Electrocardiograms of 19 consecutive patients with acute myocardial infarction complicated by early ventricular fibrillation were compared with those in 19 case-matched patients with acute myocardial infarction not complicated by ventricular fibrillation. The mean precordial QTc interval dispersion in patients with ventricular fibrillation was greater than that of patients without ventricular fibrillation (73 +/- 28 ms vs 30 +/- 12 ms, p < 0.001).
Artificial neural networks can be used to recognize lead reversals in the 12-lead electrocardiogram at very high specificity, and the sensitivity is much higher than that of a conventional interpretation program. The neural networks developed in this and an earlier study for detection of lead reversals, in combination with an algorithm for the right arm/right foot lead reversal, would recognize approximately 75% of lead reversals encountered in clinical practice.
Pericarditis has not been well associated with exercise test induced ST-elevations. This case report of a pericarditis patient who underwent exercise stress testing and other similar cases found in the literature suggest that this is an unrecognized clinical manifestation of pericarditis.
