Ischemic oculopathy.

Abstract

A 58-year-old woman had unilateral visual loss, anterior uveitis, rubeosis, and glaucoma. She subsequently was found to have a complete occlusion of the internal carotid on the same side. The clinician is reminded to consider a diagnosis of carotid insufficiency in elderly patients with anterior uveitis or rubeosis of obscure origin.

[Medical treatment of arteriosclerosis obliterans].

The author gives an overview on former and recent ways of medical therapy in peripheral arterial occlusive disease. The importance of possible elimination of the risk factors is emphasized. The author summarizes the mechanism and clinical effects of drugs affecting vasodynamic as well as thrombolytic processes. The conclusion of the review indicates that although presently there is no ideal therapy for the disease, yet in a remarkable part of cases a marked clinical improvement and inhibition of the basic pathologic processes can be produced even by the available preventive and therapeutical methods.

[Reflex dystrophy after reconstruction of the axillary artery].

Reflex sympathetic dystrophy (RSD) is a complex syndrome of pain, trophic changes and vasomotor instability affecting the limbs. Numerous theories have been suggested to explain the pathophysiology. None is universely accepted. In most of the patients reported, an antecedant event such as trauma or surgery is implicated in the initiation of symptoms. We describe a case of reflex sympathetic dystrophy developing after reconstruction of a. axillaris. To our knowledge there have only been a few previous descriptions of reflex dystrophy following vascular surgery. The mechanism of reflex sympathetic dystrophy secondary to vascular trauma is discussed. Presumely, damage to the arterial wall by trauma may initiate abnormal reflex activity and lead to RSD. We suggest that vascular surgery should be considered as a potential risk factor for the development of RSD.
