Renal function changes in diabetes.

Abstract

The present studies show that a number of functional abnormalities are present in the kidney in early diabetes, especially during poor regulation. Normalization of these functions and changes in renal and glomerular size are found during complete diabetes control. The results further support the idea that great efforts should be made to maintain the best possible control situation in diabetic patients. Our studies also indicate that antihypertensive therapy may be beneficial for young, proteinuric diabetics and may postpone the stage of renal insufficiency.

[Circadian rhythm of blood pressure in hypertension treated with enalapril and other drugs].

Twenty-four-hour "ambulatory blood pressure monitoring" (ABPM) was performed during the day at every 30 min and during the night at every 60 min in 38 persons admitted to the hospital. The subjects were divided into four groups: Group 1. healthy subjects (11); Group 2. essential hypertensive patients (7) before and during enalapril treatment; Group 3. patients with essential hypertension (10) treated with different antihypertensive drugs and Group 4. renal patients treated as in Group 3. Normal circadian rhythm was found in the healthy subjects and in the patients with essential hypertension, but no rhythm could be demonstrated in the renal patients. The high blood pressure decreased in response to enalapril in 7 patients without any decrease in the circadian rhythm. It was concluded that antihypertensive therapy does not abolish the circadian rhythm if there is any--and does not restore if it is lacking. The chances of the diagnostic use of circadian blood pressure rhythm are not impaired by the antihypertensive treatment.

Comparison of high-performance liquid chromatography and high-performance capillary electrophoresis for the determination of cicletanine in plasma.

A sensitive and selective high-performance capillary electrophoresis (HPCE) procedure was developed for the determination of total cicletanine in human plasma. The procedure consisted in extraction of the drug with diethyl ether and analysis by micellar electrokinetic capillary chromatography in a fused-silica capillary using sodium dodecyl sulphate in the run buffers and ultraviolet detection. The concentrations of cicletanine obtained by this method were compared with those obtained by a high-performance liquid chromatographic (HPLC) method used routinely. The within-run precision of the methods, expressed as relative standard deviation, ranged from 1.6 to 7.8% for HPLC and from 6.4 to 11.1% for HPCE. Both methods showed an adequate level of accuracy; the relative errors ranged from 0.02 to 3.25% for HPLC and from 0.21 to 2.90% for HPCE. The HPCE method required less than half the time taken by the HPLC method, making HPCE a useful alternative technique for the routine determination of cicletanine in plasma. Both methods were used to follow the time course of total cicletanine in human plasma after a single oral therapeutic dose of the drug.
