[Technic and results of Fox bridge plasty for covering defects of the lower lid].

The present paper communicates results of reconstruction of lower lid defects following tumor excision. Surgery was performed on 25 patients using a modified Fox procedure. Functionally and cosmetically the results were good. Three patients developed an ectropion of the lower lid, one of them requiring surgical repair. In 3 cases there was some loss of cilia, though the patients did not find this disturbing. In 2 cases a minor sagging of the lower lid border was seen. A modified version of Fox's original method is proposed as a means of avoiding these complications.

Mezlocillin concentrations in human aqueous humour after intravenous and subconjunctival administration.

27 patients received either 4 g intravenous infusion 1-3 h or 100 mg subconjunctival application of mezlocillin 1-12 h before cataract extraction. After intravenous administration the mean aqueous humour concentration of mezlocillin was 2.9 micrograms/ml after 2 h. The subconjunctival dose produced a mean aqueous humour concentration of 23 and 2.6 micrograms/ml after 3 and 12 h, respectively. These levels are above the minimum inhibitory concentration of mezlocillin for sensitive organisms.

Aggregate formation of erythrocytes and diabetic retinopathy in children, adolescents, and adults with diabetes mellitus (type I).

In vitro measurements were carried out to study the aggregation of erythrocytes in 33 children and young adolescents, three older adolescents, and 38 adults with type I diabetes. The aggregate formation of erythrocytes in stasis was increased in adult patients with both "good" and "poor" metabolic control when compared to control values. The aggregation of red cells in all children and the younger adolescents, both those under good and poor metabolic control, did not statistically differ from those of controls. Each of the three older adolescents showed an increased aggregate formation of their erythrocytes compared to controls. All adult patients under poor metabolic control exhibited various stages of diabetic retinopathy as shown by fluorescence angiography. Only one child under poor metabolic control showed an early stage of retinopathy (stage I according to Malone). The three older adolescents showed an increased aggregate formation of their erythrocytes. These patients exhibited also stage I of retinopathy. We suggest that the increased aggregation of erythrocytes of both adult and older adolescent patients may be one of the reasons for the development of diabetic retinopathy. Moreover, the normal erythrocyte aggregation of the diabetic children and younger adolescents could help to explain the rare occurrence of microangiopathies in childhood diabetes.

Hereditary ocular dysproteinhydria of the aqueous humour with crystalline deposits.

We describe a family with coloured crystalline deposits on the corneal endothelium and on the lens surface. A protein with a molecular weight of about 110,000 and an isoelectric point of 7-8 could be demonstrated in the aqueous humour, which was not found in the controls or in the serum of the family members. The pedigree is suggestive of autosomal dominant inheritance. To our knowledge this is the first report of this entity.

[Results of keratoplasty à chaud].

The results of keratoplasty à chaud in 49 eyes are reported. None of the eyes had to be enucleated. In 43% the transplant remained clear. The prognosis was best in cases of bacterial or fungal ulcers, provided that vascularization did not involve more than one quadrant. The results are compared with those of other authors who recommend the use of tissue adhesives first and only perform corneal grafting some months later.

[Psychogenic vision disorders: a case report with psychosomatic findings].

The diagnosis of psychogenic visual disturbances, a phenomenon not so very rarely to be observed, is often made by an ophthalmologist, whereas the patients concerned will seldom consult a psychotherapist. We describe somatic, psychiatric and psychological data in 8 observed cases. Results of psychodynamic interviews point to underlying, especially oedipal conflicts. Slight confidence in one's own possibilities on the one hand and incomplete defence of urge on the other may here, too, lead to functional symptoms in special conflict situations. Need for and possibilities of careful diagnostic are discussed, as well as special courses of psychogenic visual disturbances and therapeutic changes.

Azlocillin concentrations in human aqueous humor after intravenous and subconjunctival administration.

Twenty-nine patients received either 4 g intravenous infusion or 100 mg subconjunctival application of azlocillin 1-12 h before cataract extraction. After intravenous application the mean aqueous humor concentration of azlocillin was 4.4 micrograms/ml after 3 h. The subconjunctival dose produced a mean aqueous humor concentration of 78 micrograms/ml and 3.4 micrograms/ml after 3 and 12 h, respectively. These levels are above the minimum inhibitory concentration of azlocillin for sensitive organisms.

Visual evoked potentials in neurosyphilis.

The visual evoked potential (VEP) to pattern reversal was recorded in 79 patients with neurosyphilis. Sixteen patients (20%) had abnormal VEP latencies with a predominance of pathological VEP values in the group of tabes dorsalis (50%) as compared to general paresis (18%) or meningovascular forms (13%). A comparison of the frequency of abnormal VEPs with that of other ophthalmological tests (visual acuity, visual field, central campimetry, pupillary reactions, dark adaptation, optic fundus) yielded no diagnostic superiority of VEP.

Biochemical and therapeutical studies in a case of atrophia gyrata.

A thirty-six years old man from an inbred family with the typical clinical picture of Atrophia gyrata chorioideae et retinae was found to have hyperornithinemia and a partial deficiency of ornithin-ketoacid-transaminase activity. The residual activity was stimulated in vitro by high concentrations of pyridoxal phosphate. We have initiated a therapeutic study with vitamin B6 per os accordingly. Comparitively low doses may be sufficient for long term treatment. The necessity to start therapy early in life is emphazised. Possible mechanisms of the pathogenesis of Atrophia gyrata are discussed.

[Psychogenic visual disturbances in children and adults. Ophthalmological, neurological and psychodynamic findings].

In large series of ophthalmic patients there may occasionally be some in whom the reduction of visual acuity cannot be explained by organic findings. In such patients simulation is suspected. However, simulation is not the only cause of decreased visual acuity and a lack of organic pathologic findings. It is obvious that all efforts must be made to exclude organic changes as the cause of decreased visual acuity. With modern diagnostic modalities this is possible to a large extent. If there are no organic pathologic findings, a psychiatric exploration is indicated. In the eight patients presented in this paper the reduction of visual acuity was the expression of a psychologic conflict situation. The cases are described in detail and the difference between simulation and a psychogenic decrease of visual acuity is pointed out.
