[Clinical and immunologic findings in BCG osteomyelitis].

BCG-osteomyelitis is a rare but serious late complication of BCG-immunisation. Be present clinical and immunological findings in 2 children aged 9 months and 5 years, who developed BCG-osteomyelitis, following immunisation with BCG-vaccine. The affected bones were the first metacarpal bone in one and the 6th rib in the other patient. Both children remained free of recurrences after surgical removal of the granulomatous tissue. Immunological investigations in both cases demonstrated normal humoral immunity (Complement CH50, C3, C4, immunoglobulins). Except for a diminished proportion of circulating B-cells, determination of lymphocyte subpopulations and in-vitro lymphocyte stimulation with Phytohemagglutinin and PPD were normal. In both children spontaneous cell mediated cytotoxicity (SCMC) against the tumor cell line L1210, in one patient also against tumor cell line K562 was decreased. Antibody dependent Cytotoxicity (ADCC) was normal.

[Acupuncture and laser treatment in cervical and lumbar syndrome].

Eight random groups, each of 15 patients with tendomyotic cervical and lumbar syndrome, underwent acupuncture, one group receiving needle acupuncture at the "correct" sites according to Chinese medicine and another receiving placebo needles at the same sites. In a third group, acupuncture needles were applied at sites which were wholly inconsistent with Chinese practice. The remaining 5 groups took part in a single blind trial involving various types of radiation at the correct acupuncture sites, using an apparatus emitting either laser rays, red light, mixed light, infrared or no radiation at all, without interference from the therapist. All the groups showed improvement, but an analysis according to Kruskal and Wallis revealed no significant differences between type and site of treatment.

Synthesis and properties of acrylamide-substituted base pair specific dyes for deoxyribonucleic acid template mediated synthesis of dye polymers.

We have tried to construct synthetic polymers for sequence-specific recognition and complexation of longer deoxynucleotide sequences. For this purpose, we developed a method of template-directed polymerization of basic pair specific DNA ligands such as basic dyes. The template-directed polymerization consists in a copolymerization of various dyes of different specificities staying simultaneously in a binding equilibrium with DNA. In the present paper, we describe the synthesis and the properties of base pair specific monomers especially designed for performance of radical chain polymerization reactions in aqueous medium at room temperature. Different acrylamide derivatives of well-known dyes, such as AT-specific malachite green and GC-specific phenyl neutral red, were synthesized and studied for their ability of base pair specific complex formation with DNA of different base composition. Partition equilibrium dialysis and dye titration agarose gel electrophoresis were used to ensure for several dyes that they may be incorporated into different polymers via copolymerization of their acrylamido derivatives with various small base-unspecific monomers without substantial change of their binding parameters. Furthermore, we demonstrate that acrylamide and other small acrylamide derivatives can be used as building blocks for the synthesis of polymeric links between base pair specific monomers. The results and their consequences for template polymerization reactions are discussed.

Correlation between ultrastructural and functional changes in sarcoplasmic reticulum during chronic stimulation of fast muscle.

Chronic indirect stimulation of fast twitch rabbit muscle (tibialis anterior and extensor digitorum longus) with a frequency of 10 Hz induced a progressive transformation of the sarcoplasmic reticulum (SR). Ultrastructural changes as studied by electron microscopy of freeze-fractured vesicles consisted in a decrease of intramembranous particles of the concave (A) face and an increase of particles in the convex (B) face. The asymmetry of the membrane proved to be lowered. Changes in the particle density of the A face were mainly confined to the 7-9 nm particles. Electrophoretic analyses revealed a decrease in the 115,000-Mr Ca2+ transport ATPase. The reduced density of the 7-9 nm particles correlated well with decreased activities in Ca2+-dependent ATPase as well as with decreases in initial and maximum Ca2+ uptake.

[The treatment of psoriasis-arthritis with cyclosporin A, a new immunosuppressive agent].

The treatment of psoriasis and psoriatic arthritis by cyclosporin A, a new immunosuppressive agent believed to have a largely selective action on T lymphocytes, is described. Results in 4 cases show that cyclosporin A can bring about a most impressive improvement in psoriatic lesions of the skin and its appendages, but that its effect on the arthropathy is variable. There is still little certainty about the compound's mechanism of action in these conditions. Cyclosporin A may give rise to a whole range of side effects and its use--like that of other immunosuppressants--should therefore be contemplated only in severe and progressive forms of psoriasis and psoriatic arthritis resistant to other forms of treatment except cytostatic drugs.

Polyethylene glycol derivatives of base and sequence specific DNA ligands: DNA interaction and application for base specific separation of DNA fragments by gel electrophoresis.

Various base pair specific DNA ligands comprising a phenyl phenazinium dye, a triphenylmethan dye and Hoechst 33258 were covalently bound to polyethylene glycol (PEG) via ester or ether bonds. The DNA interactions of the PEG derivatives formed were shown to exhibit the same base pair specificity as the parent compounds. Since the PEG chains thus bound to the DNA could be expected to increase drastically the frictional coefficient of the DNA, the PEG derivatives were used for base specific DNA separations in agarose and polyacrylamide gel electrophoresis. The procedures, which do not require any special techniques, are described in detail. The resolution observed in agarose gels allows one to separate equally sized DNA fragments differing as little as 1% in base composition at mean travel distances of about 10 cm. Examples of gels showing the base compositional heterogeneity of restriction fragments obtained from lambda DNA, E. coli DNA and calf thymus DNA are given.

