Glaucoma of the aphakic patient is often difficult to treat. The authors present a new surgical technique which uses a portion of the trabeculum as a drain placed in the anterior chamber. Thirty eyes (19 aphakic and 11 pseudophakic) in 28 patients presenting refractory glaucoma were operated on by the same surgeon. Sixteen eyes had already undergone glaucoma surgery, 17 presented vitreous in the anterior chamber. The average postoperative follow-up was 18 months. Eighteen eyes (60%) needed no further treatment, 6 eyes (20%) were stabilized with a local treatment, 6 eyes (20%) remained hypertonic. This new surgical technique, which uses a portion of the trabeculum as a drain, allows for long-lasting filtration in the aphakic or pseudophakic patient with refractory glaucoma.
PURPOSE: The purpose of our study was to assess the long-term tolerance of aphakic contact lenses in an elderly male population. METHODS: We conducted a follow-up study of Bausch & Lomb CW 79 aphakic extended wear contact lenses fit since 1982. A total of 109 patients (115 eyes) were identified. Records of 37 patients (43 eyes) were available to determine the course of contact lens wear. RESULTS: Thirty-six eyes were contact lens dropouts. The average time to failure was 22 months. Seventeen eyes from the failure group elected to receive secondary intraocular lenses (IOLs). Seven eyes were still wearing CW 79 contact lenses at the conclusion of the study. The average length of wear in these seven eyes was 91 months. CONCLUSIONS: We concluded that the CW 79 aphakic extended wear contact lenses were successful in selected cases. However, a high failure rate was found on long-term follow-up, and approximately half of the failure were converted to secondary IOLs.
We describe a new method for placing transscleral sutures when fixating posterior chamber intraocular lenses to the sulcus. An intraocular microendoscope with an 18 gauge probe is used for direct sulcus observation and needle position assessment. The straight needle of a 10-0 polypropylene suture and the tip of the probe are placed in a 16 gauge silicone rubber tube to hold them together. Fixing the needle to the endoscope allows a direct view of its tip and requires only one hand. The other hand is used to grasp the tip of the needle when it comes out under the scleral flap after passing through the sulcus. Assessment of needle position with an endoscope avoids surgically induced iris root or ciliary body damage. Fixing the needle to the endoscope simplifies the surgical technique.
