[Psoriasis therapy with fumaric acid and fumaric acid esters].

Fumaric acid may not be regarded as an antipsoriatic drug. Beneficial effects on psoriatic lesions may be explained by secondary changes such as the acidification of gastric juice in cases of anacidity or hypacidity . Monoethylfumarate exerts true antipsoriatic activities but is by far too toxic for clinical use. Experimental investigations have confirmed an inhibitory action of the above-mentioned fumarate on nucleic acid synthesis and protein synthesis of PHA-stimulated human lymphocytes.

[The activity of imidazole derivatives in the presence of human plasma (author's transl)].

By Warburg technique using yeasts and staphylococci, any decrease in antimicrobial activity of the imidazole derivatives could be excluded in the presence of human plasma. Econazole nitrate, the classical topical imidazole derivative, and ketoconazole, an oral active antifungal, were tested. The compatibility between imidazole derivatives and human plasma is of practical importance for obtaining optimal therapeutic results in treating human mycoses.

Antifungal Agents

[Effect of long-wave ultraviolet light (UV-A) and medium-wave ultraviolet rays (UV-B) on human skin. Critical comparison].

When discussing the effects of ultraviolet radiation on human skin, one should carefully distinguish between the long wave ultraviolet light (UV-A) and the short wave radiations (UV-B and UV-C). Ultraviolet A induces immediate pigmentation but, if high energies are applied, a permanent pigmentation is elicited. This type of ultraviolet A-induced pigmentation has been called "spontaneous" pigmentation as no erythematous reaction is necessary to induce or accelerate melanine formation. Ultraviolet B provokes erythema and consecutive pigmentation. Upon chronic exposure, ultraviolet B causes the wellknown actinic damage of the skin and even provokes carcinoma. With exposures to the sunlight (global radiation), one should be most careful. The public must be informed extensively about the dangers of excessive sunbaths. The use of artificial "suns" with spectra between 260 and 400 nm is limited as it may cause the same type of damage as the global radiation. An exact schedule for use of artificial lamps is strongly recommended. After one cycle of exposures, an interruption is necessary until the next cycle of irradiations may start. Upon continual use for tanning of the skin, artificial lamps may provoke irreversible damage of the skin. Radiation sources with emission spectra of wavelengths between 315 and 400 nm exclusively are well suited for the induction of skin pigmentation (cosmetic use). Potent radiation such as UVASUN systems provoke a "pleasant" permanent pigmentation after exposures for less than one hour. The use of ultraviolet A (UV-A) does not carry any risk for the human skin.

Nephrotoxicity of drugs, evaluated by renal enzyme excretion studies.

Via various pathways, urinary enzymatic activities are changed by drugs. Nephrotoxic effects may be determined by such investigations. Numerous experimental series have formed the basis for the clinical application of this method. The pertinent data collected with analgesics and antibiotics, alone or in combinations, are discussed in this paper. The data from the literature were compared with personal investigations, in animals as well as in man. It is concluded that - under certain precautions - the nephrotoxicity of drugs can be evaluated by renal enzyme excretion studies.

Analgesics

Investigations on the action of cefazedone and gentamicin, alone and combined, on renal enzyme excretion in rats.

Administration of (6R, 7 R)-7-(2-[3,5-dichloro-4-oxo-1(4 H)-pyridyl]-acetamido)-3-([(5-methyl-1,3,4-thiadiazol-2-yl)-thio]methyl)-8-oxo-5-thia-azabicyclo[4,2,0]oct-2-ene-2-carboxylic acid (cefazedone, Refosporen) in a dose of 1000 mg/kg led to a statistically significant rise of the alkaline phosphatase activity in rat urine. The excretion of leucinaminopeptidase activity was not significantly affected. The enzyme activity in the animals' urine was raised by the administration of 100 mg/kg of gentamicin. Combined administration of the two antibiotics in the doses mentioned did not lead to any additive action. The established increases in activity were in the same range as after the administration of only one of the antibiotics.

Alkaline Phosphatase

D-penicillamine and enzymatic activities.

The influence of incubation with D-penicillamine on pure enzyme preparations and on enzymatic activities of serum and skin homogenates was investigated. Three of the nine enzymatic activities studied underwent significant changes. Such effects of D-penicillamine must be taken into consideration if therapeutic or unwanted actions of this drug are to be fully understood; they are elicited by concentrations reached under conditions used for human therapy.

3',5'-Cyclic-AMP Phosphodiesterases

[Allergy and anaphylaxis].

Acute undesirable drug reactions following the application of one of the modern amiglycoside antibiotics only occur in some exceptional cases. Those compounds reveal low sensitizing capacity, in contrast to streptomycin. Following topical applications, allergies of delayed type have been recorded, especially in the case of neomycin, a widely used topical antibiotic. Aminoglycosides exhibit anaphylactoid activity; they cause a degranulation of mast cells. Via this mechanism, histamine reactions are provoked. Such "anaphylactoid" reactions are clinically indistinguishable from true allergic (anaphylactic) phenomena and repeatedly have been erroneously interpreted. For allergological test series as well as for the correct interpretation of acute unwanted drug effects it is important to consider the anaphylactoid activity of the aminoglycosides.

[Comparative studies on the nephrotoxicity of aminoglycoside antibiotics using renal enzyme elimination in rats as a model].

Determinations of renal enzyme excretion in rats inform on the relative nephrotoxicity of drugs. The data obtained in such studies are of clinical relevance. For the screening of new aminoglycosides, the renal enzyme excretion technique is a rather simple, but valuable model. New compounds can easily be compared to well-known ones about which clinical cata are available already.--Comparative investigations with netilmicin, a semi-synthetic sisomicin derivative, and with gentamicin are reported. Netilmicin in three different doses provoked significantly less increase in urinary alkaline phosphatase and "leucine aminopeptidase" activities.

Aminoglycosides

[Specific and unspecific immune stimulation in recurrent herpes simplex].

According to the present state of our knowledge the clinical picture of the recurrent herpes simplex and the recurrent stomatitis aphthosa is due to a weakness of the immune defense. It cannot be decided yet if it is but a quantitative weakness or if it is a special form of disturbance in the permeation, which allows the virus to remain alive and to reactivate after a short period of time. Injection of a specific herpes simplex vaccine (HVH-1 or HVH-2 strain) or administration of Levamisol are at present the most promising ways of treatment. Both kinds of treatment are well tolerated, they give the same results in herpes simplex recidivans labialis, and whilst in herpes simplex recidivans genitalis the vaccine should be preferred, Levamisol should be given preference in stomatitis aphthosa on account of its broad (less specific) range of action.

[Incorporation of radioactive calcium and technetium pyrophosphate in to bones].

A method is given for simultaneous in vivo measurement of mineral and collagen metabolism of the bone. Accretion rates of 47-calcium as a chlorid into the bone are representative for bone mineralisation and of 99m-technetium labelled pyrophosphate for collagen metabolism. Compartmental models used for analysis are presented. Bone accretion rates are given in mg, or mMol respectively per unit time for the skeleton, by computation of the specific activity of 47-calcium and 99m-Technetium pyrophosphate and rational bone accretion as registered by total body profile scanning technique. The results obtained in five normal volunteers are presented.
