Circular stereotactic callosotomy: a preliminary report. Technical note.

Abstract

The authors propose a new method for performing stereotactic callosotomy, which they have named circular callosotomy. The operating device is the original Riechert-Mundinger's string electrode, which can be protruded through a side window and by rotating the probe it is possible to cut the commisural pathways to the extent required. The anatomical results of the operation can be checked using MRI scanning.

Gender effects on callosal thickness in scaled and unscaled space.

Some empirical data suggest that sexual dimorphisms in callosal morphology exist, but findings are not consistently replicated across laboratories. We applied novel computational surface-based methods to encode callosal thickness at high spatial resolution. We further examined whether callosal thickness and related gender effects are influenced by brain size adjustments achieved through data scaling. Significant gender differences were absent in scaled data, and women showed no regional thickness increases compared with men (in either scaled or unscaled data). In unscaled data, men exhibited significantly greater callosal thickness in a number of regions that may be attributable to larger brain dimensions in men. Alternatively, given their regional specificity, the observed differences in unscaled callosal thickness may contribute to gender-specific cognition and behavior.

Corpus callosum hematoma secondary to isolated inferior sagittal sinus thrombosis.

A 45-year-old female was admitted with headache and vomiting. Cranial computed tomography (CT) demonstrated a callosal hematoma. Magnetic resonance imaging (MRI) showed no venous flow and thrombus replacing the inferior sagittal sinus (ISS) lumen. Under appropriate medical treatment and close follow-up she recovered quickly and, after 2 years, was doing well with corpus callosum infarcts. Isolated inferior sagittal sinus thrombosis is an extremely rare condition with only one previously reported case in the literature. Although it is very rare, isolated inferior sagittal sinus thrombosis should be considered in the differential diagnosis of non-traumatic corpus callosum hematoma.

Core hypothermia in multiple sclerosis: case report with magnetic resonance imaging localization of a thalamic lesion.

Hypothermia is a rare condition in multiple sclerosis (MS). We report on a patient with a long-standing secondary progressive MS and six episodes of recurring hypothermia down to 29.9 degrees C with associated hypotension, bradycardia, coagulopathy and electrolyte dysequilibrium. Magnetic resonance imaging (MRI) demonstrated severe involvement of the corpus callosum with an associated lesion in the right posterior thalamus. These findings may link hypothermia in MS with callosal and associated thalamic pathology to Shapiro's syndrome, where agenesis of the corpus callosum and associated abnormalities are related to episodic spontaneous hypothermia. In MS, hypothermic episodes may be triggered by preceding infections, as shown in the present case.
