[Blood access puncture point pseudoaneurysms in two hemodialysis patients].

This is a report of blood access puncture point pseudoaneurysms which occurred in two hemodialysis patients. Case 1: A 58-year-old male had been undergoing hemodialysis treatment since June, 1975. In January, 1981 a subcutaneous mass had developed at the blood access puncture point above the previously superficialized left femoral artery. An operation was performed in February, 1981 and the mass was dissected. The same artery has been used since the operation for blood access without any problems. The dimensions of the egg-shaped dissected mass were 3.5 X 4 X 2.5 cm. A histological diagnosis of the wall of the mass showed that it was a pseudoaneurysm. Case 2: A 48-year-old female had been undergoing hemodialysis treatment since September, 1983. The left basilic vein, connected to the brachial artery, has been used for blood access. In April, 1984, a subcutaneous mass had developed at the blood access puncture point and an operation was performed within a few days. Operative findings revealed that the mass was a capsulized infected hematoma with a smooth but extremely thin and easily ruptured surface, and the section of the basilic vein surrounded by the mass showed evidence of necrotic change due to compression. The brachial artery was resutured at the region where it was previously connected to the basilic vein without disturbance of arterial blood flow. In July, 1985, an operation was performed in which a new blood access was constructed in the left thigh by superficializing the femoral artery and connecting its side to the end of the saphenous vein.(ABSTRACT TRUNCATED AT 250 WORDS)

Pharmacological studies on the anti-inflammatory action of phenolic compounds.

The mechanism of the anti-inflammatory action of phenolic compounds was examined using neutrophil chemotaxis. Chemotactic activity of guinea pig peritoneal neutrophils to N-formylmethionyl-leucylphenylalanine (FMLP) was suppressed in a concentration-dependent manner. The order of drug potency in inhibiting the neutrophil chemotaxis was eugenol much greater than thymol greater than guaiacol much greater than phenol. The concentrations of phenolic compounds used in these experiments did not induce lactate dehydrogenase (LDH) release and did not affect neutrophil viability. There was a consistent positive relation between the ID50 of superoxide anion generation in neutrophils and the inhibitory dose for neutrophil chemotaxis by phenolic compounds. A free phenolic hydroxyl group is essential for scavenging oxygen free-radicals and is also essential for inhibiting leukocyte chemotaxis, as was demonstrated in these experiments. These findings suggest that inhibition of leukocyte chemotaxis may be involved in the anti-inflammatory action of phenolic compounds, and that one of the anti-inflammatory actions of phenolic compounds is the prevention of the production of oxygen free-radicals by leukocytes.

The effect of calcium antagonists on the activation of guinea pig neutrophils.

The role of calcium ions in the activation of guinea pig neutrophil functions was examined by evaluating the effects of calcium antagonists. The data presented here show that calcium antagonists inhibit the activation of guinea pig neutrophil functions elicited by N-formyl-methionyl-leucyl-phenylalanine (FMLP) such as chemotaxis, superoxide anion generation and granule enzyme release in a concentration-dependent manner. The concentrations of calcium antagonists demonstrating the inhibition of neutrophil functions may be somewhat different for each function and higher than that of smooth muscle cells. Calcium ionophore A23187 (A23187) caused superoxide anion generation and granule enzyme release of the neutrophils. A23187 also potentiated the FMLP-induced superoxide anion generation and granule enzyme release of the neutrophils. On the contrary, A23187 neither elicited neutrophil chemotaxis nor affected FMLP-induced neutrophil chemotaxis. These results indicate the possibility that the inhibitory effect of calcium antagonists on the activation of neutrophil functions is probably not simply mediated by inhibition of calcium uptake but also by inhibition of a calcium-dependent intracellular target.

Microwave tissue coagulation applied clinically in endoscopic surgery.

On the basis of the ample basic knowledge acquired by repeated experiments, we applied our microwave tissue coagulator in endoscopic surgery, in a total of 59 patients with benign and malignant lesions encountered during a period of two years beginning in July, 1981. Hemostasis was achieved in 96.5% of all the cases with bleeding lesions in the digestive tract. Stenosis was alleviated in 86% of the cases with esophageal or rectal stenosis. Furthermore, the technique was successfully used for hemostasis and tumor reduction in inoperable early cancer cases. Our device is unique in that the electrode is thrust into tissue, which assures us of a satisfactory result. In this point, it must be clearly distinguished from an electrocoagulator or a laser coagulator, which is associated with a risk of injuring intact tissue. The range of coagulation is adjustable by changing the length of the monopolar antenna and electric output, and by employing a coaxial cable of appropriate thickness. In conclusion, our microwave tissue coagulator can be used easily and safely in clinical endoscopic surgery.
