Athletic activity after total joint replacement.

Abstract

Total hip and knee replacement surgery is a successful treatment for the arthritic hip and knee ensuring proven pain relief and return of function. Younger, more active patients who have greater expectations and higher demands are receiving hip and knee replacements. With current surgical techniques and implant designs, athletic participation should be limited to low impact, low demand, and low duration activity. Future advancesin bearing surfaces may allow for higher demand activities.

Can cervical cancer screening be stopped at 50? The prevalence of HPV in elderly women.

Although the relation between cervical cancer and the human papillomavirus (HPV) has been established beyond doubt, the introduction of HPV detection in cervical cancer screening is halted, primarily by the high rate of false positivity in relation to morbidity, since the majority of women infected with HPV will not develop lesions. To counteract overconsumption of cervical cancer screening in elderly women, we wanted to test the hypothesis that women of 50 years or older who are HPV-negative and have a cytologically normal smear might be encouraged to refrain from further screening. As a first step, the prevalence of high-risk HPV in a population of 1,936 women of 50 years and older was investigated. After an initial decline, a slightly higher prevalence can be seen with increasing age. There is a decrease in the prevalence of multiple infections with age, paralleled by an increase in single infections, especially of HPV type 16 in the eldest-age group. However, neither the decrease in multiple infections nor the increase in single infections is statistically significant. The data obtained in this study show that, even in the presence of a slight increase in the HPV prevalence in elderly women, approximately 94% of the elderly women can be withdrawn from the cervical cancer screening. However, a follow-up study will be necessary to determine the frequency of (re)infection as well as the course of an HPV infection in elderly women.

Gender- and age-specific impairment of rat performance in the Morris water maze following prenatal exposure to an MRI magnetic field.

We examined the effects of prenatal exposure (40 min/day, gestation days 12-18) of rats to a magnetic resonance imaging (MRI) magnetic field (MF) on their performance in the Morris water maze. At 2 months of age, female rats showed impaired performance. The animals spent longer time swimming and used inefficient strategies. However, no significant effects on maze performance were observed at 1 and 5 months of age. No evident maze performance deficit was detected in male rats prenatally exposed to the magnetic field. Thus, we conclude that prenatal exposure to MRI magnetic field induces cognitive/behavioral deficits in female rats at a specific age.

Non-treatment-related chronic myeloid leukemia as a second malignancy.

The characteristics of the very rare non-treatment-related chronic myeloid leukemia (nTr-CML) cases have never before been analyzed. The literature up to December 2002 was screened using the Medline database to identify cases of Tr-CML and nTr-CML. We considered five cases with nTr-CML identified among 270 newly diagnosed CML at our Department. Our report thus considers nine cases with nTr-CML compared to 77 affected by Tr-CML as a secondary neoplasm. The median age at the appearance of the first tumor was higher in nTr-CML patients compared to that of the Tr-CML group (P<0.0001). The median age at CML diagnosis was significantly higher in the nTr-CML than in the Tr-CML group (P<0.0001). The proportion of hematological malignancies as first tumor type was not different in the two groups (44% in nTr-CML versus 56% in Tr-CML). Our study underlines that nTr-CML as a second malignancy is a rare entity associated with elderly age.
