[Physiological significance of gastroduodenal blood flow and possibilities of its regulation by drugs].

The authors review the role of the gastroduodenal blood flow as a factor ensuring the secretory activity and protective properties of the gastric mucosa. The possibilities of its pharmacoregulation are also reviewed. It has been shown that the decrease of the gastric blood flow in experimental hemorrhagic shock, extravasal celiac artery stenosis, effects of indomethacin, vasopressin, etc. leads to ischemic injuries to the gastric mucosa accompanied by serious morphological and functional disturbances. The pathogenetic aspects of ischemic injuries to the stomach and duodenum are confirmed by the results of clinical examinations of patients with compression celiac artery stenosis and peptic ulcer. It has been noted that in these patients, the tissue blood flow in the mucosa in considerably decreased in the area of ulcerous defect. The lowering of the content of total phospholipids and changes in their composition were also observed both in relapse and after healing. Out of agents that improve peripheral circulation, propranolol, tavegil combined with cimetidine, trimin appeared most potent. These drugs regulate metabolic processes in gastric tissue, normalizing blood flow, reducing the content of histamine, serotonin, lipid peroxidation products, raising the level of total phospholipids and pepsin.

[Morphologic characteristics of lesions of the small intestinal mucosa in chronic ischemia].

The results of morphologic examination of the small bowel mucosa of 32 patients with the ischemic disease of the alimentary organs provoked by stenosis of a. iliaca are analyzed. The diagnosis was confirmed angiographically and in the course of operation. Aspiration biopsy of the jejunum was performed by means of Ts. G. Masevich's sound. Chronic intestinal ischemia provokes the development of the atrophic changes in the mucosa with the decrease of its thickness due to the lowering of the height of its villi. The number of the goblet cells is increasing as well as the cell infiltration, the degenerative changes in the epithelium are observed. These alterations correlate with the degree of both the vessel narrowing and collateral circulation.

[Acute nonocclusive ischemic lesions of the intestine].

The study was performed of acute nonocclusive ischemic intestinal disorders in 47 patients with etiologically heterogenic diseases: myocardial infarction, shock, essential hypertension, acute cardiovascular insufficiency, systemic vasculitis, psychoemotional stress. Pathogenetic mechanisms underlying acute intestinal disturbances in the absence of occlusions were found similar. The leading of these mechanisms is believed defective visceral microcirculation.

[Clinico-morphologic characteristics of ischemic involvement of the pancreas].

Follow-up clinical results have been analyzed for 564 patients with occlusions of visceral arteries, which presented clinically as symptoms of chronic pancreatitis in 115 patients. As a rule, chronic pancreatic ischemia leads to exocrine hyposecretion evident from the derangement of cyclonucleotide metabolism. There were also metabolic shifts in pancreatic cells related to hypoxia. Intravital biopsies revealed dystrophy of the acinar tissue as well as inter- and intralobular infiltrates of the connective and fatty tissue.

[Repeated operations on the extrahepatic bile ducts].

Experience in repeated operations on the biliary tract in 51 patients is analysed. Endoscopic retrograde cholangiopancreatography is one of the informative methods. The choice of the method of operative treatment must be individualized according to the nature of the pathological process in the biliary system. An endoscopic intervention may be an alternative to surgical operations in many cases.

[Method of surgical intervention for compression stenosis of the celiac trunk].

Experience with the surgical treatment of 265 patients with a compressive stenosis of the celiac trunk has been analyzed. Stenosis of the celiac trunk is known to be a cause of ischemic abnormalities of the stomach, duodenum, pancreas. Decompression of the celiac trunk is an effective method of treatment of the ischemic disease of digestion organs resulting in better organic blood circulation and involution of the appearing pathologic processes. In a number of patients there appeared symptoms for associated interventions due to complications of ischemic gastroduodenal ulcers or concomitant diseases. Excellent and good remote results were noted in 79,6%, satisfactory--in 16,7%, unsatisfactory--in 3,7%.

[Biogenic amine content of rat gastric tissues during reduced gastric circulation].

The content of biogenic amines in gastric tissues was studied in rats with celiac trunk stenosis 3 hours, one day, 2 and 7 days after operation. Chronic ischemia of the gastric wall was found to lead to the development of dystrophic changes in the gastric mucosa, accompanied by the decreased catecholamine content and elevated levels of histamine and serotonin.
