[Flatulence].

Abstract

At present physicians focus their medicine studies in well defined illnesses as peptic ulcer, gastric cancer, ulcerative colitis and so on. However, patients reveal their discomfort to us, that is their symptoms or group of symptoms (syndromes). For this reason, our concern for many years has been the study of symptoms and syndromes. In this review we will be looking at the concepts and information gathered with respect to intestinal gases, clinically known as flatulence.

Peritoneal tuberculosis--an uncommon disease that may deceive the gynecologist.

OBJECTIVES: To document women with peritoneal tuberculosis mimicking ovarian malignancy and to review pertinent literature. STUDY DESIGN: The records of women with peritoneal tuberculosis who were managed at the Soroka Medical Center, Beer-Sheva, Israel between January 2000 and December 2001 were reviewed. RESULTS: Four patients with peritoneal tuberculosis mimicking ovarian malignancy were encountered. Two presented with the classical symptomatology of advanced-stage ovarian carcinoma including ascites, abdominopelvic masses and elevated serum CA-125, and two presented with lower abdominal pain and adnexal mass. Laparoscopy in one patient and laparotomy in three patients revealed peritoneal tuberculosis and no malignancy. Of the three patients who had laparotomy, two underwent unnecessary extended surgery including total hysterectomy, bilateral salpingo-oophorectomy, omentectomy and bilateral pelvic lymphadenectomy, and one had conservative surgery including unilateral salpingo-oophorectomy. All patients were postoperatively treated with quadruple anti-tuberculosis chemotherapy. CONCLUSIONS: Medical awareness of peritoneal tuberculosis is still lacking and many women with this disease are initially thought to have ovarian malignancy and undergo unnecessary extended surgery. Laparoscopy including biopsies seems to be a sufficient and safe method to provide diagnosis of peritoneal tuberculosis. If laparoscopy is not feasible, laparotomy should be performed. If no malignancy is detected and the diagnosis of peritoneal tuberculosis is confirmed, unnecessary extended surgery is avoided and anti-tuberculosis treatment is started.

Mesenteric lymphadenopathy in children examined by US for chronic and/or recurrent abdominal pain.

BACKGROUND: Children with recurrent abdominal pain often undergo US to confirm or exclude organic disease. OBJECTIVE: To assess the prevalence of mesenteric lymphadenopathy on US in these children. MATERIALS AND METHODS: We prospectively studied 189 children with recurrent abdominal pain with US of the abdomen, using graded compression. The results were compared with 73 children in a control group. The children in both groups were divided into three age groups. The size, number, morphology and location of mesenteric lymph nodes were noted, as well as additional findings. Pediatricians followed the patients from 3 months to 1 year, and a repeat US study was done in 30 children. RESULTS: Mesenteric lymphadenopathy was present in 116 of 189 children (61.4%), with the greatest prevalence in boys in the younger age groups. The location of the nodes was mainly in the right lower quadrant. In the control group, 7 of 73 children had mesenteric lymphadenopathy, a significantly lower prevalence than in the study group ( P<0.001). Additional findings, apart from lymphadenopathy, were present in 27 (14.2%) of the 189 children in the study group, and in 5 (6.8%) of the 73 children in the control group. CONCLUSION: Mesenteric lymphadenopathy is a common, and often the only abnormal, finding on US in children with recurrent abdominal pain.

[Helicobacter pylori infection in children and juveniles: an epidemiological study on prevalence, socio-economic factors and symptoms].

BACKGROUND AND OBJECTIVE: In view of numerous sequelae of a Helicobacter pylori (H.p.) infection and sparse epidemiological data on the infectious status of children and juveniles, a cross-sectional study was undertaken in schools of the Aschaffenburg area. Data were recorded on the number of H.p. infections in pupils aged 14-20 years, the possible influence of various socio-economic factors, and a possible association of the infection with upper abdominal symptoms. SUBJECTS AND METHODS: In 540 pupils of grade 2 (aged 7-9 years: primary school, n = 183) grade 7 (aged 11-15: primary and secondary schools, n = 177) and grade 12 (aged 16-20 years: vocational or grammar school: n = 180), the presence of H.p. was elucidated with the 13C-urea breath test. An anonymous questionnaire was used to ascertain socio-economic status and any symptoms. RESULTS: In 51 of the 540 pupils (9.4%) the test for H.p. was positive. The infectious state increased with age, but there was no significant difference between the various age groups. Ethnically German pupils were less often infected (7.1%) than those of foreign origin (28.2%). The number of persons in a household correlated with the H.p. status. Upper abdominal symptoms were more frequently reported by infected children and juveniles and were an independent risk factor for H.p. infection. CONCLUSION: This study highlights the importance of socio-economic factors and the possible association of upper abdominal pain with H.p infection.
