabilization of a tumor process was achieved in 8. The results should be regarded as promising with relation to the patients' age, status, tumor spreading, and concomitant diseases.

[Combined treatment of primary inoperable breast cancer].

The paper is devoted to the clinical characterization and results of therapy of 45 patients with locally advanced, primary inoperable cancer. Most of the women were at the age of 40-60, of them 64% had preserved menstrual ovarian function. Multimodality therapy included radiation therapy combined with chemotherapy (UAM and CMF schemes). The breast and all zones of regional metastasis were irradiated at a single focal dose of 2 Gy, the total focal doses were 67-70 Gy to the breast, 50 Gy to the axillary and supra- and subclavicular areas, and 34-40 Gy to the parasternal area. This therapy was followed by radical mastectomy on 30 patients. A degree of expression of a clinical effect and therapeutic pathomorphosis was emphasized. Proceeding from 2-year long-term results, multimodality therapy including surgical intervention was found appropriate.

[Mitogenic action of substance P on the epithelial cells of the tongue].

Substance P (10 and 100 ng/ml) stimulates the proliferation of basal cells from rat's tongue epithelium in primary cultures with 2.5% fetal calf serum. In serum-free medium substance P have no effect on the epithelial cell growth. This neuropeptide secreted by afferent nerve fibers of the tongue epithelium is suggested to have a neurotrophic influence on epithelial cells controlling their proliferation.

[Preventive examination of the population of the medical district of an urban polyclinic].

Prophylactic examination of the population in a district of a city out-patients clinic was performed. Altogether 689 persons (298 men and 391 women) aged 16 to 64 were examined. A great prevalence of chronic non-communicable diseases (CND) and common risk factors were revealed: CHD in 16.7% of the examinees, cerebral changes in 0.6%, diabetes mellitus in 3.9%, chronic bronchitis in 9.3%, arterial hypertension (AH) in 29.6%, smoking in 26.1%, disturbed carbohydrate tolerance in 15.7%, hypercholesterolemia in 13.5% and excess body mass in 35.5%. In 40% of the patients with AH it was combined with other chronic diseases necessitating a multidisciplinary approach to CND control in the patients with AH. The study showed that the population was not ready enough for prophylactic examination.

[An experience with the conservative treatment of breast cancer in middle-aged and elderly women].

Advantages of conservative treatment of inoperable breast cancer in old and elderly age are discussed on the basis of an analysis of data on 30 cases. A split-course radical radiotherapy was combined with a prolonged tamoxifen treatment. Complete regression of tumor was registered in 16 out of 23 followed-up patients, while more than 50% regression--in I case. The study should be continued.

[Attitude of district physicians and the population toward measures regarding the prevention of ischemic heart disease].

The experience of the work on multifactorial prophylaxis of coronary heart disease performed on the basis of an urban out-patient center among 40-59-year-old males points to an inadequate response of the population to the primary (57 % after the mailing of 4 invitations) and repeated annual (50-60% of those who have had the primary examination) prophylactic examinations. Over 4 years, 22 % of patients with coronary heart disease and 32 % of patients with arterial hypertension refused to come for out-patient examinations. The number of responses was greater among individuals with risk factors of coronary heart disease. Most therapists from out-patient clinics believe it necessary to carry out prophylactic measures and would like to have more time for this purpose. Yet, many of them doubt that the population will follow medical advice with regard to changing the life style. The population is poorly aware of the role of prophylactic examinations, the effect of nutrition and bad habits on the development of cardiovascular diseases.
