Considerations on ceramic prosthesis explants.

Abstract

The authors report data obtained for 11 cases of explants of prostheses having ceramic joint surfaces. The wear phenomena observed in 9 prostheses, including 1 case with breakage of the ceramic head, was moderate: 20-22,000 debris per mu 2. In 2 cases the wear was massive, with a concentration of debris in the peri-implant tissues 5-10 times greater (100,000-218,000 debris per mu 2). An explanation for the trigger mechanism of wear phenomena is proposed: the cases of massive wear are related to loosening of the acetabulum. A histological examination of the bone tissue surrounding a stem covered with alumina (Al2O3) carried out in three cases allowed us to observe an area of demineralization measuring approximately 500 mu of tissue in contact with the ceramic coating. The osteomalacia could be caused by the diffusion of alumina ions.

A finite element study of the initiation of failure of fixation in cemented femoral total hip components.

In order to study initial mechanisms of failure in cemented femoral total hip components, an anatomically accurate three-dimensional linear finite element model was constructed and verified against experimental strain measurements in the cement mantle. Good agreement was found between predicted and measured strains. The likelihood of failure initiation due to cement-prosthesis debonding and crack initiation at voids was studied for loading conditions simulating both one-legged stance and stair climbing. The "out of plane" forces involved in stair climbing appear to be the greatest threat to the fixation of total hip replacements. In stair climbing, cement-prosthesis debonding and pore crack initiation were probable in the proximal anteromedial region of the cement mantle, and near the distal tip of the implant. The proximal stresses in stair climbing were higher than the distal stresses in either stair climbing or one-legged stance.

The effect of cement fixation on initial micromotion of the femoral component in condylar knee replacement.

Four fresh, cadaveric specimens were fitted with total knee replacements and mounted on a knee joint loading and measuring rig. Femoral component micromotion in the sagittal plane was measured with a transducer during flexion and extension of the specimen. Various condylar designs were used without and with cement. Movement followed a characteristic curve pattern and was less with cement fixation particularly for bone of low trabecular strength as determined by bone sample tests for stiffness. Cement fixation appears to be superior to cementless fixation.
