The arrangement and structure of the fascial layers of the scalp were studied in 48 red neoprene latex-injected cadavers with the aid of an operating microscope. The galea aponeurotica was continuous with a superficial temporal fascia. Deep to the galea, the subaponeurotic connective tissue was bilaminar. There was an outer, vascular, areolar layer, and an inner, avascular, membranous layer. Underlying the temporalis muscle, the pericranium was thinner and more adherent than elsewhere with no subpericranial tissue.
The authors evaluated the results obtained from the tympanic membrane reconstruction in 300 patients affected by simple, purulent or cholesteatomatous chronic middle ear otitis. In this tympanic membrane reconstruction they used: jugular vein strip of a calf and temporal muscle fascia in 150 cases. On pathological basis (simple, purulent and cholestéatomatous otitis) they used the overlay or the underlay method. In 5 years that follows, the materials (vein and fascia), if properly used, would give similar results. For such reasons the authors proposed: temporal muscle fascia autotransplantation with underlay method in partial myringoplasty; heterotransplantation of modelled jugular vein strip with overlay method in subtotal or total myringoplasty.
Urinary incontinence is a debilitating problem that affects over 20 million people in the United States. Anatomical stress incontinence in women has an excellent continence outcome following urethral suspension procedures. However, intrinsic urethral incontinence has a high failure rate following urethral suspension procedures and is usually managed with a pubovaginal sling procedure. Eight patients with both anatomical and intrinsic urethral incontinence were managed with a combined urethral suspension procedure and pubovaginal sling procedure. Seven patients remain continent while one continues to have intrinsic urethral incontinence. The combined urethral suspension procedure and pubovaginal sling procedure appears to provide a treatment option for patients with coexisting anatomical and intrinsic urethral incontinence.
