Systemic sclerosis terminating as systemic necrotizing angiitis.

Abstract

We report a 57-year-old woman with systemic sclerosis (SSc) who died suddenly, following a haemoptysis. At post-mortem, systemic necrotizing angiitis of small vessels was observed in several organs. Necrotizing angiitis has been reported as a rare complication of systemic sclerosis, and is usually lethal.

A man with vague rheumatic complaints.

A 52-year-old man presented with a four-month history of malaise, low-grade fever, decreased appetite, and a 20-pound weight loss. He complained of joint pain and swelling, proximal muscle weakness, exertional dyspnea, and a dry cough. He also noted that his fingers had turned white and then blue when chilled and red when rewarmed. He had not had pleuritic chest pain, dysphagia, dry eyes or mouth, rash, or skin photosensitivity.

[Mortality and risk of cancer in systemic connective tissue diseases].

The systemic connective tissue diseases represent a broad spectrum of multiorgan disorders. This report reviews our current knowledge of mortality and the risk of cancer in these diseases. The relationship between clinical manifestations and reduced survival in systemic sclerosis, systemic lupus erythematosus and polyarteritis nodosa is addressed in detail. Moreover, we outline the increased risks of cancer in dermatomyositis and lymphoma in primary Sjögren's syndrome, as well as cancer of the bladder in patients with Wegener's granulomatosis who have been treated with cyclophosphamide. Finally, the conflicting results regarding mortality in temporal arteritis are discussed.