Acute leukemia after cytotoxic treatment for nonmalignant disease in childhood. A case report and review of the literature.

The case history of an 11-year-old boy is reported. The child developed the nephrotic syndrome at the age of 17 months. He was treated with prednisolone, cyclophosphamide, cytosine-arabinoside and chlorambucil for the following three and a half years. Three and a half years after cessation of any medical treatment he developed anemia, thrombocytopenia and a diagnosis of acute lymphocytic leukemia (ALL) was made. Subsequently he responded to the West Berlin Protocol. Reviewing the literature, another eight cases of acute leukemia after cytotoxic treatment for nonmalignant disease in childhood have been reported. Most often the myeloblastic form of acute leukemia was diagnosed. Even though the pathogenic mechanism of acute leukemia after cytotoxic treatment is not known, the possibility must be considered that the incidence of this malignant disease is increased after cytotoxic treatment for nonmalignant diseases.

Studies on metals in meningiomas by atomic absorption spectrometry.

The content of Cr, Mn, Cu, Mg, Ca, Zn, and Fe has been determined by atomic absorption spectrometry in 30 meningiomas and in Pacchionian granulations. The elements Cr, Mn, and Cu show no distinct relation to different tumor types. The broad spectrum of data for Fe is considered to be due to a different blood volume or microbleedings in the meningiomas clinically irrelevant in nature. The Ca-values correspond with the microscopic calcifications. An obvious coincidence exists partly between the high Ca- and Mg-values, in some tumors concentrations of Mg are partly higher than the concentrations of Ca. The latter finding is in concordance with tumors without microscopic calcifications. It is assumed that the inhibitory potency of Mg against mineralization reflects the metabolic antagonism between Ca and Mg. The highest values for Zn are seen in fibromatous meningiomas. This finding possibly corresponds with the function of Zn as co-enzyme in the synthesis of proteins in this copiously reticulin- and collagen-producing type of meningioma.

Dependence of the mutagenic power of heteroatomic dyes on their DNA-base-pair specificity.

23 compounds structurally related to proflavine were tested with Salmonella typhimurium TA1537 for their ability to induce mutants in the quantitative mutagenicity test of Ames et al. Of these compounds, 13 were mutagenic. Their mutagenic power decreased with increasing GC specificity of the dye. Because the mutational site in the tester strain is assumed to consist of a run of 4 adjacent GC pairs of the same polarity we conclude that for mutation to occur the dye should not be bound within this run of identical base pairs. Some of the compounds showed a negative slope at higher concentrations. Because these dyes also showed a comparatively high tendency to dimerize we conclude that mutation induction requires the intercalator to be bound as a monomer. Those compounds that were not mutagenic either did not intercalate or were inactivated by reduction.

Immunofluorescence studies on the subcomponents of the first component of complement (C1): detection of C1q and C1s in different cells of biopsy material and on human as well as on guinea pig peritoneal macrophages.

The first component of complement (C1) is a macromolecule consisting of three distinct subcomponents, C1q, C1r, and C1s. In regard to its production site and its role in phagocytic processes it was of interest to find out whether these different subcomponents could be detected in human biopsy material only as a complex in individual cells or whether C1 subcomponents could be found on different cells. To study this question, monospecific fluorescein-labelled anti-human-C1q IgG and monospecific rhodamine-labelled anti-human C1q IgG were used. Biopsy material from human rectum was stained with fluoresceinated antisera, either by use of one antiserum or by double staining. Using this technique, these observations were made: C1q as well as C1s were detectable in individual cells in the subepithelial area of the gut. Furthermore, C1q and C1s could be found together in the same cell or separately in different cells. These findings were supported by experiments with cultured peritoneal macrophages either from human or from guinea pig. The examination of the cultured cells with the two antisera revealed that individual cells were stained either by anti-C1q or by anti-C1s antibodies. The specificity of the detection of the individual subcomponents was also proven by the peroxidase technique and by using fluoresceinated anti-human C1q F(ab')2. The membrane immunofluorescent staining revealed the presence of C1q on the membrane of the macrophage.

[Siemens I syndrome--a case history].

Report on a patient suffering from keratosis follicularis spinulosa decalvans (Siemens I syndrome). This ia a very rare clinical picture, at least in its complete form. The patient's father and brother are suffering from an abortive Siemens I syndrome. The condition is a form of hereditary parakeratosis, a spinulose keratosis with typical localization in the eyes and on the skin. Causal therapy is not possible; attempts at conservative and surgical treatment are described. Prognostically, the condition will probably come to a standstill at puberty. The description of this case is intended to illustrate the necessity of close cooperation between dermatologist and ophthalmologist.
